Boyle, Block 2, Lot 4A Septic System Permit Upgrade Request Ladies and Gentlemen: I am writing to request a permit to upgrade the septic system be issued for this property. The proposed systems will serve an existing four (4) bedroom house. Currently the lot is developed. The existing drain field is in failure and will be reused as a 4 bedroom reserve. The existing septic tank and will be abandoned per code, and a new 15008 S.T.E.P. tank with AX20 Pod will be installed. This lot is served by a private well that is located over 100 feet from this system. The surrounding developed lots are also served by private wells. There are no wells within 100 feet of this system. 1. Soils. A single test hole was excavated by Pannone Engineering Services in October of 2012, and the ground water monitored for seven days. Groundwater was observed at 3.5' in the test hole monitor tube after the seven day monitoring period. Bedrock was not encountered in the test hole. It is my opinion, based on the results of the percolation tests and overall soils appearance; an application rate of 2 gallons/day/square feet should be used for a CAT III wastewater system. 2. Soil Absorption System Design. a. See Sheet 2 of the design package. 3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches. 4. Topography: The topography slopes from southeast to northwest at approximately 4%- 5% in the area of the proposed drain field. There are no steep slopes in the area. 5. Drawing Markings: The Drawings are marked "For MoA Review Only'. When written notification that the review is complete and that there are no further comments is received from MoA On -Site Mailing: P.O. In consideration of the mutual covenants contained herein, the parties to this Memorandum of Understanding agree as follows: 1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as 46611 t/T9X located at ����� L • UO`iC6' 16/ ii L� L D't Ali'}, Anchorage, Alaska. 2. Definitions. A. Alteration. Any change to the design or function of an AWWTS that Includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Prior to performing any alterations to an AWWTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On -Site Systems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. D. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. E. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an AWWTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of IZ- 5.0—O ($ 0 .00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. The term of this Memorandum of Understanding shall be for the life of the AWWTS. The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On -Site approval is issued to the new owner or transferee of the property. 5. Alterations, Installation and Removal of Additional Eauipment. Owner agrees not to make any alterations, removal of parts or additions to the AWWTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's AWWTS Operating Permit is: /9-S 9Ea1%96,19 C. Owner acknowledges that the fine schedule for failing to maintain and repair an AWWTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. E. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for AWWTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AWWTS. 7. Nonwaiver. The failure of either party at any time to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 8. Amendmeut. A. This Memorandum of Understanding shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this contract, the only authorized representatives of the parties are: Owner: V� Anchorage: Purchasing Officer C. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of Understanding shall be brought in the Superior Court for the Third Judicial District of the State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Memorandum of Understanding. 10. Severability. Any provisions of this Memorandum of Understanding decreed invalid by a court of competent jurisdiction shall not invalidate the remaining provisions of the Memorandum of Understanding. Dimond B1vd.,.Space 1460 Anchorage, Alaska 99502 Subject: Soil Investigation for Sanitary Sewer System, Lot 4A, Block 2, Boyle Subdivision, Anchorage, Alaska _ Dear Ms. Bond: At your request of September 29, 1977, we conducted a subsurface soils investigation at the proposed location of the sanitary sewer system on the subject lot. The investigation complied with those procedures required by the Municipality of Anchorage, Department of Health and Environmental Protection. This investigation, which was accomplished on October 4, 1977, consisted of a test hole drilled to a depth of 20 feet below the existing ground surface. The test hole was sited according to your instructions and its location is shown in attached Drawing A-01. Drilling was accomplished with a rotary drill rig using continuous flight solid -stem auger with . an outside diameter of 6 inches. A sample was taken at the depth shown on the soils log in Drawing A-01. The sample will be held in storage at our lab for approximately six months. In addition, all material brought to the surface by the augers was continuously monitored by an experienced engineering geologist. The topography at the drilling site is generally horizontal. At the time of the investigation the site had original vegetation consisting of spruce trees. The top of the test hole was located at original ground surface. The soils encountered in the bore hole are shown in the test hole log in Drawing A-01. The symbols used in Drawing A-01 are explained in Drawings B-01 and B-02. This log displays specific conditions encountered at the test location. However, subsurface conditions may vary in other parts of the lot without any apparent surficial evidence of the change. Groundwater was not encountered. Bedrock was not encountered. At the time the hole was drilled seasonal frost was not present. Permafrost was not encountered. A percolation teat was performed within the bore hole at the depth shown in the attached Table 1. All depths were measured from the to_ of the hole. The data in Table 1 show average infiltration from the depths indicated to the bottom of the hole. Anchorage, Alaska 99516 Ref: Boyle S/D Lot 4A Block 2 Well and Septic Adequacy Test Dear Mr. Forrest, Per your request, we performed an evaluation of the water well and septic system which serves the subject property. The results of our investigation are summarized below. WF .t[ TF The well was tested on March 29, 2000. Production was 562 gallons of water over a period of 83 minutes. This is equivalent to 6.8 gpm, which is more than adequate for a 3 bedroom house. The initial static water level was 33 ft, and the stabilized drawdown was 19 ft. Other conditions for the Health Authority Approval (HAA) from the Municipality of Anchorage (MOA), i.e. casing height, sanitary seal, and wires conduited, were also met. The well is located 96 ft from the septic tank on the property, rather than the required 100 ft. A waiver was obtained for this encroachment in 1988. The water was tested, and results were 3.40 mg/l nitrates (maximum is 10 mg/1) and no bacteria. Therefore, the well meets all requirements for the HAA. SEPTIC SYSTEM TEST As per MOA records, the septic system consists of a 1000 gal tank and an elevated bed drainfield, both installed in November 1980. The bed consists of 4 individual trenches with 8 inches of gravel below the distribution pipes. Two monitoring tubes were installed in January, 1994, which enables observation of 2 of the trenches. It is impossible to detect fluid levels in the other two trenches. At the time of our inspection, one of the trenches was submerged, with 8.75 inches of water in the monitoring tube (MT). The MT in the other trench had 3.75 inches of water in it. A total of 109.8 gallons of water was added to the unsubmerged MT which rose the liquid level 5 inches (22 gallons per inch). Absorption was monitored in this MT 5.5 hours later and the level had dropped 7.75 inches, indicating the bed had absorbed at least 110 gals. This indicates absorption of greater than 0.312 gpm, which meets the requirement for a 3 bedroom house. Our evaluation confirms that one of the trenches has failed and one is still operable. Tank was found with 1.5 feet of cover and a liquid depth.of 49 inches. There were no monitor tubes installed in mound...An observation well was dug at the periphery of the mound. Hole was 5 feet deep and extended two feet into the sand filtering layer. Water showed at the bottom of the.hole. The sand was clean with no color or odor to indicate anaerobic conditions. 370 gallon of clean water were added to the tank: At this point water back up into the tank. Level in tank rose 13 inches. Infiltration were monitored for 60 minutes. During the first 15 the level in the tank dropped 7 inches. After 60 minutes the level had dropped 10 inches. After 15 hours the tank level was 49 inches. The residence was occupied during this period with no restrictions on water use. TEST RESULT:. This system meets the.code requirements of the Health and Social Services Department of the Municipality of Anchorage. NOTE The operational life of all septic systems depends on the local soil conditions, groundwater 'levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this septic system. We can therefore not give any estimate of how.long this system will function satisfactory for current or future occupants. The approved separation is 96.44 feet. Reasons for granting this approval are: First, the well is uphill from the septic tank. Second, you have verified water tight couplings on the septic tank. Thirds there are adequate points supporting this waiver of separation distance by the A.D.E.C. waiver guideline procedure. This waiver applies to the existing well to septic tank separation only. During an Health Authority Inspection on April 20, 1988 is was discovered that the distance between the tank and the septic tank was 96.44 feet, 3.5 feet less than the required 100 ft. The well is located up hill from the septic system. The house is situated between the well and the tank. The well log indicates water at 27 feet, however, pounding surface water was in evidence around the residence as is common in Boyle. The design of the septic system was based on ground water at 1 feet below ground surface. By going through the DEC's procedure of analyzing the potential for contamination I arrive at the following points: Ground Water Soil Sorbtion Permeability Gradient Separation Total (at 27 ft. ) 3.0 ( Silt ) 3.5 ( Silt ) 2.0 ( 0 ) 2.0 ( 97 ) 3.0 13.5 ==-=_= Conclusion: Chemical contamination possible but unlikely. As an added safe guard. Tank is a steel tank. Cast Iron was used as well as water tight couplings. Please issue an waiver for this well. WELL YIELD FROM WELL LOG: 15 gallons per minute PUMP YIELD FROM TEST: 7 gallons per minute i DATE OF INSPECTION: April 20, 1988 v TEST PROCEDURE: Well was pumped at a constant rate while the i drawdown was monitored with an acoustic probe. At the beginning of the test water level was found at 19 feet below top of casing. At a pumping rate of 7 gallons per minute the water level dropped to 33 feet after 15 minutes of pumping and remained at that level,for the remainder of the test, 35 minutes. A total of 370 gallons were pumped. The well recovered instantaneously when pumping stopped. TEST FOR E.COLI AND TOTAL NITROGEN: Water was tested for E.Coli and total nitrates on E.Coli 0. Total Nitrates 1.4mg/1. Max. allowable Total Nitrates 10mg/1. TEST. RESULTS: This well meets.the requirements of the Municipality of Anchorage. THIS WELL WILL PRODUCE MORE THAN 3 GALLONS PER MINUTE FOR MORE THAN FOUR HOURS The Municipal requirement for well flow is 150 gallons of water per bedroom per day. This well exceed this requirement. The assessment of the condition of the well applies only to the .conditions as of the day tested. The flow rate may change due to subsurface conditions that may not be observed from the surface, and changes in the land use and other factors that may impact the aquifer feeding the well.
