Therefore, because of the extemely high popula— tion density in this area with subsurface sewage systems, this office will not entertain a waiver request of the horizontal well to septic distance for public water systems for homes with greater than two bedrooms or multi—family homes until there is a public sewer system or studies show that the ground water will not be affected by the high density housing. Box 196650 Anchorage, Alaska 99519-6650 Ref: Septic System for Sand Lake Subdivision #2; Lot 1, Block 2, To whom it may concern: A site visit was made on 4/12/2006 to test the septic system for the purpose of obtaining a COSA from your department. During the site visit it was noted that the septic tank is in the driveway. There are no depressions or signs of a possible tank failure around the tank. The tank is in a less travelled area of the driveway and is not driven over often. It appears that the tank is functioning properly and has functioned in this state for approximately 25 years. It is our opinion that the tank should not be driven on. Also based upon our measurements it appears that the tank is approximately 2 feet from the foundation and the drainfield is 0 feet from the foundation. The foundation in the area of the septic tank and drainfield is a slab on grade. The septic tank is approximately 4 feet deep and appears to have no impact on the garage slab. The drainfield is approximately 4-5 feet deep at the foundation and also appears to have no impact on the garage slab. The absorption trench was placed within 8' of the southern property line. No lot line waiver was applied for or granted at the time and none is currently in place. We are therefore requesting a waiver be granted allowing the trench to remain at 8' from the property line. The underlying soils on the tot are well graded sand with some gravel which percolates at rates less than 5 minutes per inch.The trench is currently in good condition and has recently passed an adequacy test. In addition, the water sample from the well serving the lot is free of contamination from bacteria, coliform or nitrates. OF HEALTH & ENVIRONMENTAL PROTECTION Nov 19 1961 RECEIVED Re: Lots 1, 2, 3, 4, 5 & 6, Block 2, Sand Lake Subdivision #2 Site Inspection Dear Mr. Hultquist: Inspection was made today to investigate the septic system installation and the well construction on the above lots as requested by Robert Pratt of the Municipality of Anchorage in his letter of November 17, 1981. (1) All standpipies from the sewer system have been installed and capped in accordance with Municipal standards. (2) The two wells are at least 12" above finish grade and are sealed and installed in accordance with Municipal Standards. (3) All six septic systems appeared to be installed adequately with no visible depressions over the tank or leaching area, except as follows: (a) Lot 4 Block 2 has some subsidence above the septic tank area. (b) Lot 3 Block 2 has small amount of subsidence around the cleanout, 2 feet from building wall. These don't appear to be serious problems, however, they should be corrected. .o. The results of the lab analysis are satisfactory as shown on the attached lab report from Chemical & Geological Laboratories. One sample was taken directly from the well located on Lot 3, Block 2, which services lots 1, 2 3, & 4 of block 2. The second sample was taken directly from the well on lot 6 which services lots 5 and 6 of Block 2 in this subdivision. If I can be of any further service, please do not hesitate to call on me.
