Soils justification
Dear On-Site Services Engineer,
During installation, the trench was placed approximately 25–30 feet beyond the required 30-foot
distance from the designated test hole. This was an owner installation, and the trench location was
chosen due to site contours and required trench depth.
There are three test holes on this lot. Two were done by KND engineering in 2002, and one was
done by Forge Engineering in 2024. All three test holes show very similar soils and near the same
perc rate. Because of this we are confident most, if not all, of the lot has the same soils and no
difference was noted during inspections.
Based on this, we request approval of the installed trench location, despite it being beyond the
standard 30-foot distance from the test hole.

b. Covered, sealed, and heated to prevent freezing
Special Provisions:

A test hole is required prior to construction of the absorption field, in the location shown on the site plan, to
confirm minimum separation to impermeable soil and seasonal high groundwater, as well as percolation
rate. Construction may proceed at your own risk prior to 7-day groundwater monitoring. If results require
a design change, construction shall stop pending approval of a change order. Please submit results with
the inspection report (or change order, if required).
The water line alignment is to be shown on the record drawing.

Septic system design
Dear On-Site Services Engineer:
The owner of the above lot intends to build a 3-bedroom home on the property, so we are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home as well as the propo sed well, septic location
and alternate site. No conflicts exist between this proposed system and any other well or septic
system, whether on this lot or adjacent lots.
The ground surface on the lot slopes from the east to the west. The slopes are pretty uniform at
between 25-30%. Although the trench will be built on steep slopes, it meets the criteria of
15.65.210.B.4.a.i-iv, including the vertical separation distances measured from the appropriate
sides of the trench. The profile demonstrates the required separation distances.
As the home site has moved from an older design with test holes, we are using the test holes in the
file to design the system, with a new test hole specified to be dug prior to installation. The file is
unclear exactly which of the two test holes are which. We have designed using the deeper of the
two test holes and will ensure the new test hole dug demonstrates adequate vertical separation to
impermeables and groundwater.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all
wells and surface water, and more than 5’ away from the septic tank. The alternate site is located
more than 6’ away. Please refer to the attached test hole logs, plan and profile pages for the septic
design. If this design is followed, there will be no adverse impacts to adjacent properties.

Septic System Owner-installer Agreement
The On-site Water and Wastewater Section (On-site) may issue an approval for a homeowner
to perform work on an on-site wastewater disposal system to serve that individual's owner-
occupied, single-family or duplex home if the homeowner meets and agrees to the following

Dear Ms. Butler:
An On -Site Water/Wastewater Permit, number SW040045, issued by this office for a single-family
system, expired on April 06, 2005. The permit was valid for 365 calendar days. '
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $115.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for anew permit, the fees are: $460.00 for a wastewater permit and $175.00 for a well
permit.

Subject: New sewer/well permit — Sun Valley, Block 1, Lot 4
Gentlemen:
The owner has requested we proceed forward to obtain a septic permit on the subject
lot. On November 18, 2002 two test holes were performed for the proposed system.
The results of these tests are attached. The general slope of this lot is from north to
south at a grade of approximately 20-4070.
We have designed our system utilizing the existing testholes that were excavated for
the proposed 3 -bedroom house. The lot will be served by an individual well, located on
the southern portion of the lot. We propose to install one 5' wide shallow trench.
Water was not encountered during the excavation or during monitoring.
There are no public or private wells within 200' of our proposed system location except
as noted. There is no surface water within 100' of the proposed system and there are no
known curtain drains within 50'. We do not expect there to be any adverse effect on
adjacent lots by the development of this system.
