All construction must be in accordance with:
1. The attached approved design.
2. All requirements specked in Anchorage Municipal Code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72 ) and Drinking Water Regulations (18AAC80 ).
3. The engineer must notify DSD at least 2 hours prior to each inspection. Provide notification by calling
(907) 343-7904 ( 24 hours ). ( Not required for a Water Supply Permit only).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
-WHEN THE WELL HAS BEEN COMPLETED AND SATISFACTORY WATER PRODUCTION HAS BEEN
DETERMINED, THE WELL DRILLER SHALL PLACE AN APPROVED SANITARY SEAL ON THE WELL HEAD AND
NO PITLESS ADAPTER OR PUMP SHALL BE INSTALLED UNTIL AN APPROVED WASTEWATER DISPOSAL
SYSTEM HAS BEEN CONSTRUCTED.

Ref: Exploratory Wells for Proposed Denaly Subdivision Phase 2,
We request a well permits to have three exploratory wells on the proposed lots 32, 45, and 46 of
Tract A-1, Denaly Subdivision for the purpose of subdivision. Attached is a site plan for the
proposed well locations. After completion of the wells, a draw down test will be performed on
these three wells for the purpose of subdividing the properties.

Subject: Hi -head artesian well - construction mitigation measures.
Dear Sir:

Water well construction/ drilling in the above referenced subdivision has commonly
encountered hi -head and hi -flow conditions from the twin unconsolidated aquifer around
200_ feet. Measured characteristics can be on the order of 300tt qpm free flowing with a
shutoff head of 20 psi Ge. •46 feet)! Following are the construction measures we have used
to successfully control these characteristics on the first five (5) wells In this subdivision.
1) Continuously dry grout throughout the total length of the cased well bore 0e.
10-12 sacks of No. 8 bentonite granules per well).
2) Allow the well to flow controlled without excessive pressure buildup, for several
days to allow the dry grout to hydrate and bind with the surrounding formation
material.
3) Shut-in the well with our fabricated, mechanical, artesian well packer -which
holds the water level dam below the frast•line..
We have been pleasantly surprised by the ability of the dry grout to hold and prevent
washout (i& vertical up/dawn water flow) of the well casing annulus. This also constitutes
proof of the ability of the dry grout method to prevent vertical dowel movement of
contaminants in the casing omulus.
