At the very beginning of the project, I was asked to dig test holes without any property line staking. The septic system was installed within the 30 feet radius of the test per MOA requirements, but my original drawing was incorrect in the location. The installer (backhoe operator w/ Bradley Company) stated during the installation that he was an employee of the Bradley Company and that they were MOA certified. Since this appears to be false, then I have used my name as the installer on the form to get final approval on the system. The well log and pitless adaptor installation were delivered to me about 2 weeks ago and should be in the file. The asbuilt form has been updated. A test hole was excavated at the northwest corner of the lot and found silty sand for the complete 18' depth. My perc was 8 minutes per inch after a 4 hour pre-soaking. No water was observed during excavation or after the 7 day monitoring period. The soil log consisted of silty sands with some silty gravel mixed throughout the entire depth. An application rate of 0.6 gallons per day per square foot will be used for the design. The slope of the lot is 5 to 6 percent for most of the lot, then flattens out to 2 to 3 percent along the southwest comer. No pounding of water has been observed near the site or in the surrounding area. None of the surrounding lots will be impacted by this new system as the overall plan shows for 200' radius. To Whom it may concern: This is a letter of explanation concerning the history and testing of the septic system on the above referenced lot. The septic system was installed in the December 2005, then the house burned down approximately 3 years later. The house was not rebuilt but the foundation was backfilled and the lot left vacant. Now the lot has been sold to Chris Humphrey and he plans to remove the old foundation and construct a new home for his personal residence with the existing well and septic system. The leach field was tested by filling the system with over 2000 gallons of water then monitoring the water level for 24 hrs. The leach field passed the absorption test without any question and from my inspection the septic system meets all of the current Municipality of Anchorage On -Site requirements for a 4 bedroom system. After the house has been constructed, fall 2013, a new COSA will be submitted for this lot. The new owner will be submitting a new set of house plans to the MOA for a permit and needs the On -Site Department to be apprised of the septic system status.
