Approval of the plat for 18 months subject to:
1. Resolving utility easements.
2. Correcting the following:
a. Place grid number, NW54, and the case number, in the title
block.
b. Map information: West of platted area, the block label is
missing.
C. Street names: Vicinity Map: "East" is missing from East
Eagle River Loop Road; "Ave" missing from Baronoff Ave, and
Baronoff is misspelled. Crestview Ln, Eaglewood Dr and
Loops mislabeled.
3. Submitting the following to Zoning Code Compliance:
a. An as -built survey, with respect to proposed lot lines, to
verify compliance with yard setbacks, clear vision area
requirements, etc.
b. Parking layout and parking calculations to verify compliance
with AMC 21.45.080.

3. Providing the following to Development Services, On -Site Water &
Wastewater Division: Information to satisfy the requirements specified
in AMC 21.15 and AMC 15.65 must be submitted for each lot of this
proposed subdivision. This information must include, but may not
necessarily be limited to:
a. Soils testing, percolation testing and groundwater monitoring must
be conducted to confirm the suitability for development using on-
site wastewater disposal systems. Ground water monitoring must
be conducted during high ground water season in either the fall
(October) or spring (April -May).
b. Areas designated for the original and replacement wastewater
disposal system sites must be identified and must meet all criteria
specified in AMC 15.65, including slope and setback requirements.
c. Topographical slope information must be submitted.
d. Supporting documentation on water availability must be provided.
4. Submitting the following to Zoning Code Compliance:
a. An as -built survey, with respect to proposed lot lines, to
verify compliance with yard setbacks, clear vision area
requirements, etc.
b. Parking layout and parking calculations to verify compliance
with AMC 21.45.080.

5. Resolving with the Municipal Surveyor the need to remove the
following plat note: All distances and bearings shown are record

1. The need for a variance for length of the flag pole portion of
proposed Lot 1.
2. Redesign to provide access for all three lots via either a flag or
access easement, with the width and location to be resolved with
the Traffic Department, to ensure adequate sight distance. If there
is an access easement, enter into an access agreement through
Zoning Code Compliance.

The existing well and on-site wastewater system serving this property are not permitted
and have not been approved by the MOA. Prior to approval of the request, the applicant

site water and wastewater disposal regulations. Reserve site requirements also apply.
