The well was drilled on February 28, 1976. Total depth of well is 200 feet and cased to 200 feet. The sewer system was installed on September 22, 1975 and an additional upgrade was added to it on August 11, 1983. Total absorption area of the trench is 841 square/feet. On the permit issued July 29, 1983 for the upgrade portion, a waiver was granted by Larry Montgomery of the MOA, for the separation distance from the well to the leach area. This distance is 78.5 feet from the well to the standpipe. Notes are written on the back of the permit. On 1/16/86, an adequacy and well flow test were performed and both are satisfactory. From a site inspection, we see the distance from the subject well to the neighbors septic tank in 90 feet. This neighbors legal is Lot 7 -- Block 6 — Mountain Park Estates til. The sewer system was installed September 22, 1975 and there were no wells present then. On November 12, 1980, the neighbors septic tank was replaced, apparently in the same location as the original one, and inspected by John Fennedy of the MOA, who approved its location and distance from the subject well. We therefore request that you 'recognize the existing waiver for the subject well to the subject sewer system and the approved distance from the subject well to the neighbors septic tank. 1200 West 33rd Autnue.
