All construction must be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
1. The entire subsurface disposal field and/or alternate is not within a 30 foot radius of a
percolation test. The Engineer needs to do an additional percolation test prior to the construction
of the septic field. Please submit stamped and signed results with the As -built Inspection Report.
If the results require a design change, construction of the system will stop pending On -Site review
and approval. AMC 15.65.060.B.3
2. If the existing 3 bedroom system is not decommissioned but a diverter valve is used, a note
must be made on the Inspection Report stating that only the system designed for 4 bedrooms
shall be tested for any future COSAs.
3. If, during construction, sewer rock from the existing seepage pit is found construction of the
system will stop pending On -Site review and approval.

Ladies and Gentlemen:
I am writing to request a septic system installation permit for the above referenced property. The
owner would like replace the existing system. The proposed system will serve a 4 -bedroom
single-family residence. Soil logs, design calculations, a site plan, design drawings and
construction specifications are enclosed for your review.
Design Calcs: Groundwater observed at a depth of 8 feet below ground surface
(9/4/14).
Soil Rating. From Testhole 7/30/14
5 min/in = 1.2 gal per sq.ft/day

Required Area per Bedroom: 150/ 1.2=125sq.ft.
Total area required: 125 x 4 = 500 sgft
System: We are proposing a new 1250 gallon septic tank followed by a wide trench absorption
field. The absorption field will be 70 feet long and 5 feet wide and contain 2.0 feet (603 sq.ft.
effective) of sewer rock.
Soils: Test holes were excavated on July 3e, 2014. See the attached soil logs. Ground water
monitoring will continue through construction of the septic system. Groundwater was observed
at a depth of 8 feet below ground surface on Sept 4 2014.
Surface Water: There are no surface waters within one hundred feet of the proposed septic
system upgrade.

Topography: The ground level slopes down towards the south at a 10-15% grade
Waivers: None
The installation of this septic system will not prevent wells and septic systems from being
installed on the adjacent lots. The proposed septic system will not change the general slope of
the area. Ponding and/or concentration of surface runoff will not result from this installation.

In accordance with our conversation concerning the repair of the above seepage pit, on
October 17, at the DHHS offices, I am submitting the following three Record Drawings
for addition to the file on this lot.
was called to the lot on October 14, by Mr. Steve Cowper, renter of the three bedroom
single family house on this lot. He had noted a hole in the rear yard, and had correctly
identified it as a septic system problem. During my investigation, I noted a 2 ft. diameter
hole into a log crib seepage pit of unknown dimensions. My subsequent conversation
with you the following Monday, indicated that, as this was a simple repair, a formal design
and review was not necessary.
1 ran a topo survey of the lot, and prepared the attached drawings, which I sent out for
bids to four certified installers. Mr. Bob Dean of Dean Construction & Development was
the low bidder. On November 1, Mr. Dean repaired the existing seepage pit as noted on
the record drawings. The repairs were accomplished in compliance with all requirements
of the current Wastewater Disposal Regulations, as amended. The excavation was
opened and backfilled on the same day.

cannot be granted until the following items have been
completed:
(1) The water analysis report needs to be submitted
to this office from the Chem Lab, 5633 B Street,
for our review.
4_t(2) The septic tank pumped with the number of gallons
on the receipt to verify it size. This will need
/ to be verified by a registered engineer.
(3) An adequacy test needs to be performed on the existing
leaching area. This test will determine if the system
is adequate according to National Standards. A listing
of private firms performing the test is enclosed. This
report needs to be submitted to this office for our
review.
(4) The top of the well casing needs to be tightened so
that it is water tight.

I am writing to request a separation distance waiver between the existing well and theexisting
septic tank on the above referenced lot.
The well on lot 7A was drilled some time in the late 197as or early 1980's. There is no well log
for this lot. The surrounding lots have wells that were drilled approximately the same time. I have
attached the copy of those well logs. I found that it is cased to over 40 feet, the static water level
was 88 feet below ground level when I conducted a HAA investigation on June14, 2003, and the
total depth is approximately 190 feet. The ground water in this area typically flows from the east
to the west. The septic tank is located west of the well and approximately5 feet lower than the
well. A well flow test showed the static water lowered34 feet while flowing at approximately 2.2
G.P.M. Water samples were taken during the HAA investigation and indicate thatthere is no
bacteria andS-;qlmg/l of nitrates present in the water from this well. The septic tank in its
current location is approximately 91 feet from the well.
The soils on the adjacent lots were logged to be sandy gravel to a depth of 50 to 100 feet below
ground, with gray clay underlying to a depth of approximately 170 feet. Water bearing sand and
gravel were found under the clay layer to a depth of approximately 185 feet below ground level.
The well logs are attached.
As outlined under 18 AAC 80.020 and 18 AAC 72.021(a), I have calculated the following points:

16-25 Almost sure to be free from any form of contamination from household sewage.
