Septic System As Built
The well driller on the project has stated the well was placed prior to the requirement for
a Pump Installation Log. He is unwilling the produce the log due to the additional labor
required.
The septic system and well information has been transferred to the current copy of the
inspection report. We have also attached a copy of the Building Permit Report
indicating the final electrical inspection has been completed and approved.
The absorption trench length was extended during construction at the request of the
owner to provide additional capacity and extend the life of the trench. The work was
completed within the parameters of the original design and is within the radii of test
holes completed for the work.

A PERCOLATION TEST SHALL BE PERFORMED AT THE ORIGINAL AND UPGRADE ABSORPTION FIELDS SO
THAT A THIRTY FOOT RADIUS CIRCLE COVERS THE FIELDS. THE SOILS TEST MUST BE COMPLETED
PRIOR TO THE BEGINNING OF CONSTRUCTION OF THE PROPOSED WASTEWATER SYSTEM. THE ONSITE
DEPARTMENT NEEDS TO BE CONTACTED, IF THE TEST HOLES DETERMINE A CHANGE IN THE DESIGN.

Soils found in the front of the subject lot are unacceptable for construction of an
absorption trench as the percolation rates were found to be in excess of 120 minutes
per inch. Acceptable soils were found behind the house at the locations shown on the
attached site plan. We have relocated the absorption trench to behind the house and
added a STEP tank to pump the effluent from the house to the new trench location.
The distribution line in the trench will be constructed 2' below the surface with a total
depth of 12'. No groundwater was noted in any of the test holes on the lot and none
has been found during the monitoring period. The well location has been moved to the
southwest comer of the lot to provide sufficient room for the septic system. Please
modify the well and septic system permit to allow placement of the well and septic
system as shown.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 2, Ravencrest Subdivision intends to construct a new six -bedroom
home on the lot. We are therefore requesting a permit be issued for the construction of
a new well and septic system to accommodate the home. The attached Site Plan and
backup documentation identify the location and configuration of the proposed well and
septic system along with the alternate site. Also identified on the plans are the
locations of the adjacent wells in the subdivision. No conflicts exist between the
proposed septic system and the proposed well and those on adjacent lots. Drainage
arrows are shown indicating the current drainage patterns. The drainage patterns will
be maintained after construction.
Due to the lateness of the year we are requesting the soils test completed during the
subdivision phase of the project be used to design the new system. A new soils log and
percolation test verifying the design parameters will be furnished with the as -built for the
new system. The existing test hole was placed in late April and early May of 2003 and
indicated gravel with varying amounts of silt and sand. The percolation rate was
determined to be 23 minutes per inch and will be verified. We are proposing to place a
new 2,000 -gallon septic tank and a 75' long by 3' wide by 10' effective depth absorption
trench to treat and dispose the effluent generated in the house. The elevation of the
distribution line in the absorption trench will be 2.5' below the surface and the total
depth will be 12.5'. The test hole log indicates no groundwater was encountered
during the monitoring period and no bedrock was found.
The ground surface on the lot slopes at shallow grades from east to west and north to
south. The new trench will be constructed parallel with the slope as much as possible

in conformance with Municipal requirements. The new septic system will be
constructed a minimum of 100' from the existing well and any surface water in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.
