Septic System Inspection Report
On Tuesday, August 25, 2009, I inspected the septic system constructed on Lot 2,
Block 2, Mt. McKinley View Estates Subdivision to see if the Cleanouts and Monitor
Tubes had been straightened and additional fill placed over the system.
All of the vents are now vertical and additional fill has been placed. The tank now has
more than 4' of cover and the trenches have at least 3'. The conditional approval
placed on the system can now be removed.

Lot 2, Block 2, Mount McKinley View Estates is a very large lot more than 550' wide.
Test holes were placed on the lot before any of the lot lines or comers had been
marked. The field technician tried to locate the test holes as accurately as possible
during the field study, but was unable to find the exact locations. The absorption trench
was centered on the test holes as shown on the as built drawing. The exact location of
the test holes are shown on the as built. This location varies from the design plan due
to estimation errors made by the technician during field measurements.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:.
· -~The owner of the subject property intends to construct a new four-bedroom home on
:the lot. We are therefore requesting a permit be issued for the construction of a new
well and septic system to serve the home. The lot is in excess of 5 acres in surface
area. No conflicts exist with adjacent septic systems or wells and the proposed septic
system and well on this lot. The attached Site Plan and backup documentation identify
the location and configuration of the proposed well, septic system and alternate site.
The current drainage pattern is identified on the Site Plan. The drainage pattern will be
maintained after development of this lot is complete.
Test holes were recently placed on the lot in the location of the proposed absorption
trenches and at the flattest portion available on the lot. These holes indicated clean
gravel with some sand and silt (GM) in the area of the absorption trench and alternate
site. The accepting soils were tested and found to have a percolation rate of between 1
and 5.4 minutes per inch in the accepting soils. An application rate of 1.0 was utilized
in the design of the absorption trenches. Groundwater was not encountered dudng the
original test hole placement and was not found on May 30, 2006. Our design includes
the placement of a new 1,250-gallon septic tank along with 2 - 53' long by 5' wide by 1'
effective depth absorption trenches to collect and treat the sewage generated in the
house. The bottom of the trenches will be 3' below the surface. The distribution pipe
will be 2' below the surface. A minimum of 3' of cover will be placed atop the trench
and a minimum of 4' of cover will be placed atop the new septic tank.
The ground surface on the lot slopes at steep grades from east to west near the
absorption trenches as shown on the Site Plan. The absorption trenches will be

constructed parallel with the slope of the ground surface as much as possible in
conformance with Municipal requirements. The new septic system will be constructed a
minimum of 100' from the well on this and on the adjacent lots. It will also be
constructed a minimum of 100' from any surface water in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on existing
septic systems in the ama or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

The Municipality of Anchorage Development SerVices Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties serVed by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served. by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,

A recent inspection of the septic system on Lot 2, Block 2, Mt. McKinley View
Subdivision indicated several cleanouts and monitor tubes were out of plumb with the
surface and must be straightened to provide access to the system. In addition, slightly
less than 3' of cover was found over the absorption trenches in some areas. Heavy
snowpack in the area makes it impossible to correct these deficiencies at this time. We
therefore request a conditional approval of the septic system be issued at this time
pending completion of the deficiencies by June 15, 2007.
The cost to complete the deficiencies is estimated at $1,000.00. The owner of the
property has agreed to place in escrow a total of $1,500.00 to cover the cost of the
repairs to be made. We hereby request the conditional approval of the septic system
and the Certificate of On Site System Approval to allow the transfer of the property.
