Response to Comments/Lot Line Waiver Request
Ladies and Gentlemen:
I am writing in response to your comments received for the above reference project. The
Record Drawings were submitted to your department on November 7, 2006 for final
approval to operate, and a Certificate of On -Site Approval (COSA) request was submitted
on December 11, 2006. You have completed a Rush review and submitted comments on
the submittals. I will address your comments below and in the order they were
presented:
11-VComment: Discrepancy in information submitted. As -built site plan and survey have
different locations for various items.
Response: Record Drawing was revised per As -Built Survey and measured swing ties.
Revised Record Drawing site plan is attached.
& Comment: Incomplete, missing pump log.
Response: I contacted Sullivan Water Wells and the Pump Log is attached.
cf' Comment: Proposed system too deep for soils information submitted.
Response: New System was installed nine feet below the original ground elevations.
Maximum allowable depth was eleven feet.

Lot Line Waiver Request: I am requesting a lot line waiver be granted for the reserve
system location. The house was moved north on the lot, which moved the location of the
drain fields further north. The reserve field will need to be installed to within 6 feet of the
north property line to maintain the 14 feet separation from the primary field. The area
north of the lot line is a dedicated airfield and granting this waiver will not affect the
future development of this lot or the northern lots.

Well & Septic System Upgrade Permit Request
Gentlemen:
I am writing to request a permit to construct a new well and septic system be issued for
this property. The proposed systems will serve a proposed three-bedroom house. The
proposed well will be located in the south west corner of the lot, over 100 feet from the
proposed septic system location. The surrounding lots are served by private water
systems and are over 100 feet from the proposed soil absorption system.
1. Soils. See the attached soils log. Ground water was monitored for seven days
through the October high ground water period. No ground water was encountered in the
test holes after the water -monitoring period. Bedrock was not encountered in the test
hole. It is my opinion that the overall soils appearance of the soils, an application rate of
0.45 gallons/day/square feet should be used using a conventional treatment system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
The proposed systems will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The average topography in the area of the proposed septic system is
approximately 5 percent. The proposed system will be installed on the northern portion
of the lot near an existing airfield. There is no development north of the proposed
system.
The proposed installation will not affect the future development of the surrounding or

A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
24 of Eklutna Heights Stewart Add. subdivision. This inspection revealed a
nitrate concentration of 6.69 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Approval (COSA) based only upon the representations given In paragraph 4 by an Independent professional civil
engineer registered In the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also Issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of Issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
