The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given In paragraph 4 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of on -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of on -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineers work.

Investigation, based on procedures outlined In the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure Indicated herein. I further verify that based on the
Information obtained from the Municipality of Anchorage files and from my investigation and inspection, the

and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG, LID. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines 8 Regulations. The reported results described the performance otthe
system under the conditions encountered at the time of the test, and separation
distances measured to readilyldentirtable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of Ihls report Is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any

Certificate of On -Site Systems Approval # HA060357
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on N2 of Lot 34, of
T12N R4W SEC 33 Subdivision. This inspection revealed an arsenic
concentration of 27.8 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On -Site Water and Wastewater Program
website (www.muni.org/onsitg) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Certificate of On -Site Systems Approval # HA060357
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on N2 of Lot 34,E of
T12N R4W SEC 33 Subdivision. This inspection revealed an arsenic
concentration of 27.8 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information Ion

This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Septic Tank Data
The original septic tank is a concrete tank and has been in use since
before 1958. There was a new steel septic tank installed in 1976
adding compacity to the original concrete tank. The concrete tank
and the newer steel tank was permitted and approved in 1976, but
was not formally waived by the On-site department. The total depth
of the tanks is 10 feet for purpose of calculating points.
Well Data
The well was installed some time before 1958. The total depth of the
well is unknown. There is casing to a depth of 40+ feet determined
by Garnees Engineering. For the purposes of calculating waiver
points the well log from Lot 35 sec 10 was used. The well depth of
this well is 245, and is one of the shallower well logs provided. The
well logs submitted by the Gayness Engineering Group shows there
to be a consistent soils strata in the area. This well to septic tank
encroachment has existed for more than 30 years and the water
sample results are with in municipal standards.

The existing 3 bedroom house is served by a private well and septic system. We request you
grant a 50 foot separation distance waiver from the well on the referenced property to th i septic
tank. The following items are justification for the waivers:
• The lots are generally flat in the area. There is a slight slope traveling from the well on the
subject property to the septic system. If the septic system was to overflow, it appears that the
effluent would not travel toward the well head. The house is also between the well and the
septic tank. Which would also prevent it to run towards the well, if the tank began to leak.
• The location of the septic system is in a very visible area so that if any effluent was to
surface, it would be noticed and the problem corrected.
The other path of contamination is subsurface migration wastewater should the tank begin to
leak. Recent water sample results indicated nitrate levels and coliform bacteria results to be
satisfactory. Attached are well logs from surrounding properties showing that the average depth
of the wells are relatively deep, ranging from 230 to 442 feet, with many layers of gravel and
silty sands, which would prohibit a septic leak. Based upon the aforementioned facts, it i ppears
that there is minimal risk associated with the 50 foot separation distance waiver.
