The gravel depth is the minimum depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection.and approval by this departmen
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 fee
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

Subject: Mountain Park Estates #1/Block 6 -- Lot 6
The above referenced lot consists of a four (4) bedroom single
family residence with an individual well and on—site sewage
disposal system. The well was drilled on February 28, 1976.
Total depth of well is 200 feet and cased to 200 feet. The
sewer system was installed on September 22, 1975 and an
additional upgrade was added to it on August 11, 1983. Total
absorption area of the trench is 841 square/feet. On the
permit issued July 29, 1983 for the upgrade portion, a waiver
was granted by Larry Montgomery of the MOA, for the separation
distance from the well to the leach area. This distance is
78.5 feet from the well to the standpipe. Notes are written
on the back of the permit.
On 1/16/86, an adequacy and well flow test were performed and
both are satisfactory.
From a site inspection, we see the distance from the subject
well to the neighbors septic tank in 90 feet. This neighbors
legal is Lot 7 -- Block 6 — Mountain Park Estates til. The
sewer system was installed September 22, 1975 and there were
no wells present then. On November 12, 1980, the neighbors
septic tank was replaced, apparently in the same location as
the original one, and inspected by John Fennedy of the MOA,
who approved its location and distance from the subject well.
We therefore request that you 'recognize the existing waiver
for the subject well to the subject sewer system and the
approved distance from the subject well to the neighbors
septic tank.

The owner of the subject property does not have a buyer yet,
so we are not submitting a Health Authority application at
this time. However, we would like to have these waivers on
file so they do not cause a delay at a later time.

A FLOW TEST WAS PERFORMED ON THE WELL. 900 GALLONS OF WATER WAS
PUMPED AT A RATE OF 4.4 GPM OVER A DURATION OF 3.5 HOURS.
THE DRAWDOWN WAS 17.2 r WITH A RECOVERY TIME OF 120 MINUTES
AND THE STATIC WATER LEVEL WAS 154.7 FEET.
THE WELL IS ADEQUATE FOR THIS 4 BEDROOM HOME.

adjacent lot to the south) has been waived to 90 feet.

On February 8, 1983 our company collected a water sample from the house
located on Lot 6 Block 6 Mountain Park Subdivision. The property owner
is Mr. Slaughter. The water analysis was satisfactory.
The well has a seal and the casing stands approximately 3 feet above the
ground. The electrial wires are completely enclosed in conduit. All
standpipes are capped.
