Box 196650 Anchorage, AK 99519-6650 RE: Prator SID, Block 2, E/2 Summary of Work Performed To Whom It May Concern: A conditional COSA was issued on 1-27-09 for the subject property, with the requirement that additional cover be installed over the trench by 6-15-09. This work has been performed. The tank was recently changed out, and the inspection report and asbuilt drawings are attached. A Quanics Flow Director was installed after the tank to distribute flow to the trenches, in proportion to their absorption area. Cleanouts were installed to allow for cleaning the flow lines but not going through the Flow Director. An abandoned well was uncovered, as described in my letter dated May 20, 2009. The well has been capped with a steel plate. The well appeared to be plugged with sand. The approximate location is shown on the asbuilt drawing. The well has now been decommissioned as approved. There is no documentation on the abandoned well. It went unnoticed previously, because it was buried. It appears to be plugged with dirt at the surface. The well log for the well in service shows 20 ft of casing, into bedrock, with a total depth of 225 ft. It was drilled in 1981 by Penn Jersey Drilling Co. The recent water quality tests in 2006 and 2008 had no bacteria, no Arsenic, and nitrates at 1.85 and 2.86 mgA. discussed the situation of the abandoned well with Brian Wille of Aarow Pump and Well Services of Anchorage. He recommends that the abandoned well be decommissioned by leaving it above grade and welding a steel cap on. Cutting it off below grade may present a hazard. He did not recommend grouting the well, as it would probably damage the existing well, due to the close proximity. Therefore, we plan to decommission the abandoned well in this way. A repair job is also planned on the septic system, as we discussed on the phone. This will not need a permit, but I wish to document k. There are 2 trenches in the drainfield; one (1981 installation) has 670 ft2 of absorption area and the other (1985 addition) has 200 ftZ. The trenches were previously separated by a diverter valve, but it was removed and a 'Y" installed, as approved in 2006. During the septic adequacy test in October 2008, ft was noted that the smaller trench was surcharged. The larger trench was tested and passed. A repair is now planned to replace the Y with a Quanics flow director which can be adjusted from the surface to split the flow variably between the trenches. The flow can be adjusted preferentially to the trench with the larger absorption area. will notify you when the work on the well and the septic system has been completed. The recent water quality tests in 2006 and 2008 had no bacteria, no Arsenic, and nitrates at 1.85 and 2.86 mg1l. discussed the situation of the abandoned well with Brian Wille of Aarow Pump and Well Services of Anchorage. He recommends that the abandoned well be decommissioned by leaving k above grade and welding a steel cap on. Cutting h off below grade may present a hazard. He did not recommend grouting the well, as It would probably damage the existing well, due to the close proximity. Therefore, we plan to decommission the abandoned well In this way. A repair job is also planned on the septic system, as we discussed on the phone. This will not need a permit, but I wish to document It There are 2 trenches in the drainfield; one (1981 Installation) has 670 ftz of absorption area and the other (1985 addition) has 200 W. The trenches were previously separated by a diverter valve, but it was removed and a "Y" installed, as approved in 2006. During the septic adequacy test In October 2008, it was noted that the smaller trench was surcharged. The larger trench was tested and passed. A repair is now planned to replace the Y with a Quanics flow director which can be adjusted from the surface to split the flow variably between the trenches. The flow can be adjusted preferentially to the trench with the larger absorption area, will notify you when the work on the well and the septic system has been completed. Box 196650 Anchorage, AK 99519-6650 RE: Prator S/D, Block 2, East 112 Septic System Test for COSA To Whom It May Concern: Attached is a COSA application for the referenced 4 bedroom house. The inspection report indicates that this system was initially installed in 1981 and modified in 1985 by adding a trench extension. In 1987, the trench extension was exposed and further documented by Tobben Spurkland, P.E. This letter will serve to further document the construction of the system and detail the septic test performed. The original trench is 67 ft long, located along the north property line, and has a sump at the end of it. On June 29, 2006, we measured the level of water in the trench above the lateral line, indicating that it is surcharged. The "trench extension" added in 1985 is 20 ft long, constructed in an V shaped configuration. There is a monitoring tube at the crook of the "l." and one at the end. There are no cleanouts in the trench extension. This trench is separated from the original trench with a diverter valve and had been out of use for many years. The owner indicated that they had previously used this trench without problems. On June 29, 2006, this trench was dry, as measured in both monitoring tubes. The trench extension was presoaked with 1258 gallons of water on June 29, 2006 by introducing 733 gallons of water from the well into the south monitoring tube, and 525 gallons of water into the north monitoring tube from a truck.. COSA Application for PratorBlk 3 E/2 Watkins Engineering, Inc.; July 24, 2006; page 2 On July 1, 2006, the trench extension was tested. Both MTs were initially dry, having absorbed all of the presoak water. A total of 240 gallons of water were introduced into the south monitoring tube (MT2), which caused a rise in the tube of 88.5 inches. Within 30 minutes, the level dropped to 60.25 inches, indicating that the initial level of 88.5" was abnormally high due to introducing water into the same tube as the measurement was from. Within another 64 minutes, the water level was at 45 inches, indicating absorption of greater than 600 gallons per day. Within 15 hours, the monitoring tube was dry. Water was also introduced into the north monitoring tube (MT1). A total of 421 gallons of water caused an increase of 37 inches. Within 40 minutes, at 8:00 pm, the water level had dropped by 2.25 inches. By the next morning, after 13.7 hrs (820 minutes), the monitoring tube had 24 inches of water in it, indicating absorption of approximately 260 gpd. Based upon the above reported data, I believe that the absorption rate of the trench extension is greater than the required 600 gpd for 4 bedrooms. If you need further information, please call me at 349-1851. Yours truly, Cindy W. Ellis, P.E.
