An On -Site Water/Wastewater Permit, number SW070322 issued by this office for a single-family
system, will expire on 12 December 08. Permits are valid for 365 calendar days from date of issue.
If this was a well permit and the well has been drilled, a well log and a pump install log must be sent
to this office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

Well and Septic System Permit
We recently cross sectioned the slopes fronting the proposed absorption trench on Lot
10, Block 1, Susitna View Estates Subdivision. We then plotted the 25% slope from 2'
above the distribution line to its daylight point on the slope. The distance on Section
No. 1 is slightly more than 35'. At Section No. 2 the 25% slope does not daylight. The
results of our survey and the cross section of the slope is shown on the attached
drawings. In addition we are submitting a request for a waiver to allow the separation
distance to a slope greater than 25% to be reduced to 35'.

Absorption Trench to Slope Waiver
Dear Onsite Services Engineer:
Lot 10, Block 1, Susitna View Estates Subdivision has limited area for construction of a
septic system due to steep slopes and the presence of a stream to the east. We have
located an area with acceptable soils, but it is situated slightly more than 35' from a
break in grade greater than 25%. We are therefore applying for a waiver allowing
placement of the absorption trench at this location.
We have designed an Advantex Treatment System f
requirements for the absorption trench. The use of this
possibility of contaminated effluent surfacing on the face

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer.
The owner of Lot 10, Block 1, Susitna View Estates Subdivision intends to construct a
four-bedroom home on the lot. We are therefore requesting a permit be issued for a
well and septic system to serve the new home. The available area for the septic system
is limited due to the location of wells in the area, step slopes and the stream to the west
making it difficult to place a conventional septic system. We have therefore decided to
place an Advantex Treatment System on the lot to provide the most effective treatment
of the septic effluent prior to discharge into the surrounding soil strata. The location of
the absorption system is shown on the attached Site Plan. Drainage patterns are also
shown on the plan and will not be modified as a result of the proposed construction.
The test holes placed on this lot revealed silty gravel with some sand. The percolation
rate in the materials encountered was close to 60 minutes per inch. Groundwater
seeps were found between 15' and 16' during the excavation but were noted at 12'
below the surface during monitoring. Our design provides for the bottom of the
absorption trench to be as shallow as possible. We are therefore proposing to place a
four-bedroom Advantex Treatriient System. The absorption trench will be 60' long x 5'
wide x 4' effective depth. The distribution line in the trench will be placed at 2' below
the existing ground level. Total depth of the trench will be 6' below the surface. 3' of
fill will be placed above the trenches and 4' over the septic tank to meet Municipal Code
and prevent freezing.
The ground surface of the lot slopes at 30% to the west from the Feodosia Drive, but
becomes shallower near the location of the primary absorption trench. The trench will
be constructed parallel with the slope as much as possible in conformance with
Municipal requirements. All components of the new septic system will be constructed
a minimum of 100' from any surface water and 100' from the well proposed for this lot.

No conflicts exist between the proposed well and septic system and those in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

ONSITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 10, BLOCK 1, SUSITNA VIEW ESTATES SUBDIVISION
GENERAL:
1. The scope of this project includes the procurement and placement of a
new Four Bedroom Advantex Treatment System with an AX -20 -Mode
1a 40x94 Pod to serve the new home on the lot. System will be
placed at the location shown on the Site Plan. The new septic system
also includes the construction of a new 60' long X 5' wide X 4' effective
depth absorption trench at the location also shown on the Site Plan.
The distribution line in the trench will be placed 2' below the existing
ground surface. The total depth of the trench will be 6' below the
surface. The new system must be placed a minimum of 100' from the
proposed well on the lot and those on adjacent lots and 100' from any
surface water located on or near or on the lot.
2. Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. The Advantex Treatment System must be installed in conformance
with specifications and regulations furnished by the manufacturer,
Anchorage Tank and Orenco Systems Incorporated. No variances
from the manufacturer's specifications will be allowed without
permission from the manufacturer and Municipal representatives.
2. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.

3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.
6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the bed shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
6. Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.

MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5' to 2.5" in diameter with less than 3% passing
the #200 sieve.
SAND GRADATION:
1. Sand used to level the new absorption bed must have the following
gradation:
50% Passed the No. 4 Sieve
2% Passed the No. 100 Sieve
1% Passed the No. 200 Sieve
2. The sand source must be approved prior to use.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.

Anchorage General Permit Conditions:
General conditions:
1. Water Quality Certification. You must comply with all conditions specified as part of the Alaska
Department of Environmental Conservation Water Quality Certification, which is part of these
General Permits (GPs).
2. Historic and Archaeological Sites. These GPs do not authorize activities, Including the use of
material borrow sites, In or that adversely affect known historic properties listed or potentially
eligible for the National Register of Historic Places, nor any such sites listed or found to be
potentially eligible in the future. The permittee shall stop work when cultural resources are
Inadvertently discovered during the course of work and notify the Corps. The Corps will enter Into
consultation with the Alaska State Historic Preservation Office to determine If historic properties will
be affected by the permitted work. Work shall resume after cultural resources have been evaluated
pursuant to Section 106 of the National Historic Preservation AcL
3. Inspections. You must permit the District Engineer, or his designated representatives, to Inspect
the authorized activity at any time deemed necessary to ensure that it Is being, or has been
accomplished In accordance with the terms and conditions of these GPs.
4. Maintenance. You must maintain the authorized activity In good state, and in conformance with
the terms and conditions of these GPs. You are not relieved of this requirement even if you
abandon the authorized activity, although you may make a good faith or legal transfer to a third
party, as described below. Should you wish to cease to maintain the activity or abandon it without
a good faith transfer, you must obtain a modification of the GP authorization from the Corps
Regulatory Branch either directly, or through Municipal Planning Staff. Such modification may
require restoration of the area.
6. Sate or Transfer of Property. If you sell or transfer the property associated with the authorized
activity, you must obtain the signature of the new property owner as the transferee In the space
provided on this document and submit a copy of that page to the Municipal Planning Department
and/or Corps Regulatory Branch, along with appropriate documentation of the transaction. You

must submit this Information within 30 days of the property transfer, to validate transfer of the GP
authorization to the new party.
Project site restrictions and design criteria:
1. Construction Timing Window. To protect nesting migratory birds, as required by the Migratory
Bird Treaty Act, no work p.e., Gearing, grading, excavation• stockpiling, or placing of fill) may occur
on the site between April 15 and July 15, unless: a) the site is sufficiently disturbed prior to April 15
to eliminate suitable nesting habitat: and, b) construction activity will occur continuously through
June 15.
Note., Clearing orotherdisturbanceofthe site prfortoApril 15may require Corps orother
authorization.
2. Local Flooding. The project shall Incorporate design and construction featureshnethods
necessary to ensure that the proposed project will not cause flooding of adjacent properties or
public Infrastructure.
3. Stormwater Functions. The applicant shall demonstrate that the project plans Include sufficient
measures to effectively replace the she's pre -project stormwater retention and filtration capacity.
The measures shall be sufficient to approximate zero net runoff increase for storms with 2 -year
frequency and no more than two times net runoff Increase for storms with a 10 -year frequency,
including flows from subdrains. Verification that the project's stormwater design meets this criterion
must be obtained from the Municipal Wetland Management Section or its designee, and must be
Included with the GP application package.
Project -specific conditions:
1. Time Umit GP authorization of this project expires on: September 6, 2008. If you need
more fime to complete the authorized activity, submit a request for extension under these GPs to
the Municipal Planning Department prior to the expiration date. You may not continue work beyond
the expiration date until receiving notification to that effect from the Municipal Planning Department
or the Corps Regulatory Branch.

4. Offset Unavoidable Impacts. To offset the unavoidable adverse Impacts of the project to the
aquatic environment, the permittee:

Environmental Conservation for Instructions, and defer any further activity until you receive ADEC
approval to continue.
8. Endangered Species. The activity must not jeopardize the continued existence of a threatened or
endangered species, as identified under the Endangered Species Act, nor endanger the critical
habitat of such species.

THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
ADVA.iTay, 7-a-c4T&ts-" —
Treatment System (AWWTS), described as Sy Srtsm located

2. Definitions. 957*-rES
A. Alteration. Any change to the design or function of an AW WTS that
Includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AW WTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
These approvals certify that the systems are adequate for the homes that
they support and meet the codes that were in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Ntaintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction) An On -Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating) An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On -Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. Fee. Owner shall pay to Municipality an annual fee of /jO�j L-
($ 0.00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Tenn. The term of this Memorandum of Understanding shall be for the life of the
AWWTS.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On -Site approval is issued to
the new owner or transferee of the property.
5. Alterations, Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AW WTS

C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification guidebook
for AWWTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AWWTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.
