Ref: Proposed Septic Upgrade for The Villages Tideview Subdivision; Lot 9
To whom it may concern:
The existing 4 bedroom house is served by a private well and septic system. The septic system is
currently in the state of failure and needs to be upgraded. A test hole was excavated north of the
existing drainfield. The drainfield will be designed around the 30 foot radius of this test hole.
Due to the presence of shallow bedrock and steep slopes over the majority of the property, we are
proposing that a 1500 Advantex treatment system and two 5 -wide trench type drainfields be
installed. One test hole was excavated on the property. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that an application rate of 3.0 gallons/day/ft'
should be used for the Advantex treatment system.
2. TRENCH DESIGN:
a. Percolation Rate: 17.1 minutes/inch
b. Proposed Application Rate: 4.0 gallons/day/ft'
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 150 ft'
f. Total Depth: 3 feet (maximum)
g. Effective Depth: 0.5 feet (minimum)
h. M.O.A. Approved Sand Filter: 1 foot
i. Width: 5 feet
j. Minimum Length: 30 feet total length (2 @ 15 feet long each)

3. SURFACE WATERS: There is a man made pond over 100 feet from the proposed
drainfields that is leaking from the outlet creating a puddle just to the north of the pond less than
100 feet from the drainfields. We are proposing to fill and grade this area so that no ponding
occurs within 100 feet from proposed septic system.
4. TOPOGRAPIIY: As can be seen on the attached topography site plan the average slope in
the area is approximately 15 to 20 percent running northeast to southwest. During our site visits
by our field technician, Susan Oswalt, it was noted that the lot has been regraded since the aerial
topography map was performed. It appears that during the original installation of the septic
system, the area was graded so that the slope were decreased from a 25 to 30 percent slope to a 5
to 10 percent slope running east to west in the area of the proposed drainfield. To the west of the
proposed drainfield is a slope greater than 25 percent. At a 25 percent slope 2 feet above the
proposed drainfield, the slope line does not daylight within 35 feet of the downhill drainfield. We
request a 35 foot waiver to the slope greater than 25 percent. The area is heavily vegetated and
given the porosity of the soils, it is our opinion that there is minimal risk with the granting of this
waiver.
5. WAIVER REQUEST: Due to the limited space on the property, we request a 5 foot lot line
waiver from the north lot line to the drainfield. We are unaware of any adverse impact on the
neighboring property with the granting of this waiver.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an Independent professional civil
engineer registered In the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
Investigation, based on procedures outlined In the Health Authority Approval Guidelines for this application,
shows that the onsite water supply and/or wastewater disposal system is(are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
Information obtained from the Municipality of Anchorage riles and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations In effect at the time of Installation.

Engineer's Comments:
In conducting this evaluation, GEG, Ltd. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines Q Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwaterlevels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd, can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
otherperson orpartyIs not authorized, nor will It confer any legal right whatsoever.
