This letter is to document that homeowner Mr. David Fuller, 5331 Shaun Circle,
is.trained and authorized to service his own system. Mr. Fuller is the only
homeowner within the municipality with this arrangement.
The system VeriComm reports No Condition Detected each month and
Authorized Service Provider (A+ Home Services) has visited the site to verify
proper maintenance has been conducted. No other alarm conditions have been
reported via VeriComm since 2002.
This arrangement is valid for as long as Mr. Fuller is willing and able to do the
required maintenance duties. This arrangement will cease when:
• Mr. Fuller no longer wishes to perform this maintenance
• Anchorage Tank determines that maintenance is not properly conducted
• Phone connection to Veri-Comm is disconnected
• When the property is sold (new owner would be placed under customary
Monitoring & Maintenance Agreement)

A design was previously permitted (SW010436) for this property on 10116/2001. We arc
revising the design to utilize the recently approved Advantex technology. Specifics regarding
the design are summarized as follows:
1. DRAINFIELD DESIGN: ADVANTEX TREATMENT SYSTEM
a. Percolation Rate: 9.6
b. Allowable Application Rate forAdvantex: 5.0 GPD/FT, designed for 3 GPD/FT=

2. SURFACE 'WATERS: There is no surface water within 100 fat of the proposed septic
system upgrade.
3. TOPOGRAPHY: As can be seen on the attached topography drawing, the area for the
proposed upgrade slopes from southeast to northwest at a grade of 15% to25%. In short, there
are no slope concerns.

4. LOT LINE WAIVER: A 1 foot lot Iine waiver was granted with the initial design. We are
proposing to put the Advantex drainfield I foot from the lot line.
5. «'ELL ENCROACHMENT NVAIVER: A 75' waiver was granted for the initial design.
We arc proposing to increase the separation distance between the well and the drainfield to 85'.
6. CLOSING: 1 am open to any suggestions from your department, which would be an
improvement to the proposed design. I am unaware of any adverse impacts this installation

Ref: Septic System Upgrade for Lot 18, Block 1, Willamson Subdivision
(Bottomless Intermittent Sand Filter -ISF)
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The existing system
is a "bootleg" septic system and its size and components arc undocumented. The existing
drainfield is surcharged and must be upgraded. We are proposing that a 500 gallon lift station
and a Bottomless Intermittent Sand Filter (ISF) system be installed. Comments regarding the
proposed upgrade are summarized as follows:
1. GENERAL- A test hole was excavated on the property In the only area suitable for a septic
system upgrade; due to the surface water and well setbacks we arc given a very limited area on
the property, it is our opinion that a Bottomless ISF system is the most viable option.
2. SOILS: Attached are logs which shows the soil classifications, groundwater monitoring, and
the percolation test results.
3. DRAINFIELD DESIGN: Bottomless Intermittent Sand Filter (ISF)
a. Percolation Rate: 9.6

1. Sand Material: In accordance with M.O.A. latest standards
m. Pea Gravel: 100% passing 3/8" sieve, less than 201/o passing the 1/4" sieve, and
less than l%passing the #8 sieve.
We arc proposing to excavate down to a depth of 5 feet (maximum - remove all organics), place
a minimum of 6 inches of sand, install the air supply line, and cover it with 1.5 feet of sand. On
top of the sand, we will place 6 inches of 3/8 inch pea gravel, with the pressure laterals midway
in the layer. We will use a conventional lift station (Anchorage Tank), equipped with a
programmable timer so that flow can be intermittently dosed to the ISF.
4. SURFACE WATERS: There is no surface water within a 100 foot of the proposed septic
system upgrade.
5. TOPOGRAPHY: As can be seen on the attached topography drawing, the area for the
proposed upgrade slopes from southeast to northw-cst at a grade of 15% to25%. In short, there
arc no slope concerns.
6. MATERIALS AND CONSTRUCTION PRACTICES: The materials used, and the
construction practices will comply with the MOA' "Intermittent Sand Filter Design, Installation
& Maintenance Manual". The contractor should read this document prior to construction.
Copies are available at the Municipal Onsite Services office (4700 South Bragaw).
7. LOT LINE WAIVER: We would like to request that your department issue a 1 foot lot line
waiver from the.wcst lot line and the proposed ISF trenches. We do not expect any adverse
impacts to occur due to the fact that the property to the west (Lot 19, Block 1, Williamson) is
vacant.
S. Al ELL ENCROACHMENT \VAIVER: We are requesting that your department issue a
separation distance waiver of 75 feet from the existing well on the subject property to the
proposed ISF trenches. The following are some mitigating factors for the justification of the
requested waiver:
The subject well is located uphill from the septic system, so it is impossible for surfacing
effluent to migrate towards the well. The only path for contamination would be subsurface.
The bootleg septic system previously installed on the subject property was encroaching upon
the subject well. Recent water samples from this well (copy attached) indicated no coliform
bacteria and undetectable nitrate levels. This Is confirmation that the geological conditions
arc such that the aquifer is not being adversely effected.
Attached is a number of drilling logs for wells in the general vicinity. It can be seen in
virtually every log that there is numerous layers of either till, clay/gravel, hardpan, or other

confining geological profiles Clearly, based upon the current water quality, these layers have
served to protect the aquifer.
9. CLOSING: I am open to any suggestions from your department, which would be an
improvement to the proposed design. I am unaware of any adverse impacts this installation
would have on adjacent wells or septic systems. If you have any questions, please contact me at

• The subject well is located uphill from the septic system, so it is impossible for surfacing
effluent to migrate towards the well. The only path for contamination would be subsurface.
• The bootleg septic system previously installed on the subject property was encroaching upon
the subject well. Recent water samples from this well (copy attached) indicated no coliform
bacteria and undetectable nitrate levels. This is confirmation that the geological conditions
are such that the aquifer is not being adversely affected.
• The location of the septic system is in a very visible area so that if any effluent was to
surface, it would be noticed and the problem corrected.
• Attached is a number of drilling logs for wells in the general vicinity. It can be seen in
virtually every log that there is numerous layers of either till, claylgravel, hardpan, or other
confining geological profiles. Clearly, based upon the current water quality, these layers have
served to protect the aquifer.
The other path of contamination Is subsurface migration should the tank begin to leak. As can be
seen on the attached well log, the aquifer is relatively deep, with approximately 190 feet of silt,
sand, and hardpan soils that have served to inhibit the migration of untreated wastewater into the
aquifer. Recent water sample results indicated nitrate levels and coliform bacteria results to be
satisfactory. Based upon the aforementioned facts, it appears that there is minimal risk
associated with }he 7S foot separation distance waiver.
