Dear Reviewer,
The above referenced property is currently served by a community septic system.
The owner wishes to install a private system.
The proposed septic system is a 4-bedroom Advantex System with a lift station and AX-20 POD. Review
of available documents indicates that a CAT II system was design but not installed. Extensive soil
explorations by Garness Engineering show that the soils are poor. Similar to the Garness design we are
proposing to install an advanced treatment system in the fill layer. The fill layer has variable thickness and
variable percolation rates ranging from ~11 minutes per inch to nearly 50 minutes per inch. Based on this,
we are proposing to use an application rate of 3 gpd/sqft.
The bed system will be installed over a 2’ thick layer of moa filter sand. The sand layer will be 2’ thick and
will extend 4’ beyond the footprint of the bed. The sand helps ensure that that there is at least 2’ of
accepting soils below the bottom of the sewer rock.
Note that the previous design was for 5-bedrooms and used a larger bed footprint. This design is for 4-
bedrooms and uses a smaller footprint for the bed with a similarly sized area of filter sand.
Our review of available documentation and field investigation show that this project will not adversely
impact any nearby Wells, Wastewater disposal systems, replacement disposal sites, or drainage flowing
onto and off of the subject property. The neighboring lots are small, but based on available information, it
appears they will require AWWTS systems or community septic which should leave sufficient space given
the reduced separation requirements for AWWTS systems.

TIiIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

(AMC.) 1 .65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
Advanced Wastewater Treatment Systems. The Municipality grants permission to the

2. :Maintenance. Repairs and Alterations.
(Owner is required to read. understand and initial each section)
AP -Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer'ss representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically 5400 to $600).
I; Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular :maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

AtOwner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
AukOwner acknowiedges that the fine for failing to maintain and repair an AWWTS may be
assessed in accordance with AMC 14.60.030.
Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
Owner agrees that any sale or transfer of title of the property will not occur' without a new
Certificate of On -Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor,'installer and approved by the Municipality are the governing
Guidelines for the construction, maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring of the AWNVTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5 Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
5. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
?. Severability. Any provisions of this Agreement decreed invalid by a court of competent
Jurisdiction shall not invalidate the remaining provisions of the Agreement.

Ref: Proposed Septic System Upgrade for Grecian Hills; Block 2, Lot 3
To whom it may concern:
The existing 5-bedroom house is served by a private well, a private septic tank and a
community drainfield. Per conversations with the owner, he would like to have an onsite
septic system. Two testholes were excavated on the property as well as 4 percolation
benches. As shown on the site plan, the initial testhole was excavated in the center of
the proposed drainfield. However, it was found to be fill material (see attached soil log)
and backfilled (without a standpipe) as it was hoped that a better soil profile would be
found elsewhere. During the excavation of Testhole #2 (southeast testhole), an organic
layer was found (approximately 1.5 feet) with native ML material below. Due to the soils
visually having a high "fines" content, two percolation benches were excavated (east
and southwest of the TH standpipe — shown on site plan). The percolation bench (perk
bench #1) to the east did not perk. However, the bench to the southwest (percolation
bench #2) perked at 10.9 min/inch (in the native ML material). In addition to the initial
two percolation benches, two more benches were excavated (bench #3 and #4). Two
percolation tests were attempted in the native (ML) material and neither perked faster
than 120 min/inch. In short, 3 of the 4 percolation tests in the native ML material were
impermeable (slower than 120min/inch). Due to this fact, two percolation tests were
performed in the organic layer for bench #3 and bench #4 (each having an organic layer
of approximately 3+ feet). They percolated at 48min/inch and 20 min/inch respectively.
As can be seen on the design drawing and attached soil logs, some of the proposed
area will have less than 2 feet of receiving soils (for vertical conveyance). However, the
southern percolation bench perked at 10.8 min/inch in native material (3.0 gpd/ft2) and
therefore would only require 250 ft2 of drainfield area for a 5-bedroom home
(750gpd/3gpd/ft2 = 250 ft2). We are proposing to install a 1530 two-compartment
Infiltrator Tank, a 1060 Infiltrator single-compartment Infiltrator septic tank, an ATS-8
Aerocell Pod, and a mounded bed type drainfield. Comments regarding the design are
as follows:
1. SOILS: See attached 2018 testhole logs which show, soil classifications, percolation
tests and groundwater monitoring.

2. DRAINFIELD DESIGN: See attached design drawings for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 50 feet of the proposed
tank or drainfield.
4. TOPOGRAPHY: There are no topography concerns. See the attached design
drawings.

Thanks for contacting us about this property. ADEC has no objection to MOA processing the application for a
new private wastewater system for one of the lots currently served by the Grecian Hills Community system.
Since the Grecian Hills Community system was not constructed as approved and does not have Approval to
Operate, the remaining lot connected to this system also needs to apply for approval to continue using the system.
If the remaining home connected to the system is also the owner of lot where the system is located,it might be best
for that owner to apply for a permit through MOA and have the system"converted" to a private system only.
Otherwise,the system should be decommissioned since it is not approved.
Please let us know how you intend to proceed so we can update our file.

Tonya,
The MOA has received a permit application for a private wastewater system on a lot that is currently being served by the
Grecian Hills community system. Unfortunately,that system has never been fully constructed. I think the fields have
been built but the Advantex system has not been installed. Per the owner requesting the septic system, there are only 2
houses using the community system and the owner of the lot with the community field is requiring him to disconnect.
It is unclear if the lots that were to be served by the community system are required to use that system or if they can
develop a private septic system for their own use. Please let me know if we can proceed with issuing permits for private
wastewater systems.

Dear Mr. Pannone:
On April 13, 2005, Afay 26, 2005, and July 28, 2005 our office received engineering plans for
your planned community wastewater collection treatment and disposal system. Ou August 2,
2005 we met and additional information was provided The system consists of a collection
system with individurl septic tams sized for the homes. a l ft station for eigts dwellings, the
package treaimem system by Advantex having a 10,000 gallon recirculation tank and two
Advarsex AX 100 treatment filters and a pressurized bed type onshe disposal system which is
120 feet long by 35 feet in wide.
The issuance of an on site wastewater disposal permit to monitor the wastewater is under review
with preliminary discussion indicating that this permit is in complete and will require monitoring
down gradient of the disposal Feld.
Department has reviewed the engineering plans submitted for the subject project and conditional
approval to construct the proposed system is hereby issued A "Ceztitic2te to Construe" for the
cot uat-ty wastewater system is enclosed. Please use the referenced plan number in
correspondence regarding this system.
Ibis approval is Contingent upon compliance with the following conditions:
1) A maintenance ntmement strut be in place to assure the proper operation of the entire
wastewater system Include the pumping schedule for the septic tanks, contractor
maintaining the lift cation and the package ttestmeel system, and the engineer that will
be contacted to reviewing the proper operation of the design. If the contacted agencies
proving the maintenance services are altered the department must be contacted to allow
the department to have proper eorrAct infortnanor. if a problem occurs.
2) If the applicant fails to constrict, alter. install, or modifY the system wi0tin two (2) years
at= the date that the depattmeat issues as approval to COM=t, the approval is void,
and plants mast be tesubmintA [along with associated fees], for departmect review and
approval.

3) Deviations Earn approved plans which affect capacity, flow, opetatioq major design of
units, point of discharge, materials of major system components (such as pipe, lagoon
liners, etc.), or separation distances, ra= be approved by DEC in writing prior to their
implec ca tion.
4) This approval is contingent upon eomplimce with the conditions of Wastewater Disposal
Raowlationa, 18 AAC 72135, Construction Cerrf cation. The noted station of the
regulations requires that a "Cerdficarion of Conrtrucdan" be completed and submitted to
the Department within ninety (90) days of Completion of const -action. As -built plans or
record drawings should indicate any changes or deviations from the approved plans to
facilitate fical review. A 'Certification of Consovction" form is enclosed for your use.
5) Thls approval is coctingmt upon your receipt of any other state, federal or local
authorizations which arc required for you: project. You are required to obtain all other
necessary rnnhorizntions before proceeding with your project. This system serves more
than twe ry (20) people, the oompleed Class V Nection Well Inventory Form for the
EPA Underground injection Control Program is on Mo. The MAred State Wastewater
Disposal Permit is in review and all cond3ion of this permit will need to be complied
with.
6) You ate advised that if arms developmem will require placing fill in we.Lsads or working
in a ari=a), river, or lake, permits from tbo U.S. Army Corps of Engineers and the Alaska
Dcpartmem of Fish and Game may be required. A Coastal Projects Questionnaire will
help you identify other permits and approvals that may be required foe your project.
This approval does not imply the granting of edditicnal authorizations nor obligate any state,
federal or local regulatory body to grant required authorizatiors.
Any person who disagrees with this decision may request an adjudicatoryhearing in accordance
with 18 AAC 15.195-18 AAC 15340 or an informal review by the Division Director in
accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division
Diroctoc. 555 Co:dav'a Stmt, Anchorage, Alaska 99501, within 15 days of yom'rcceiving the
decision. Adjudicatory hearings requests must be delivered to the Commissioner of the
Department of Environmental Conservation, 410 Wiloughby Avenue, Suite 303, Juneau, Alaska
99801, within 30 days of your receiving tree decision. If a hearing is not requested within 30
days, the right to appeal is waived.
