Well and Septic System Permit Request
Ladies and Gentlemen:
I am writing to request that a permit to install a new well and septic system be issued for this lot. The
proposed systems will serve a new four-bedroom house. Currently the lot is undeveloped. The surrounding
lots are served by private water systems and are over 100 feet from the proposed soil absorption system. The
new well will be located in the northern portion of the lot, greater than 100 feet from the proposed system
and from the neighboring septic systems.
1. Soils. The soils were investigated during the subdivision process and are on file with your
department. Ground water was verified this spring. No ground water was monitored in the test holes.
Bedrock was not encountered in the test hole to a depth of 15. The native soils have sufficient sand
component as not to require a two foot (2') sand filter. It is my opinion that the overall soils appearance of
the soils, an application rate of 1.0 gallons/day/square feet should be used using a standard treatment system.

3. Surface Water: There is no surface water within 100 feet of the proposed system. The proposed
systems will maintain at least 100 feet from all surface water and drainage ditches.
4. Topography: Lot 4 slopes to the southeast at approximately 10 percent based on 1997 mapping in
the area for the proposed septic system. The proposed installation will be located in the southern portion of
the lot, greater than 50 feet from slopes greater than 25%.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

SPECIAL PROVISIONS TO SPECIFICATIONS
1. ALL CONSTRUCTION SHALL BE INSTALLED AS SPECIFIED IN THE MOST CURRENT EDITION OF THE MUNICIPALITY OF
ANCHORAGE STANDARD SPECIFICATIONS (MASS) FOR COMPONENT PARTS AND MATERIALS USED IN CONSTRUCTION OF
ON-SITE WASTEWATER DISPOSAL SYSTEMS.
2. ALL WORK SHALL BE IN ACCORDANCE WITH THE ATTACHED SPECIFICATIONS.
3. SCOPE OF WORK: 4 -BEDROOM HOUSE, NEW TANK (1,250 GALLONS), NEW DRAIN FIELD (50LFx2'WIDEx6'EEFFECTIVE
DEPTH, 9' TOTAL DEPTH). INSTALL BOTH THE PRIMARY & RESERVE FIELD. INSTALL BALL -TYPE DIVERTER VALVE BETWEEN
THE TWO FIELDS.
4. CONTOUR INTERVAL IS FOUR (4) FOOT AND IS BASED ON 1997 MAPPING.
5. GROUNDWATER IS NOT EXPECED TO BE ENCOUNTREO DURING EXCAVATION, AS EVIDENCED BY THE SOIL TEST HOLE
INFORMATION. IF AN APPARENT WATER TABLE IS OBSERVED IN ANY OF THE EXCAVATIONS LESS THAN 14 FEET BELOW
