Septic System Upgrade Permit Request
Gentlemen:
I am writing to request that a permit to construct an upgraded septic system be issued
for this property. The proposed systems will serve an existing four-bedroom house. The
lot is served by a community water system and the well is located greater than 200 feet
away from the proposed system.
1. Soils. A single test hole was excavated on May 19, 2006. See the attached soils
log. Ground water was monitored for seven days through mid-May. A known high
ground water period. Ground water was not encountered in the test holes and did not
appear in during the monitoring period. Bedrock was not encountered in the test hole.
A percolation tests was performed. It is my opinion that the overall soils appearance of
the soils, an application rate of 6 gallons/day/square feet should be used using an
advanced treatment system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
The proposed systems will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The average topography in the area of the proposed septic system is
approximately 5 percent. The northern portion of the lot drops off at approximately 15 to
20 percent. The proposed system will be installed along the southern property line.
The existing 2,000 STEP tank and discharge basin was installed in 2003. They will be
inspected for corrosion and replaced if excessive corrosion is found. The proposed
discharge line to the new field will maintain a vertical separation to the water line of over
18 inches. The proposed installation will not affect the future development of the
surrounding or existing lots. There are no wells or septic systems within 100 feet of the
proposed septic location.
If you have any questions or concerns, please contact me at 227-3522 or 272-8218.

Septic System Replacement Permit Request - Failed Septic System
Gentlemen:
I submitted a permit application on June 1, 2002 for the subject lot. Your department issued a
permit for the installation of a newAdvantex system based on my design. The initial design was
based on a three-bedroom house. The Owner has informed me that his house has beenremodeled
to a five -bedroom house.
I am resubmitting the design showing afive-bedroom system. The Owner still intends to utilize an
Advantex system, but it will have a 2000 -gallon tank to service the five -bedroom house. The drain
field was increased in size to handle the effluent from a five bedroom house.
I request your department modify the permit to allow installation of the five bedroom system per
the revised design.
If you have any questions or concerns, please contact me at 227-3522 or 272-8218.
Sincerely,

Septic System Replacement Permit Request
Gentlemen:
My firm was contacted to investigate the possibility of a failed septic system serving this
lot. The existing trench was inundated with liquid when I arrived. The system is indeed
in failure. A single test hole was excavated on April 22, 1998 on the lot next door for that
lots upgrade. The proposed replacement trench will fall inside this test hole's 30 radius.
The soils report and a percolation test result is attached. Ground water was monitored
for seven days. No bedrock was encountered in the test hole. No ground water was
encountered in the test hole or at the monitoring on May 25, 1998. Due to the limited
area available on the lot, I propose using an Advantex Treatment System, (AX20 Pod),
with an auxiliary pump discharge unit to lift the effluent to the disposal area.
The lot is approximately 21,538 square feet in size. Lot 32 slopes to the north at
approximately 1-3 percent in the area of the test holes, and then steepens to
approximately 10 percent on the northern portion of the lot. A creek flows along the
north property line. The 100' creek set back was staked. The proposed installation will
be located on the western portion of the lot. The lot is served by AWWU water system.
The proposed location is greater than 200 feet away from existing neighboring wells and
25 feet from the water service lines. The surrounding wells are located greater than 200
feet from the proposed installation. The proposed installation will not affect the future
development of the surrounding or existing lots. See the attached design.
If you have any questions or concerns, please contact me at 227-3522 or 272-8218.
Sincerely,
—Steven . Pannone, P.E.

Engineer's Comments:
In conducting this evaluation. GEG. LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the lime of the test, and separation.
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements crib& ADEC or MOA DSD. The content o1 thus report is for
the sole benefit of the owner listed above. Any reliance upon cruse of this report by any
otherperson or partyis not authorized, nor will it confer any legal right whatsoever.

REFERENCE: Valli Vue S/D #2; Block 4, Lot 32: Configuration of Advantex System for a Five
Bedroom House.
To whom it may concern:
The subject lot is served by a community well and a private septic system consisting of a 2000
gallon S.T.E.P. tank with an Advantex AX -20 treatment system and two individual drainfields.
The Advantex system and one drainfield were installed in 2002 (SW020172). Another drainfield
was installed in 2006 (SW060176). The 2002 install was permitted and approved as a five
bedroom system by your department. The 2006 upgrade was listed as four bedrooms on the
inspection report. Per the homeowner, the house currently has five bedrooms. We request that
you approve the system for 5 bedrooms in its current configuration. The following items are
justification for granting this approval:
Your department previously approved the system as five bedrooms in 2002.
While the AX -20 treatment pod is not large enough to treat 750 gpd to Category III
standards, the likelihood of the household consistently using even 600 gallons per day is
remote. The average water usage for a four bedroom residence is typically between 250
and 300 gpd. Therefore, it is reasonable to assume that the water usage for a five bedroom
residence is less than 300-400 gpd.
The 2006 drainfield, which was tested for the current C.O.S.A. approval, has an absorption
area of 528 square feet and is oversized by a factor of 4.2 for a 5 bedroom system. (6.0
gpd/ft', minimum 125 W for 5 bedroom system, 528 ft2 actual) In short, the drainfield has
adequate size to absorb 750 gallons per day. It passed a 5 bedroom adequacy test on
6/23/2014.
In short, the system appears to be sufficient to serve the regular needs of this house and given that
your department has previously approved the existing system as 5 bedrooms, we request that your
department grant a C.O.S.A. for five bedrooms.
