Dear Mr. Garness:
On July 21, 2009, the Department received the record drawings and the construction
documentation for the construction of an onlot wastewater treatment system with a new
2,500 gallon two compartment septic tank and a new soil absorption system for an 8
bedroom assisting living facility at the above location. In addition, the existing onsite
wastewater system was decommissioned properly.
The new 2500 gallon septic tank was installed per plan and will pump into an onsite
soil absorption system which has three 40 foot long wide trenches for disposal. The
trenches have an effective depth of 4 feet and are 5 feet wide; this design allows for a
reduction factor of 0.5 to be applied. The disposal system has an area of 960 ft2. The
soil is well graded gravel with sand and cobble. A percolation rate of slightly slower
than 1 minute per inch was measured in the area of the field. Use of an application
rate of 1gallon per ft2 was used with a reduction factor of 0.5 to determine the required
infiltrative area to be 600 ft2.
The Department has reviewed the record drawings and finds the construction with
some changes to the collection system shall operate per the original approved plan
therefore in accordance with Wastewater Regulations 18 AAC 72.240 final construction
approval for the proposed system is hereby issued. A "Certificate to Construct" for the
community wastewater system is enclosed. Please use the referenced plan number in
correspondence regarding this system.
This approval does not imply the granting of additional authorizations nor obligate any
state, federal or local regulatory body to grant required authorizations.
Any person who disagrees with this decision may request an adjudicatory hearing in
accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division
Director in accordance with 18 AAC 15.185. Informal review requests must be
delivered to the Division Director, 555 Cordova Street, Anchorage, Alaska 99501, within
15 days of receiving the decision. Adjudicatory hearings requests must be delivered
to the Commissioner of the Department of Environmental Conservation, 555 Cordova
Street, Anchorage, Alaska 99501, within 30 days of the decision. If a hearing is not
requested within 30 days, the right to appeal is waived.

Dear Mr. Beattie:
An On -Site Water/Wastewater Permit, number SW030487, issued by this office for a single-family
system, will expire on November 19, 2004. The permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $115.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $460.00 for a wastewater permit and $175.00 for a well
permit.

Septic Upgrade Permit
Gentlemen:
My firm was contacted to investigate the possibility of installing of a new system at the
above referenced property. Currently the lot is developed with an undocumented
system.. The house was constructed in approximately 1970, so I am assuming the septic
system and well was installed about the same time. The septic system is in failure and
backing up into the house. The well was abandoned in September of 2003 when the
house was connected to AWWU water. The house cannot be connected to AWWU sewer
system due to the trunk lines being located greater that 200 feet from this property. A
single test hole was excavated on the property on October 25, 2003 and the log is
attached. Ground water was checked on November 10, 2003. No water was found at the
time of excavation or during the monitoring period. Bedrock was not encountered in the
test hole.
Lot 20 is 10,058 sf in size. The owner also owns lot 19, to the east. Lot 20 slopes to the
southwest at a rate of approximately 1 percent. The proposed installation will be located
in southern portion of the lot. The proposed location will be greater than 100 feet away
from the existing surrounding wells and 25 feet from the water service lines. The
surrounding systems are located greater than 200 feet from the proposed installation.
The lots surrounding this lot are undeveloped, and do not conflict with the proposed
installation. The proposed installation will not impact the future development of the
surrounding or existing lots. See the attached design. The new system will maintain
over six feet vertical separation to the bedrock and over four feet vertically to the ground
water.
