4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
1. Construction may proceed at your own risk before the 7 day water monitoring is
complete. Please submit stamped and signed results with the As-built Inspection Report. If the
results require a design change, construction of the system will stop pending On-Site review and
approval.

Ref: Revisions to Permit Number OSPlll007 for a Septic System Design Upgrade for
The Tablelands Subdivision; Lot 7, Block 4
To whom it may concern:
Per the special provision of the referenced permit, we excavated a test hole on the property. Our first attempt at
excavating a test hole near the reserve drainfield site, we found that buried debris and organics were
encountered which made this area not suitable for installing the new drainfield. We then excavated a test hole
further downhill and found suitable soils. Attached is a soils log which shows the sols classification and the
percolation test data. The percolation rate was found to be 48 minutes/inch. Based upon our septic adequacy
test, the existing drainfield has absorption capacity of 313 gpd and is adequate for 2 bedrooms. Since the house
is a 4 bedroom residence, we are proposing that a 2 bedroom upgrade be installed around the 30 foot radius of
our test hole. A flow divider will be installed after the tank and double cleanouts which will evenly split the
flow between the existing drainfield and the new drainfield. Attached is the revised design drawing will shows
the new design criteria. The drainfield is to be installed at the base of a 25-35% slope and the slopes downhill
from the drainfield are a 15 to 25% running approximately west to east. Over 50 feet downhill from the
drainfield, the slopes increase to over 25 percent. In short, there are no slope concerns with this installation.
We request that the permit be revised to accept the new drainfield location and design criteria.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems. If

Ref: Proposed Septic System Upgrade for The Tablelands; Lot 7, Block 4
To whom it may concern:
The existing 4 bedroom house is served by public water and a private septic system. The system was installed
in 2006 and is in a state of failure and needs to be upgraded. We are proposing to install the 5-wide reserve site
and utilize the existing 1250 gallon septic tank. The tank is less than 5 years old, so it is reasonable to assume
that the integrity of the tank has not been compromised. There are two test holes filed with your department.
The drainfield will be designed around the 30 foot radii of these test holes. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached logs which show the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specificationso
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: The average topography below the drainfield is a 20 percent slope running
approximately northwest to southeast.

Septic System Adequacy Test, and Upgrade Proposal for a Septic System Serving a Single
Family Residence within the Municipality of Anchorage (MOA).
Mr. & Mrs. Walljasper,
Per your request, we inspected the septic system, which serves the subject property for the
purpose of obtaining an MOA Certificate of Onsite Systems Approval (COSA). Our findings are
summarized as follows:
SEPTIC SYSTEM: The septic system consists of'a 1250 gallon septic tank a trench type
drainfield. The system had been vacant for over 30 days and required an initial pre-soak. On
1/6/2011 the drainfield was inspected and found to be dry. Over a period of 395 minutes, a total
of 2600 gallons of water was introduced into the system raising the water level in the monitoring
tube 2 inches above the invert of the distribution line. The following day the drainfield was
inspected and the water level had dropped 8 inches below the invert. Water was introduced into
the system for a second time to test the drainfield. Over a period of 90 minutes a total of 630
gallons were introduced into the system which caused a rise in the monitoring tube of 8 inches~
right to the invert of the distribution line. After monitoring the drainfield for 1085 minutes the
liquid level had dropped 3 inches in the monitoring tube. This corresponds to an absorption rate
of 313 gallons per clay. Based upon this data, it was determined that the drainfield does not meet
the MOA absorption requirement of 600 GPD. Althoug~ the system may continue to be
functional, a new septic system will need to be designed .and installed prior to transferring title on
the house. The following is a summary of the engineering costs associated with upgrading the
septic system.
· Services to date for the records research, pre-soak, septic test, and preparation of this
report: $1150.00
· Engineering ser, Tices to desiD~ the septic system: $420.00*. This is based upon a
conventional gravity flow septic system with no waivers to property lines, surface waters,
or nearby wells. We intend to use the test hole data on file at the Municipati_s:y of
Anchorage for desig-n purposes.
· M.O.A. sewer upgrade permit: $530.00* Payable to the M.O.A.
· Engineering services to inspect the installation of the septic system and prepare as-
built drawings, assuming a conventional gravity flow septic system with a single
trench/bed and no sand filter: $900.00.. Upon request, we can provide you with our
standard rate sheet, which lists the inspection costs for other types or configurations of

The cost for the excavator to perform the installation is un'known at this time. After the soils
testing and design have been completed, we will provide you with a bid for the septic installation.
Upon completion of the installation we will need to prepare the MOA Certificate of Onsite
Systems Approval paperwork and deliver it to the MOA. The MOA will charge $250.00 to
process the COSA.

This is in reply to the three comment sheets. The steepest slope was measured downslope ofthe
constructed trench & the replacement trench. Analysis was conducted in accordance with the
4/17/04 guidance by James Cross, PE. A profile was drawn with the existing ground slope, the
25% slope line and a line intersecting a point 2.5' above the system extended to the existing
ground at a 35' distance.
The Tablelands, B4 L7: The existing ground dips over 25% at a distance of 15'to 29' from the
replacement trench. The overall slope stays above the 25% line. The slope ofthe line 2.5' above
the system is ~17%.
The Tablelands, B4 LI2: The existing ground dips over 25% at a distance of 25' from the
replacement trench. The overall slope dips below the 25% line at a distance of 30' from the
replacement trench. The slope of the line 2.5' above the system is (~ 21%.
The Tablelands, B5 LI: The existing ground dips over 25% at a distance of 22' from the
constructed trench. At this distance a cut-bank is present. This cut-bank requires grading to less
than a 25% slope. Spinell Homes Inc. requests that a conditional approval be granted in order for
the work to be completed next construction season.

A lot is being developed. The soil test was good. Groundwater was monitored during the
high water month of May and is not present. A conservative design has been completed,
similar to Block 4, Lot I I.
The neighboring lots are mostly developed. This site should not negatively impact any of
the other area lots. The lot slope is relatively flat and drops off in the rear of the lot.
Please review the wastewater system design for the single family home. I have included
design plans & specs, design guidelines, & soil tests. If there is need for additional
information or clarification please give me a call.

SPECIFICATIONS & DESIGN GUIDELINES
Wastewater System Sizing: This is a new 4-bedroom, single family home. Most of the
neighboring lots are developed. Groundwater was monitored during the high time of the
year and was not present. A conventional septic system is designed similar to the design
on lot I I, Block 4. This lot is over 1 acre in size and no adverse impacts are expected
from development. The neighboring lots are similar. The lots are served by a public
water system. No conflicts to the other lots will take place by this septic system
construction. The easements are located on the drawing and are not encroached upon.
Soil tests revealed sandy soil with Iow silt. An application rate of I GPD/FT: with 0.5
reduction factor for utilizing a 5' wide trench. Trench Length = 600 FT:/5'x .5 reduction
factor = 60'; I line ~ 60'. No bedrock was encountered.
Specification Requirements: All components and work must comply with the Municipality
of Anchorage Specifications (AMC) & State of Alaska Drinking Water Regulations and
Wastewater Regulations.
· Two compartment, 1250 gallon septic tank. Watertight couplings on inlet & outlet.
· 5' minimum between the tank and bed. 10' to property lines.
· 3' of cover or insulation is required for trench; an equivalent of 1" insulation for each foot
soil cover.
· Tank & solid pipe must be set on well compacted, stable soil.
· 4 inch diameter cleanouts with airtight caps are required 1' to 4' from foundation wall, prior
to any 90 degree bend in 4 inch line, in each tank compartment, and two adjacent opposing
cleanouts between the tank and the absorption field, not more than 10' from the tank
positioned to provide cleanout access towards the tank and towards the absorption field.
· All cleanouts must extend to at least ground level.
· In solid pipe runs, ASTM D-3034 may be used in lieu ofcast iron.
· Trench to be placed level, minimum of 4' to groundwater, 6' to bedrock from drain-rock.
· Drain rock to be ½ inch to 2 ½ inch screened. Drain rock to be distributed uniformly
throughout the trench.
· Perforated pipe to be installed level with perforations down.
· Silt barrier (filter fabric) to be installed above the drain rock.
· Smeared trench sides must be raked or scarified before drain rock placement.
· Backfill over drain rock must not be less than 36".
· The finish grade must be mounded to promote drainage over the trench.
· Insulation must be placed over any pipe installed under driveways or parking areas.
· Approved pipe materials include cast iron, PVC ASTM D3034, PVC ASTM F789, ABS
ASTM D2661,
Sewer Service Line is minimum 2% slope.
If the slope exceeds 25% within 50' of trenches, regrade to maintain it is less than 25%.
Septic Tank to be pumped every two years or when required.
Insulation board to be extruded direct burial polystyrene (Dow Styrofoam HI or equal)

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered In Ihe State of Alaska..Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties 'served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C wall and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
