Dear Mr. Poet,
Eagle River Engineering Services (ERES) is in receipt of your comment sheet for the Inspection
Report a noted above. The following additional information and clarifications are being
provided to you so that you may continue your review of this Inspection Report.
1) A copy of the well log is being provided.
2) A copy of the pump installation log is being provided.
3) Please see modified site plan re -locating the reserve site to a more acceptable location as
requested by the Muni. Very little, if any fill will be required, as the primary leachfield site and
the fill area below it create the required 35' at 25% downgradient of the proposed reserve site.
4) A copy of the electrician's statement is being provided.
5) The homeowner has indicated that he will be seeding the fill area below and above the
leachfield. Any fill areas in excess of 1.5 to ]I slope shall utilize "jute" or burlap -type
erosion control fabric Installed and anchored per manufacturer's installation
recommendations.
6) Please see above note regarding seeding of leachfield and fill areas. The area on top of the
drainfield will be seeded by the owner.

Septic Tank/Well Waiver Application
Dear Mr. Roth:
After receiving the certified as -built survey for the above referenced property, it was noticed that
the well was drilled too close to the septic tank installed on the lot.
The purpose of this letter is to request a waiver for the separation distance between the installed
Advantex tank and to the drilled well. The distance we are requesting to be waived is from 100'
to 85', well to septic tank. We offer the following points in favor of granting this waiver
3. The gradient from the well to the septic tank is downhill. Effluent from a
malfunctioning septic tank would flow in a direction away from the well head.
4. The well is deep and confined in bedrock from a depth of 8 feet below ground
surface. The well casing is seated 36 feet deep into the bedrock, effectively sealing
the water supply from any potential surface contamination.
For the reasons listed above, we believe that granting this separation distance waiver of 85' from
septic tank to well will not impact adjacent properties' ability to support on-site septic systems
and reserve sites. To our best available knowledge, granting of this waiver will not affect the
function or longevity of this lot's or neighboring septic systems, nor impact water quality. If you

Narrative & Permit Application.
Dear Mr. Roth:
Eagle River Engineering Services (ERES) was contacted by the owner of the property noted
above to design a well and septic system to serve a new 4 bedroom house Test holes on the
upper part of the lot indicated free -draining soils from 1 to 4 feet below ground surface, with
shallow bedrock. One test hole was excavated lower down on the property, and was found to
have 8' of soil above the bedrock layer. We propose to design the new drainfield at this lower
test hole location since it has a deeper soil deposit. Fill material will have to be imported on
order to create the full 35' slope setback at 25% as required by the municipality of Anchorage.
Water table monitoring indicated no water table to a depth of 8'. Advantex technology shall be
utilized to minimize construction costs and to provide better quality effluent. The leachfield and
septic tank shall be sized for 4 bedroom single family residence.
The topography at the leachfield site is a consistent 15%-20% on a natural bench. Below this
bench, the slope increases to nearly 60%. Fill shall be imported to create a slope of 24% for 35'
below the bottom edge of the drainfield.toward the southwest. ERES is not responsible for any
retaining wall design that may be required for stabilizing this fill wedge.
A natural drainage gully is present along the southern boundary of the parcel. No surface water

The homeowner installed a curtain drain and manhole for his foundation drain. We have shown
its location on our site plan. This was not an engineered curtain drain, and it is not for the
purposes of lowering a water table to create useable soil for a septic system.

The proposed 4 bedroom septic system and well will have very limited impact on adjacent
properties for the following reasons:
1. The surrounding lots are large and currently vacant, so there is room for wells,
septic systems and alternate sites.
2. Drainage will not be affected and is not a major consideration in our design.
Installation of this 4 bedroom Advantex septic system and well, to our best available knowledge,
will not adversely affect the wells or septic systems or reserve areas on adjacent lots. If you have

LEGAL: Broadwater Heights Lot 2 Block 4

A. GENERAL
1. The septic plan is for a3 or 4 bedroom single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of health and State Department
of Environmental Conservation requirements.
4. All soil tests are advisory to the design and arc to be verified or modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or casements and to locate any
adjacent multi -family wells. A licensed surveyor should locate any utility easements and exact 100'
radius from the existing and neighboring onsite wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. Any remaining open test hole excavations shall bctillcd and monitor tube removed.
B. ADVENTEX SYSTEM AND SEPTIC TANK
1. Septic tank shall be a fiberglass or steel 1500 -gallon tank capacity of MOA approved construction for
use with Advantex system. Install as per current manufacturers recommendations for conditions in
Alaska.
2. Install one AX -20 filter pod over septic tank capable of treating600 gallons per day of waste flow.
3. A municipal inspection report or receipt from a licensed electrician shall be provided to the
engineer verifying lift station wiring b all applicable codes
C. DRAINFIELD
1. The drainficld is to be located as shown on the site plan.
2. The total depth of the initial drainficld excavation is to be at a maximum 2' deep, or as required to
remove the topsoil layc; and end in native sandy SM type soil.
3. Continuous heavy mil visquccn shall be placed full depth of the sand and sewer rock on the downhill
edge of the leachficld. Leveling sand, approximately 0.5' average depth will be required to level the
trench after removing topsoil layer.
4. A one foot (1.0') layer of sewer rock is to be placed over the level sand layer. The 4" effluent piping is
to be buried within the gravel layer with 2" of gravel cover over the piping, and 1.0' effective depth
beneath the effluent pipe
5. The completed drainfield gravel and piping is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of Y or equivalent is to be placed
over the bed. Mounded side slopeson uphill side and ends are not to exceed 3:1.
7. The septic tank and lcachficld must not be closer than 100' to any existing private well, 150' to any
Class "C" well, or 200 feet to any community well.

During a recent COSA on-site inspection and test of the potable water
supply well on Block 4, Lot 2 of Broadwater Heights subdivision, the well's
productivity was determined to be 0.64 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 4 -bedroom
residence is 0.41 gallons per minute. Although the subject well currently
exceeds this minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.

Certificate of On -Site Systems Approval (COSH). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC" 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AW WTS that were included
in the original design which would allow the AW WTS to continue to
perform as designed.
Permit. An On -Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AW WTS.
3. TPS. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AVMM is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.

septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service at hent Wirt+ as AW WTD
manufacturer of the AW WTS for the entire term of the AW WTO In addition, it
shall be the responsibility of the Owner during the teem of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AW WTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acirnowle dges that the fine for failing to maintain and repair an AW WTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AW WTS upon 24 hours written notice.
Cs. Owner agrees that any sate or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
H. Owner agrees that the AW WT$ installation and maintenance requirements as
provided by the AW WTS vendorlinstailer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AW M.
6. Nonwsiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any effect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereaft to enforce each and
every provision hereof.

A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are.

representative or unauthorized means shall be void
S. Jurisdiction! Choice of I.aw. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
g, QPVei W ity, Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.
