Apparently S&S engineering removed the 1000 gallon tank they had just earlier installed and replaced it with a 1250. they also extended the existing absorbtion field. S&S Engineering apparently made these changes because they mistakenly thought the house was a 3 bedroom home. -The original health authority done by S&S engineering was submitted to our department approximately on 9/20/02. The adequacy test that was done showed there was no fluid in the trench. Water was added to the trench and after a period of 60 minutes the trench returned to the original fluid level of 0 inches according to S&S Engineering. Dan Roth did a follow-up inspection to verify the numbers on S&S Engineering's adequacy test. Dan had found there was +20" in the trench, which would assume the adequacy test S&S had performed was not done correctly. S&S also applied for a waiver with the submittal of the HAA. They requested a foundation to septic tank waiver of 1 foot. During there inspection they did not examine the as -built information close enough, which revealed the tank was actually 6' away from the foundation. This would mean S&S engineering did not need to request this separation distance waiver. And finally the well that was inspected by S&S engineering did not have a sanitary seal, which is required to be fixed at the time of IiAA. Dans's site inspection of the IIAA found that S&S engineering made mistakes in their HAA and adequacy test. S&S engineering returned to the site to redo the HAA and adequacy test. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. . 4. Septic tanks installed with less than 4 fl. of cover shall be insulated. 5. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. 6. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. LEACIIFIELD BED INSTALLATION: Excavate the proposed Bed Area to the depth shown on the design. The bottom of the excavation shall be within 2 inches of level. If the bottom of the excavation becomes smeared, it must be raked or scratched (ruffed -up) before gravel or sand placement. 2. If a sand layer is required, place sand over entire excavation to the required depth shown on the design. The top of the sand layer must be within 2 inches of level. 3. Sewer rock shall be placed uniformly throughout the entire bed. The distribution piping is to be of PVC (ASTM D3034 or equal). All joints are to be solvent cemented. Gravel depth below the distribution pipes shall be a minimum of six (6) inches. Gravel depth above the perforated pipe shall be a minimum of two (2) inches. 4. The distribution piping must be as shown on the design. The distance between the outermost perforated distribution pipes and the sidewall of the absorption bed must be no more than three feet. 5. Silt barrier material must be installed between the final gravel layer and the native soil backfill, unless insulation is used, then the silt barrier should be installed over the insulation. Ensure the silt barrier covers the entire gravel surface before placing backfill. 6. Monitor tubes shall be of four (4) inch diameter, installed at the locations shown on the design, and extend a minimum of 12 inches above final grade. The portion of the monitoring tube extending through the gravel depth shall be perforated six (6) inches below the bottom of the horizontal distribution lines. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finished grade over the bed must be mounded to prevent the formation of a depression after settling. MINIMUM MATERIAL SPECIFICATIONS: 1. Any septic tank proposed for installation must be constructed by a Municipally approved septic tank manufacturer. 2. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. 6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the #200 sieve. 7. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements, which ever specifications applies. Page I Lot 7, Block s, Preuss subdivision 13 November 12, 2002 INSPECTIONS: Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. 2. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. 3. The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre -construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractors activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractors activities. Final acceptance of the contractors work rests with the owner and the M.O.A. WELL 94SPECTION S now TEST The mitigating factors involved which support the issuance of the waiver are as follows: 1. The topography of the referenced property slopes from north to south SITEPLAM which would prevent any surfacing effluent from flowing toward the well. 2. Septic tanks are not considered a continuous source of contamination & would not normally contribute contamination to the soils. ROADOESN;H 3. The well log (attached) for the referenced property shows clay and gravel layers which would confine any released sewage from this well's water SM TEST bearing sand and gravel at about 12 feet to 25 feet & again at 126 feet to 165 feet and 261 feet to 312 feet. 3. Water samples taken from the well located on the referenced property PEROOLAnM TEST indicated 0.223 mg/I for nitrate and 0 colonies/100 ml for bacteria. 5. The septic tank to foundation separation waiver distance requested is 1 foot. STRUCTURAL& This tank has endured the test of time, and no problems are evident. The attached well logs from surrounding properties show static water levels from 292 feet to 347 feet. This indicates the water table slopes to the west at approximately 12%, which would carry effluent away from the well. 7. An original report submitted previously by another engineering firm, citing the waiver distances requested, was previously approved which supports the acceptance of these waivers. We therefore recommend a waiver for the 82 feet separation distance between the existing private well and the existing septic tank, and the 1 foot separation distance between the existing septic tank & the existing foundation. Total coliform test on the water was satisfactory with a zero count. The adequacy test on the grainfield was marginally acceptable for a four bedroom house. The following discrepancies were noted: 1. Distance from the well to the septic tank is about 82 feet. The minimum regulatory distance is 100 feet. The drain line is 123 feet from the well, so the contamination should be sufficiently far from the well. 2. The septic tank seems to be less than two feet from the foundation, but has not caused any evident problems in this four year old home. It is not practical to move the tank as it is under a building overhang and a porch. 3. The 1000 gallon septic tank was adequate for the original three bedroom home before the 1981 expansion that added one bedroom. As the present family consists of only four people, the tank should be adequate. The design criteria for new systems is to assume two people per bedroom and a four bedroom house should have a 1250 gallon septic tank. If a family with more than six people were to use the system, a supplementary tank should be added in series with the existing tank. As the septic system is marginally acceptable, I recommend that the septic tank be pumped yearly to help extend the life of the drainfield.
