Subject: Rabbit Creek View and Heights, Block 3H, Lot 14A – 16373 Diane Drive
Septic System Design
Dear On-Site Services Engineer:
The absorption trench on the subject lot has failed and must be replaced. We are therefore
requesting a permit be issued to allow the construction of a new septic system. The system will
be designed and constructed to accommodate four bedrooms even though the house currently has
only three. The existing Advantex treatment tank and pod was placed in 2014 and will remain in
service. The existing absorption trench will be decommissioned and abandoned in place.
The attached Site Plan and backup documentation identify the location and configuration of the
existing well and septic system and proposed absorption trench. No existing wells are less than
100’ from the proposed septic system and the well on the lot is more than 50’. The surface drainage
on the lot is to the northwest as shown by the surface contour lines on the plan. The drainage
pattern will not be changed as a result of this project.
The new absorption trench will be constructed to accommodate horizontal conveyance to transport
the effluent through the organic material surrounding the absorption trench and away from the
area. The distribution line in the trench will be placed at 1.5’ above the existing ground with the
bottom of the drain rock at 1.0’ above ground to provide 3.0’ of clearance from the in situ silt
below which has been found to be impermeable based on a test hole completed on the lot in June
of this year. Other test holes completed on the lot indicate permeable material may be encountered.
The water table was recently monitored at 3.5’ below the surface. A minimum of 3’ of MOA
approved coarse sand will be placed below the trench and above the in situ layer. A 1’ sidewall
composed of organic material and peat will be utilized for the horizontal conveyance.
A subdrain is in place on the lot near the proposed absorption trenches location. This drain will
be removed in its entirety to a minimum of 10’ from the new trenches. The excavated area will be
backfilled with in situ material. The backfill material will be compacted to the same extent as the
in situ material currently in place.
The trenches will be placed a minimum of 5’ from all lot lines and more than 10’ from the house
foundation and foundation drain. We are requesting the side slopes of the mound over the trench
be constructed with 2:1 slopes to minimize the area required for the mound. The slopes will be
stabilized with landscaping fabric, topsoil and grass seed after construction.
The ground surface on the lot in the area of the absorption bed slopes to the northwest at a shallow
grade. The new trenches will be constructed on a flat section of the lot, but slightly out of parallel
with the contours of the slope. MOA approved sand will be utilized as necessary to level the
trenches above the in situ GM/ML material. The new septic system will be constructed a minimum
of 50' from all wells and any surface water. If the system is constructed in accordance with our
design it will not conflict with adjacent wells and septic systems.

THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this

herein the "O ER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AW WTS), described as

Alteration. Any change to the design or function of an A W WTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AW WTS that were included
in the original design which would allow the AW WTS to continue to
perform as designed.
Permit. An On -Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AW WTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AW WTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations, Installation and Removal of Additional Euuipment. Prior to
performing any alterations to an AW WTS, the owner agrees to obtain an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AW WTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AW WTS
service and maintenance provider approved by the municipality and the
manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
H. Owner agrees that the AW WTS installation and maintenance requirements as
provided by the AWWTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AW WTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties

C. Any attempt to amend, modify, or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severability. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

Septic System Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install a new septic system be issued for this property. The proposed
systems will serve a proposed three (3) bedroom house. Currently the lot is undeveloped. The proposed
system will utilize a 1500g S.T.E.P. tank with AX20 Pod and auxiliary pump vault. The surrounding
developed lots are served by private wells. There are no wells on surrounding lots within 100 feet of this
system and there are no septic systems installed on the surrounding lots within 100 feet of the proposed
well.
Soils. One test hole was performed on this lot by Pannone Engineering Services in 2005, and groundwater
was monitored for seven days. Ground water was observed at three point one feet (3.1') below the
surface in the test hole monitor tube in May of 2005. Bedrock was not encountered in the test hole. It is
my opinion, based on the results of the percolation tests and overall soils appearance; an application rate
of 2.0 gallons/day/square feet should be used for a CAT III wastewater system.
1. Soil Absorption System Design.

2. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
3. Topography: The existing topography slopes from the south to north at approximately 5% in the
area of the proposed drain field. There are no steep slopes within 50' of the proposed drain field. The
proposed drain field will maintain 50' separation all steep slopes.

5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.

are no surrounding wells within 100 feet of the proposed septic location.

Well and Septic System Permit Request
Ladies and Gentlemen:
I am writing to request a permit to construct new well and septic system be issued for
this property. The proposed systems will serve a proposed three-bedroom house.
Currently the lot is undeveloped. The well will be located in the northeast comer of the
lot, over 100 feet from the proposed septic system.
1. Soils. See the attached soils log. Ground water was monitored for through the
seasonal high ground water of spring and fall of 2005. Ground water was measured at a
depth of 3.1 feet below ground surface on May 15th of this year and 3.5 feet on October
15th. Bedrock was not encountered in the test hole. It is my opinion that the overall soils
appearance of the soils, an application rate of 0.3 gallons/day/square feet should be
used using a conventional system and 2.0 gpd/sf using a class III advanced treatment
system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
The proposed systems will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The average topography in the area of the proposed septic system is
approximately 2 percent in the area of the SAS (based on existing topographic survey
conducted in 1988).
S. Reserve System: The reserve system is based on the use of an Advantex AX20
system. The site plan shows the reserve system size and location, as well as the design
calculations and details.
6. House Location: This lot is sized to the minimum area required by the municipal
code. The location for the septic system is the only location that is acceptable on this lot.
The Owner agrees that they will not move the house from the area shown on the
Engineering Site Plan, or they will coordinate with the Engineer before moving the
house. Moving the house into the reserve septic area can cause the approval of the
entire system to be denied.
The proposed installation will not affect the future development of the surrounding or
existing lots. There are no wells or septic systems within 100 feet of the proposed septic
location.
