Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The absorption system on the subject lot has failed and must be replaced prior to the
issuance of a COSA. We are therefore requesting a permit be issued for the upgr@de
of the septic system to accommodate the six -bedroom home on the lot. The new
absorption trench will be constructed at the alternate site approved by the permit issued
in 2006. The attached Site Plan and backup documentation identify the location and
configuration of the existing and proposed septic system. The existing Advantex
Treatment Tank has been in service since 2006 and is suitable for continued use
according to maintenance records. The existing absorption trench will remain in service
and be connected for future us with a flow diverter valve. We have also identified the
locations of the existing well on this lot and adjacent lots. No conflicts exist between the
proposed septic system and the wells or water services on this or adjacent lots.
Drainage arrows are shown indicating the current drainage patterns. The ground
surface in the area of the proposed trench is virtually flat. The drainage patterns will be
maintained after construction.
The test hole utilized during the initial design indicated Silty Gravel (GM) in the
absorption zone of the trench. The percolation rate was determined to be 12 minutes
per inch. No groundwater was encountered during the placement of the test hole and
none has was found during the monitoring period. The previously approved design
however was completed with a water table at 11' below the surface. We have
designed the new trench with an application rate of 2 gallons per day per square foot
which is much more conservative than the previous design. We are proposing to place
a 53' long trench by 5' wide with a 3' effective depth. The total depth of the system will
be 6' with the distribution line at 3' below the existing surface.

The ground surface on the lot in the area of the new trench is flat. The trench will be
placed parallel to the contours of the ground as much as possible and in conformance
with design criteria. A minimum of 100' will be maintained between all components of
the septic system and the proposed well and from any surface water in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will, be maintained.

Proposed Septic System Design Change
To Whom It May Concern:
Permit No. SW0500182 was issued on June 17, 2005 to install a septic system on
the subject lot. It is proposed to change the design from an Intermittent Dosing
Sand Filter to an AdvanTex AX20 advanced wastewater treatment system.
The proposed design is based upon a test hole dug by Roger Shafer, P.E., of S&S
Engineering on March 19, 1992. The hole was dug to 13.0 ft with no water
encountered. After a 7 day monitoring period, no water was detected, but on June
8, 1992, water was detected at 11 ft. The proposed design is based upon a 5 ft
separation to this level, with the bottom of the drainfield at a maximum depth of 6
ft. On April 11, 2006, the test hole was dry.
The proposed drainfield will be installed in a silty gravel layer between 3.0 ft and
6.0 ft. A percolation test was conducted in March 1992 by S&S between 5.5 ft and
6.5 ft and resulted in a rate of 16 minutes per inch. An application rate of 4.0
gpd/ft' is used in the design. The soil log is attached.
The drainfield is designed for 6 bedrooms, which is over -sized. The proposed
house is 5 bedrooms, and the tank is sized for 5 bedrooms, at 2500 gallons. One
AX -20 pod is planned for the system, which has been approved by the
manufacturer, Orenco. The system will be maintained by Anchorage Tank &
Welding, monitored 2417 with the VeriComm system, and inspected at least twice
annually. If conditions dictate, another AX -20 pod will be installed. There is space
available on site for a reserve drainfield.

A water sample revealed a nitrate concentration of 7.37 milligrams per liter (mg/L).
The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 mg/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. Please see the attached "Nitrate Fact Sheet" for important
information regarding nitrate.

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT,herein the"AGREEMENT" made and

(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System(AWWTS),
described as Advantex Treatment System

2. Maintenance,Renairs and Alterations.
(Owner is required to read,understand and initial each section)
47/1/- Throughout the term of this Agreement,the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
-bta It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance,adjustment(s), replacement costs,and inspection costs. This
includes an annual maintenance fee(typically$400 to$600).
\it• Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance.

Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system,which could include sewage backup and costly repairs or drainfield
replacement.

by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction,maintenance and repair of the Owner's AWWTS.
- ' Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system,or upon transfer of title,and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions,nor in any way affect
the validity of the Agreement or any part hereof,or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
