Attached for your use are the following items: 1. RPKA Inspection Report 2. As -Built 3. Supporting Historical Soil Logs — Percolation Test Reports The enclosed Inspection Report and As -Built are for the conventional wastewater disposal system for residential structure totaling five (5) bedrooms located on property and which is privately owned by Rodger Sullivan. Reviewed percolation rates in the area range from 4 to 28 min/in. It appears that as the poorly graded sand with gravel becomes siltier, the percolation rate increases. Based on the reports reviewed, percolation rates and soil logs for the area suggest that the time required to percolate water in the poorly graded sand with gravel also tends to increase with depth. Of the soil logs reviewed, no impermeable layers were detected. The typical soil profile for the area is composed of 6 to 12 inches of organic soil followed by a combination of varying percentages of sand, gravel, and silt. Attached are: 1. A site plan with stamp and original signature of engineer. 2. Design and calculations with stamp and original signature of engineer. 3. Results of soils, percolation and water table test with stamp and original signature of engineer. An onsite sewer system is proposed for a new 5 -bedroom house. There are no existing onsite water or sewer systems located on adjacent lots or within 100 feet of the proposed system. The ground surface slopes across the lot from northwest to southeast, varying approximately to 4 to 6 percent. An existing natural slope of greater than 25 percent (avg. 2H to IV) is located on the east side of the lot, reference attached site plan. Onsite surface runoff is directed to the south and east. This drainage condition is to be maintained after construction is completed. An area of reserved space will be provided south of the proposed drain field for replacement or modification of the disposal field. The both the proposed and replacement drain field locations are placed on the lot based on MOA slope setback memorandum dated April 7, 2004, attached. No groundwater or bedrock was detected at the time of excavation on May 26, 2004 or on June 3, 2004. We do not anticipate any impacts to adjacent properties. Please note that a test hole and percolation test has not been performed for the alternate field site. We respectfully request that a conditional approval by the Municipality to begin construction of the subject system, provided that a test hole and percolation test is completed by the undersigned for the alternate field site and submitted to MOA for approval prior to construction of the proposed system. A test hole has also been excavated on Lot 4 to the north, as well as two other test holes for the subdivision within 75 feet, all of which have the same soil characteristics to Lot 4, reference attached test hole logs. If a property has a wastewater disposal system that was constructed prior to 1986 and the absorption field encroaches a change In slope greater than 25%, an upgrade of the failed system may be approved in the same location if the following applies: 1. No other location exists on the property to construct an upgrade drain field with proper separations. 2. The engineer field verifies that no effluent ever surfaced on the slope surface. 3.
