4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
1. Foundation cleanout and sewer line joints within 10' of water line are to be encased in bentonite
chips.
2. The distance between the water line and sewer line at the house are to be documented.
3. Deck support integrity is to be maintained throughout installation.

Ref: Proposed Septic Upgrade for Meadow Brook; Block 3, Lot 8-13;
To whom it may concern:
The existing 3 bedroom house is served by community water and a private septic system. The existing
septic system is in a state of failure and needs to be upgraded. We are proposing to install a new 1000
gallon septic tank and a deep trench type drainfield. One test hole was excavated on the property. The
drainfield will be designed around the 30 foot radius of this test hole. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY & SLOPE WAIVER: As can be seen on the attached design drawing the
topography around the drainfield runs northwest to southeast at a 5-10% slope. There is a cutbank located
30 feet from the proposed drainfield. As can be seen on the attached drawing, at a 25% slope (starting 2.5
feet above the drainfield) the potential point of daylighting is greater than 50' horizontally from the
drainfield. At this time we request that your department issue a 30 foot separation distance waiver from
the cutbank to the proposed drainfield.
5. LOT LINE WAIVER: We are requesting that your department issue a 4 foot lot line waiver from the
proposed drainfield to the west property line. Per MOA records the drainfield located on Meadow Brook;
Block 3, Lot 9 is located greater than 10 feet from the property line. In short, there are no encroachment
concerns between the drainfields.
We are unaware f any adverse impacts this installation would have on adjacent wells or septic systems.

Ref: Waiver Requests for Meadow Brook; Block 3, Lot 8-13;
To whom it may concern:
LOT LINE WAIVER: We are requesting that your department issue a 4 foot lot line waiver from the
proposed drainfield to the west property line. Per MOA records the drainfield located on Meadow Brook;
Block 3, Lot 9 is located greater than 10 feet from the property line and will have a minimum separation
distance of 16 feet to the drainfield serving Lot 9. In short, there are no encroachment concerns between
the drainfields.
POTENTIAL WATER/SEWER LINE WAIVER: We are requesting that your department issue a 1
foot separation distance between the existing water line and the proposed sewer line. The encroachment
was not confirmed due to the water line running underneath the deck so it is only a potential that the two
service lines could be encroaching. In any case we are proposing to encase the foundation cleanout and all
joints within 10 feet of it in bentonite chips.
PROPOSED DRAINEIELD TO DECK SONOTUBES (FOUNDATION) WAIVER: We are
requesting that your department issue a 2+ foot waiver from the proposed drainfield to the deck
foundation. Justifications for the waiver are as follows:
• There is no potential for effluent to migrate into a living area. .
• Once the new drainfield is installed, there is no potential for it to "collapse", nor is there a danger
of the foundation being undermined.
We are unaware of any adverse impacts the granting of these waivers would have on adjacent wells or

engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)

ATTACHMENT TO C. ABSORPTION FIELD DATA, Date of Adequacy Test
A septic test was attempted on September 10, 2005, at which time the system was in a
failed condition. The system consists of a 6" diameter sump with the holes in the
distribution pipe facing upward instead of in the bottom position. The owner had the
septic tank and the 6" sump pumped by J.R. Pumping on September 17, 2005, and they
also back -flushed the distribution line in the leach field. The sump did have at least one
hole down approximately 12" below the invert of the distribution pipe. J.R. Pumping's
employee stated that he could see the bottom of the sump and it had been capped off.
A septic test was then conducted on September 21, 2005, but the system failed to absorb
450 gallons. A 2" monitoring tube was driven by Stewart Gilbert of STG on October 9,
2005; however, the monitoring tube was driven approximately 21-3/4" lower than the
bottom of the 6" sump.
A second test was conducted on October 10, 2005, at which time the system again failed
to absorb the required 450 gallons. On October 23, 2005, Alaska Terra Lift was
contracted to terra lift the leach field area.
A third test was conducted on November 22, 2005, and the system absorbed
approximately 390 gallons in 4 hours and 20 minutes, which exceeds the Municipality's
requirements.
This test indicates the condition of the septic system on the date of testing. No
representation is made regarding the future life or performance of the system. Many
factors affect the life and performance of a septic system, including the quantity and
quality of wastewater flow, frequency of septic tank pumping, and changes in
groundwater conditions.

On behalf of my client, Mrs. Shirley K Taylor, I am requesting a waiver on a separation distance
of less than ten feet from septic leach field to property line on the above referenced property. As
shown on an as -built drawn in December, 1976 the leach field is approximately one foot away
from the property line.
There are no adverse effects that will be incurred on the adjacent properties as the area involved
is on public water. The last sump pipe is greater than 15 feet from the actual pavement, so it will
not interfere with snow removal. The lawn and shrubs from this property and the adjacent
properties extend out to the pavement. Attached is a copy of the as -built.
