Ref: Septic System Upgrade for Lot 23, Eklutna Heights Stewart Addition Subdivision
To whom it may concern:
The existing 4 bedroom house is served by a private well and septic system. The existing septic
system consists of a 1000 gallon septic tank and a deep trench type drainfield. The existing septic
system is in a state of failure and must be upgraded. A test hole was excavated west of the
existing septic system in the area of the proposed septic upgrade. The septic system will be
designed around the 30 foot radius of this test hole. We are proposing that a new 1250 gallon
septic tank and a dual deep trench type drainfield be installed. Comments regarding the design
are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. TRENCH DESIGN:
a. Percolation Rate: 8 minutes/inch
b. Allowable Application Rate: 0.8 gallons/day/112
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 750 it2
f. Total Depth: 9.5 feet (max.)
g. Effective Depth: 5 feet
h. Width: 2.5 feet
i. Reduction Factor: N/A
j. Minimum Length: 76 feet total length (2 @ 38 feet long each)
k. Effective absorption area = 760112
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.

4. TOPOGRAPHY: As can be seen on the attached design drawing, the average topography of
this property is a 1% to 5% percent running from approximately west to east.
5. LOT LINE WAIVER: We request a I foot lot line waiver to the west and south lot line.

is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.

Approval (COSA) based only upon the representations given in paragraph 4 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also Issues COSAs upon request to homeowners. Certificates of Onsite Systems
Approval are valid for 90 days from the date of Issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LID. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time ofthe test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the

Certificate of On -Site Systems Approval # 060548
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
23 of Eklutna Heights Stewart Addition subdivision. This inspection
revealed a nitrate concentration of 5.4 milligrams per liter (mg/L) was
reported for the property's well water sample. The Environmental
Protection Agency (EPA) has established a maximum contaminant level
(MCL) of 10.0 mg/L for public drinking water systems. While private wells
are not subject to this regulation, EPA standards are based on existing health
information and can therefore be used to gauge the relative quality of water
from private wells. Please see the attached "Nitrate Fact Sheet" for
important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Ref: Waiver request for Eklutna Heights Stewart Addition; Lot 23.
To whom it may concern:
We request that your department issue a 1 -foot waiver from the existing septic tank and
drainfield to the garage addition. The garage consists of footers that are approximately I foot
deep and with a dirt floor. The garage slab has not been poured at this time. The septic tank and
drainlield are approximately 34 feet below grade and should not interfere with the garage slab or
footers. We are unaware of any adverse impacts this waiver would have on the septic tank and

engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or 6 wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.

In conducting this evaluation, AKWWC, Inc. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AKW WC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSO. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

During a recent Health Authority Approval on-site inspection and test of the
potable water supply well on Block , Lot 23 of Eklutna Heights
Stewart Addition subdivision, the well's productivity was determined to be
0.56 gallons per minute. The minimum well productivity required by this
Department (AMC 15.55) for a 4 -bedroom residence is 0.41 gallons per
minute. Although the subject well currently exceeds this minimum
requirement, all parties concerned are advised that the production capacity of
the well may fluctuate. Restriction of non-critical water uses such as
washing cars and watering lawns and gardens may be required.
This advisory must be attached to all copies of the subject Health Authority
Approval.

Test Procedure:
This well was inspected and a water flow was established to determine the
operating capacity for a 3 -bedroom residence. 'I lie well casing is approximately
28 inches above ground with drainage away frim the well head. The well casing
sanitary seal is adequate for protection from contamination. The static water
level at the beginning of our test was 170.5 feet below the top of casing (TOC).
The initial flow rate of 2.35 gallons per mirnte (gpm) was reduced and then
increased as the recovery rate was maintained. An average rate of approximately
0.62 gpm was maintained during the last two and a half hours with minimal well
drawdown and full recovery. It is our opinion that this will produce sufficient
quantities to meet the municipal and state requirements for this three-bedroom
residence.

Test Results:
This well will produce at least 0.62 gallons per minute for more than four (4)
hours. The entire water system can deliver approximately 900 gallons of water
per day.
The Municipality of Anchorage and the State of Alaska, Department of
Environmental Conservation requirement for well flow 3�; 150 gallons of water
per bedroom per day. This well exceeds this re�ui ement. The assessment of the
condition of the well applies only to the conditions as of the day tested. The flow
rate may change due to subsurface conditions that may not be observed from the
surface, and changes in the land use and other factors that may impact the
aquifer feeding the well.

ENGINEERING STUDIES
1. The horizontal separation distance from the well to the septic tank is 95 feet.

Five percent of the required separation distance is requested to be waived.
2. The samples taken from the well located on the referenced property indicated

3. The referenced lot is on a 20% slope away from the well head towards the
septic tank. The house is also located between the well and the septic tank. This would

4. From the well log on the referenced property we can see that the well depth is
400 feet. The static water level was reported to be 170 feet. The distance between the

tank and the water table exceeds 160 feet.
5. The soils on the well log for the referenced lot indicate clay/gravel and

sand/gravel between the ground surface and the aquifer. Also attached are neighboring
well logs.

A water and well inspection was performed on this property. The
well casing was examined and found to be equipped with an adequate
sanitary seal and all wires were in conduit. The ground around the
well casing adequately sloped away from the well. At the same time
this inspection was performed, a water sample was taken from the hose
bib at the front of the house and submitted to Chemical and Geological
Laboratories of Alaska for coliform bacteria analysis. The results of
this test were satisfactory.
