Box 196650 Anchorage, AK 99519-6650 Subject: New sewer & well permit — Eklutna Hts., Lot 7A Block 1 Gentlemen: The owner has requested we proceed forward to obtain a well and septic permit on the subject lot. On May 15, 2001 we dug one testhole for the proposed system. The results of this test are attached. We propose to install a 5' wide shallow trench for the 2 - bedroom house. Water was not encountered during the excavation or monitoring. The general slope of this lot is from east to west at a grade of approximately 3 - 6%. An individual well will serve the lot. An existing shallow well of approximately 40' deep will be properly abandoned and decommissioned and a new well drilled in the immediate vicinity. There are no public or private wells within 200' of our proposed system location except as noted. There is no surface water within 100' of the proposed system and there are no known curtain drains within 50'. Box 196650 Anchorage, AK 99519 Re: Eklutna Heights Lot 7A, Blk 1 Narrative & Holding Tank Application Dear Mr. Cross: The proposed holding tank and well installation will have very limited impact on adjacent properties for the following reasons: 1. The surrounding lots are large. 2. Immediate neighboring septic systems are all +30' distance. There is no development on the adjacent lots to the east, south and west. 3. The lot has been proven to be unsuitable for conventional septic system use, therefore a holding tank is specified. 4. The lot is considered a developable wetland area and fill will be placed to allow a driveway surface. We did not observe any standing water in this fall when we began our initial investigation. A permit has been obtained for wetlands development. Drainage will not be affected and is not a major consideration in our design due to the holding tank design. The wastewater plan is for a single family residence only. 2. The drawing and or site plan shall be a part of this specification. 3. All materials and workmanship shall meet the Anchorage Department of Health requirements. 4. All excavations and depths are advisory and are to be verified in the field by the contractor to meet Municipality of Anchorage requirements. 5. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any adjacent multi -family wells. 6. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer approval. B. HOLDING TANK 1. The holding tank excavation is to be over -excavated 1' depth and backfilled with compacted NFS Type II soil. 2. The tank is to be anchored by four, 2,200 lb. concrete blocks, two on each side of tank with 'h" galvanized cable straps over the tank. Each pair of blocks located at 1/3 the length of tank from each end of the tank. These blocks are available at Klondike Concrete. 3. The tank is to be installed level. 4. The tank is to be a tank that is approved by the Municipality of Anchorage with corrosion prevention standard, for septic tank as per Uniform Plumbing code. 5. Holding tank must have two -six (6) inch diameter standpipes with airtight caps to provide pumping access. The standpipe must extend at least twelve (12) inches above the surface of the ground. 6. Holding tank must have a watertight manhole to provide access to the interior of the tank. The manhole must be at least twenty (20) inches in diameter. 7. A holding tank must be equipped with a high water alarm which registers both visually and audibly Iasi& the dwelling. The alarm must be positioned to allow at least three hundred (300) gallons per bedroom of additional storage after the alarm has been activated. 8. Alarm and wiring to be to Municipality of Anchorage code. 9. The tank is to be installed with 4' of unsaturated earth cover.
