RE: Septic System Inspection Report - Lot 10 of Shangri La Subdivision
Dear On -Site Services Engineer
F. Robert Bell and Associates was retained by BTFI Construction, LLC to provide third party inspection services on the
installation of the septic system on Lot 10 of Shangri La Subdivision.
Three inspections were performed by F. Robert Bell and Associates:
The first inspection took place on June 15, 2006. Brian flarten, P.E., the contractor and design engineer, had realigned the
drainfield trenches from their original design to better accommodate the relatively flat location and broke the trenches into
two runs, one 40 feet in length and the second 45 feet in length. Note this is 20 LF more than the original design required.
The 1500 -gallon, 10 -gauge septic tank had been installed, but not backfilled. The drainfield trenches had been excavated, the
monitoring tubes were installed and installation of septic rock commenced after our inspection.
The second inspection took place on June 16, 2006. Both trenches had been filled with washed rock and the drainfield piping
was installed. Washed rock was placed 2 to 4 inches above the top of the piping, geotcxtile silt barrier was in place on top of
the washed rock. The final piping from the septic tank to the drainfield was being completed.
A final inspection of the completed piping was performed on June 17, 2006. All piping, the drainfields and the septic tank
were insulated with 2" of direct -bury board foam prior to backfill.
Through all three inspections, we found the construction to be in compliance with the plans and specifications as approved by
the MOA for this project. Workmanship and quality of construction were more than adequate.

General
I have been retained by Tom and Amber Allen to provide Civil Engineering services for the design and construction of
a residential water and wastewater system on the above referenced property.
The subject property is being developed for a 4 -bedroom, single-family home, which may be remodeled at a future
date to include a fifth bedroom. Therefore the wastewater system has been designed to accommodate five bedrooms.
This water and wastewater design is consistent with the lot development plan submitted to your department during the
approval process for the Shangri-La Subdivision. See attached Figure I for a copy of this plan.
Soils
See the attached soils log which details soil classifications, groundwater observations and percolation rates found in
the test hole performed on the subject property. Additionally, the original subdivision development testhole data has
been attached to show consistency in the soils found on the subject property.
Drainfield Desien
Percolation tests provided a percolation rate of 4.10 minutes per inch for Lot 10. We are proposing a conventional, 5 -
wide shallow trench system for the wastewater treatment on Lot 10.

There are no adjacent surface waters within 100 feet of the proposed well and septic improvements.

Topriaraphy
The subject property is sloping from south to north with average slopes of 15-20%. However, there is a slope across a
portion of the front (north end) of the lot that was created during the road construction. This slope has a grade of 35%.
It was found impractical to keep the wastewater disposal system (primary and reserve) greater than 50 horizontal feet
away from this slope, so an alternate method was used to determine the practical horizontal separation required from
this slope. Please refer to design sheet 5 of 5 for a detailed analysis of our proposed slope separation. The calculations
show that the proposed replacement system, the field closest to the slope, has a theoretical horizontal separation of 86
feet, much greater than the 35 feet minimum established by this method.
Additionally, the type and percolation rate of the soils found on the subject property are not of the type that would
allow lateral migration and subsequent "day lighting" of the wastewater effluent anywhere along the slope in question.
Groundwater and Bedrock
No groundwater was encountered during testhole excavation or subsequent monitoring. Bedrock was encountered at a
depth of 14.5 feet during testholc excavation. Our design keeps a minimum vertical separation distance of seven (7)
feet from the bottom of the proposed trench to the bedrock layer.
Closing
The development of this property will not preclude the adjacent properties capacity to construct future water or
wastewater systems. I am unaware of any adverse impacts this installation would have on any existing adjacent wells
or septic systems. Additionally, this design is consistent with the overall lot layout approved by your department for
development of the lots within the Shangri-La Subdivision (see Figure 1.)

Single Family On-site Wastewater Disposal System
Construction and Material Specifications
Subject Property: Lot 10, Shangri-La Subdivision
Date: April 18, 2006
General.
1. The scope of the project includes the procurement and installation of a 5-bedroorn on-site
wastewater disposal system at the location depicted on the attached site plan drawings. The scope
of work also includes the installation of a new 1,500 gallon, 2 -compartment septic tank, a 5 -wide
drainfield comprised of I trench, 5 feet wide x 65 feet long, 7.5 feet deep with 4 feet of septic rock
below the distribution piping. The location of the drainfield is shown on the attached site plan
drawings. The total depth of the field will be 7.5 feet below existing ground. All components of the
proposed septic system must be constructed a minimum of 100 feet from any surface water or
water wells in the area. A minimum of 4 feet of earthen cover is required over the septic tank and a
minimum of 3 feet of earthen cover is required over the drainfield. In addition to the required
earthen cover, 2' thick, 40 psi, direct bury board foam shall be installed directly over all installed
piping. Drainfield shall be installed parallel to slope contours.
2. Construction shall be in accordance with the approved site plan and design drawings and any
special provisions noted on the Municipal well and/or septic permit. All materials, components,
workmanship and construction practices shall comply with the Municipality of Anchorage's latest
ordinances regarding wells and septic systems, as well as MOA, Development Services
Department, On-site Water and Wastewater Program (DSD) policies.
3. The contractor is responsible for obtaining all underground utility locates, layout of the proposed
system and verifying and maintaining all required separation distances to lot lines, surface water,
wells, steep slopes, etc.
4. The property owner shall be responsible for final grading any depressions formed from settlement
of the backfill material. The property owner is also responsible for revegetation of all areas
disturbed by septic system installation activities.
5. Contractors installing wastewater disposal systems must be certified to do so by the MOA DSD,
On -Site Water and Wastewater Program. Property owners installing their own systems must
receive advanced approval from MOA DSD prior to system installation activities.
6. The contractor shall ensure that ALL materials being used are in compliance with the most current
guidelines established by the Municipality of Anchorage. Please refer to oStandards and
Specifications for Component Parts and Materials Used in Construction of On -Site Wastewater
Disposal Systems', dated December 2000, as provided by MOA DSD, On -Site Water and
Wastewater Program.
Septic Tank Installation:
A new septic tank, of the size specified in the design drawings, shall be procured from a qualified
manufacturer/supplier if the existing septic tank is found to be defective. Construction shall include

two 4" diameter cleanouts, for pumping access. In the case of Advantex Treatment Systems, refer to
the attached design drawings for the size and type of tank to be installed. Installation of these
systems will follow the installation procedures outlined in the Advantex installation manual.
2. The burial rating of the tank shall be adequate for the actual burial depth encountered.
3. The tank shall be level and sufficiently bedded to prevent settling or shifting of the tank.
4. AM standpipes on the tank shall extend a minimum of 12 inches above the final grade.
5. Tanks installed with less than 4 feet of cover shall be insulated. See general note 1.
6. The sewer service line from the house to the tank shall have a minimum slope of 2% unless prior
approval is obtained from the engineer.
7. The inlet and outlet lines form the tank shall be bedded in drainrock or the soil shall be adequately
compacted to prevent future settlement of the soil.
8. Septic tank inlets, outlets and cleanouts shall be fitted with watertight couplings (Femco, Caulder, or
equal.)
9. A clean-out shall be installed 1 to 4 feet from foundation.
10. Post tank clean -outs shall be installed per MCA requirements. Does not apply to pressurized lines.
11. In areas with high groundwater, the septic tank shall be sufficiently anchored to resist buoyant forces
when there is not adequate soil cover to resist the uplift force. The contractor shall refer to the
engineer for direction regarding this matter.
12. The tank backfill shall be graded sufficiently to ensure positive drainage away from the tank.
13. All holding tanks and lift stations shall have an audible and visible alarm inside the residence being
served.
14. All lift stations shall be wired in compliance with all MOA guidelines and the Uniform Electrical Code.
A municipal electrical permit and inspections must be obtained in areas where required by the MOA.
15. The contractor shall verify that the septic tank/lift station
water well, 150' from any class 'C' well and 200' from
separation distances must also be maintained.

discrepancies are noted, contact the Engineer immediately for clarification.
2. All drainfield trenches shall be installed parallel to slope contours.
3. The bottom of all trenches shall be within 2* of level. If the sidewalls or bottom of the excavation
become smeared, they must be raked or scarified prior to placement of drainfield filter sand or septic
rock.

4. A minimum of 2 feet of accepting soils is required below the drainfield rock for a 5' wide trench. The
contractor shall verify this condition prior to placement of the septic rock. All pockets of
unacceptable materials shall be removed and replaced.
5. The trench or bed must be a minimum of 5 feet from the test hole excavation.
6. All distribution piping must be installed level with the perforations facing downward, atop a level bed
of drainfield rock. Septic rock shall then be placed atop the distribution piping to provide a minimum
of 2 inches of cover above the piping. The septic rock shall not be Opyramided" over the top of the
pipe, but shall be installed level across the entire width and length of the trench or bed. .
7. A silt barrier or geotextile fabric must be placed between the drainfield rock and the native backfill.
Installation of rigid board foam DOES NOT eliminate the need for the silt barrier. The silt barrier
shall cover the entire drainrock surface.
8. Monitor tubes must be 4" in diameter and installed in the locations depicted on the design drawings.
The portion of the monitoring tube below the septic rock must be perforated and the portion above
the septic rock shall be solid. The monitoring tube shall extend a minimum of 12" above finished
grade.
9. Backfill over the final gravel layer must not be less than 2 feet. Insulation must be installed when the
backfill depth is less than 3 feet.
10. Drainfield trench backfill shall be mounded to prevent depressions from forming due to subsequent
settlement of the backfill material. Grade areas adjacent to the drainfield to direct surface runoff
away from the absorption trench.
11. The contractor shall verify that the drainfield is a minimum of 100' from any residential water well,
150' from any class 'C' well and 200' from any community water wells. Additional separation
distances must also be maintained.

1. A minimum of two inspections are required by the Municipality of Anchorage. These inspections
must be conducted under the supervision of a Professional Engineer licensed to practice in the State

of Alaska. The first inspection must take place upon completion of the excavation for trenches, beds
or pits and before the installation of any filter sand or drainrock. A septic tank may be set in place,
but may not be backfilled. A tank may be set in place, but cannot be backfilled.
The second inspection must be conducted after the placement of the filter sand, drainrock,
monitoring tubes, standpipes, cleanouts, filter fabric and insulation. No backfill should be in place at
the time of inspection.
2. The contractor shall provide a legible copy of all field notes relating to system layout and
construction to the Engineer for use in preparing the record drawings for the proposed septic
system.
3. The contractor shall coordinate all inspections with the Engineer. A minimum of 24 hours notice is
required prior to start of construction. If necessary, a pre -construction meeting will be held on-site.

OBJECTIVES.
Document groundwater depths in test hole monitoring tubes located at each of the 22 proposed
lots (see toils report, Sept 2000. KND Engineering)
FINDINGS:
Groundwater depths were measured starting 15 May 2DO2 'The configuration of the new
subdivision layoutwas revised inMarch2002to satisfythe slopeto size raquwementforR-IOSL
zoning Lots 9.12. 20, and 22 did not have test holes due to the realignment of property lines
The existing test hole at Lot 6 was located at the edge of a partially excavated hole where silt
from the upper soil layer was allowed to run into the hole over time, precipitation carried silt
into the hole and monitoring tube. creating a seat and both were filled with water
On May 16. 2002, now test holes where dug for Lots 6. 9. 12, 20. and 22 using a tacked
excavator Monitoring tubes (4 -inch perforated drainpipe) were installed at each hole and
depths and soil types were recorded All the new test holes showed soils consisting of gravels
and sand with traces of silt and some cobbles at depth No silt layers where found and the
percolation rates are expected to be similar to those found at the other 22 test holes
Groundwater levels were monitored until May 28. 2002 and the measured depths are listed in
the table enclosed Test holes am listed by now lot location along with the original lot number

Arsenic Advisory
Certificate of On -Site Systems Approval # 070083
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
10 of Shangri-La Subdivision. This inspection revealed an arsenic
concentration of 16.4 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. information on
arsenic is available from the On -Site Water and Wastewater Program
website (www.muni.org/onsitc) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
