Sand Lake Subdivision 02
This office has become aware that there have been a number of three
bedroom homes built in the subject subdivision with on—Lot waste
discharge systems. Therefore, because of the extemely high popula—
tion density in this area with subsurface sewage systems, this office
will not entertain a waiver request of the horizontal well to septic
distance for public water systems for homes with greater than two
bedrooms or multi—family homes until there is a public sewer system
or studies show that the ground water will not be affected by the
high density housing.

Investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage rites and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines B Regulations. The reported results described the performance of the
system under the conditions encountered et the time of the test, and separation
distances measured to readily Idonfifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will It confer any legal right whatsoever.

Ref: Septic System for Sand Lake Subdivision #2; Lot 1, Block 2,
To whom it may concern:
A site visit was made on 4/12/2006 to test the septic system for the purpose of obtaining a COSA
from your department. During the site visit it was noted that the septic tank is in the driveway.
There are no depressions or signs of a possible tank failure around the tank. The tank is in a less
travelled area of the driveway and is not driven over often. It appears that the tank is functioning
properly and has functioned in this state for approximately 25 years. It is our opinion that the
tank should not be driven on. Also based upon our measurements it appears that the tank is
approximately 2 feet from the foundation and the drainfield is 0 feet from the foundation. The
foundation in the area of the septic tank and drainfield is a slab on grade. The septic tank is
approximately 4 feet deep and appears to have no impact on the garage slab. The drainfield is
approximately 4-5 feet deep at the foundation and also appears to have no impact on the garage
slab. We request you grant 2 foot waiver from the septic tank and a 0 foot waiver from the

A site visit was made on 4/12/2006 to test the septic system for the purpose of obtaining a COSA
from your department. During the site visit it was noted that the septic tank is in the driveway.
There are no depressions or signs of a possible tank failure around the tank. It is our

This waiver approval applies to the existing absorption field to property line separation
only. Any future upgrade to the on-site wastewater disposal system will require all

THIS AGREEMENT, made this 14th day of June, 2006, by and between BRADFORD C.
WARNER and their heirs, administrators, and assigns, hereinafter called "PERMITTEE", and the
Municipality of Anchorage, a municipal corporation organized and existing under its Charter and the
laws of the State of Alaska, hereinafter called the "PERMITTER".
WITNESSETH:
WHEREAS, PERMITTEE is the owner of the following described real property:
LOT 1, BLOCK 2, SAND LAKE #2 SUBDIVISON, according to the official records
thereof, on file in the office of the District Recorder, Anchorage Recording District,
Alaska, and;
WHEREAS, PERMITTER owns and/or maintains the real property more particularly
described as follows:
The sixty (60) foot right of way of WEST 80'h AVENUE, immediately to the
NORTH of PERMITTEES property as shown on Plat No. P-176, on file in the office
of the District Recorder, Anchorage Recording District, Alaska, and;
WHEREAS, PERMITTEE has placed septic vents upon the referenced real property which
encroach five (5) feet upon the PERMITTER'S sixty (60) foot right of way of West 80'h Avenue.
NOW, THEREFORE, it is mutually agreed between the parties hereto that:
1. The PERMITTER, acting through the Director of Development Services,
hereby grants to the PERMITTEE the privilege of allowing septic vents to
encroach five (5) feet upon the PERMITTER'S sixty (60) foot right of way, as
shown on "Attachment A," included herewith.
2. The PERMITTEE agrees forever to indemnify, defend, save and hold harmless,
the Municipality, its officers and employees, from any and all lawsuits, claims
or actions brought to any person for or on account of damage to property or
injury, disease, illness or death of persons, including all costs and expenses
incident thereto, arising wholly or in part from or in connection with the
existence of, alterations, maintenance, repair, renewal, reconstruction,
operation, use or removal of the encroaching septic vents, as placed upon the
PERMITTER'S five (5) foot right of way of West 80'h Avenue.
3. The PERMITTEE shall not assign or transfer any of the rights granted herein to
another individual or company without first notifying and securing the approval
of the Director of Development Services.

4. This Agreement and Permit grants PERMITTEE no interest in PERMPITER'S
real property whatsoever, except only the encroachment rights described herein.
5. The PERMITTER reserves the right to revoke this permit upon twenty (20)
days written notice to the PERMITTEE. The PERMITTEE agrees upon such
notice of revocation, to move said encroachment(s) from the West 801 Avenue
right of way in which it is placed. Should the PERMITTEE refuse or fail to
comply with said written notice, the PERMITTER may, without further notice
to the PERMITTEE, remove or cause to be removed the encroachment(s), and
the PERMITTEE hereby agrees to reimburse the PERMITTER for all costs
incidental to the removal thereof.
In addition to the mutual promises heretofore made, the PERMITTEE hereby promises to pay
the PERMITTER a one-time permit fee of $115.00.

The Anchorage Water & Wastewater utility received a request from you on June 9, 2006, for a
letter addressing AVW Lis, position regarding the proposed septic vent pipes and asphalt
encroaching Into the right-of-way located on the north portion of Sand Lake #2 Bik 2 Lt 1.
AWWU is not using the right-of-way, has no objection to the encroachment and hereby issues
this letter with stipulations to allow the encroachment of the proposed fence and retaining wall
and by using it the property owner(s) agrees to the following -
1. AWWU will be held harmless, now and forever, for any damages or injury to any person
as a result of the encroachments;
2. All applicable codes and regulations will be observed and maintained within the right-of-
way;
3. This letter will In no way preclude AWWU from -full -use and enjoyment of its rights within
any portion of the right-of-way,
4. The property owner shall pay any additional andlor.extraordinary costs incurred to
accommodate the encroachment during any future regdired construction or reconstruction
of AWWU facliities;

engineer registered In the State of Alaska. Certificates of Health Authority Approval are required for the transfer of

Absorption Trench to Lot Line
Dear On Site Services Engineer:
The septic system on Lot 1, Block J Sand Lake Subdivision No. 2 was installed in
1981. The absorption trench was placed within 8' of the southern property line. No lot
line waiver was applied for or granted at the time and none is currently in place. We
are therefore requesting a waiver be granted allowing the trench to remain at 8' from
the property line. The underlying soils on the tot are well graded sand with some gravel
which percolates at rates less than 5 minutes per inch.The trench is currently in good
condition and has recently passed an adequacy test. In addition, the water sample
from the well serving the lot is free of contamination from bacteria, coliform or nitrates.
A waiver has been granted to the septic system from this Class C Well, which is
located on the adjacent lot.
Placement of the absorption trench at its current location has not impacted welts or
adjacent septic systems. We therefore recommend a waiver be issued allowing the
existing trench in its current location at 8' from the south property line.

the drainfield at a steady rate of 6.5
gal. per minute. The tube was dry at the
beginning of the test. After adding 450
gallons the water depth in the tube was
three inches. The soil absorbed the water
at the same rate as the water was

Site Inspection
Dear Mr. Hultquist:
Inspection was made today to investigate the septic system installation
and the well construction on the above lots as requested by Robert
Pratt of the Municipality of Anchorage in his letter of November
17, 1981.
(1) All standpipies from the sewer system have been installed and
capped in accordance with Municipal standards.
(2) The two wells are at least 12" above finish grade and are sealed
and installed in accordance with Municipal Standards.
(3) All six septic systems appeared to be installed adequately with
no visible depressions over the tank or leaching area, except
as follows:
(a) Lot 4 Block 2 has some subsidence above the septic tank
area.
(b) Lot 3 Block 2 has small amount of subsidence around the
cleanout, 2 feet from building wall.
These don't appear to be serious problems, however, they should be
corrected.

satisfactory regarding the water well serving the above
subject property. However, we will need the following
additional information:
(1) After final grading, were the standpipes to the sewer
systems in place and above ground level?
(2) was there any depression over the sewer systems; leaching
area and septic tank?
(3) Dos the ofeAnchoragesandction of State ofthe Alaskaer wells meet Municipal
regulations?
The additional information submitted to this office will need
to be stamped by an engineer on a original copy.

This letter is to verify that I performed the water sampling and
had the water tested in accordance with Municipal standards. The
results of the lab analysis are satisfactory as shown on the
attached lab report from Chemical & Geological Laboratories.
One sample was taken directly from the well located on Lot 3,
Block 2, which services lots 1, 2 3, & 4 of block 2. The second
sample was taken directly from the well on lot 6 which services lots
5 and 6 of Block 2 in this subdivision.
