Ref: Proposed Well and Septic System for Lot 3, Block 1, Sleepy Hollow Subdivision
To whom it may concern:
The proposed 3 bedroom house will be served by a private well and septic system. Two test
holes were excavated on the property. The septic system will be designed around the 30 foot
radii of these test holes. We are proposing that a new 1000 gallon septic tank and a deep trench
type drainfield be installed. The well is to be drilled in the area shown on the attached site plan
and design drawings. Comments regarding the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that the insitu sandy soils will act as a sand filter
and that an application rate of 1.2 gallons/day/ftZ should be used.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Proposed Application Rate: 1.2 gallons/day/flZ
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 375 ftZ
f. Total Depth: 12 feet (max.)
g. Effective Depth: 8 feet
h. Width: 2.5 feet
i. Reduction Factor: N/A
j. Minimum Length: 25 feet total length
k. Effective absorption area = 400 ftZ
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.

4. TOPOGRAPHY: As can be seen on the design, the average topography in the area of the
proposed drainfield is a 5 to 20 percent slope running from approximately northeast to
southwest. In short, there are no slope concerns.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

Certificate of On -Site Systems Approval # OSC141613
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 1, Lot 3 of
Sleepy Hollow #1 Subdivision. This inspection revealed an arsenic
concentration of 13.6 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On -Site Water and Wastewater Program
website (www.muni.or /onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Certificate of On -Site Systems Approval (COSA) # OSC141613
During a recent COSA on-site inspection and test of the potable water
supply well on Block 1, Lot 3 of Sleep Hollow 41 subdivision, the well's
productivity was determined to be 0.41 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 3 -bedroom
residence is 0.33 gallons per minute. Although the subject well currently
exceeds this minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

We are requesting your department issue a 5 foot lot line waiver from the existing drainfield to the
property line to the south. The neighboring property (Sleepy Hollow # 1; Block 1, Lot 4) is undeveloped
(no water or wastewater utilities) and is also owned by the same owner as Lot 3 (Jon Carpenter). Since
the both properties are owned by the same owner we hope that the 7 day notification of the lot line
waiver be circumvented. In short, there is minimal risk in approving this waiver request.
We are unaware of any adverse impacts this waiver would have on adjacent wells or septic systems.
