system, will expire on 03 Jun 08. Permits are valid for 365 calendar days from date of issue.
If this was a well permit and the well has been drilled, a well log and a pump install log must be sent
to this office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

Dear Sirs:
The purpose of this letter is to provide the required design narrative in support of our application
for a permit to install an "Advantex" advanced treatment system to serve a proposed 4 -bedroom
residence on Lot 6A, Block 1011, Rabbit Creek Heights S/D, located on Paine Road. A soils log, perc
test results, a site plan, design drawings and specifications are enclosed for your review.
As can be seen from the enclosed log for test hole #1 there is a stratum of silty sandy gravel
between 1 and 5 feet below ground level that peres at a measured rate of 2 minutes per ilt.h. Beneath
this is a dense gray gravelly silt and water seeps were encountered at the interface between the two
strata. Because the monitored water level goes up and down in the silt stratum it does not appear to be
impermeable. The topography of the lot in the area of proposed construction slopes downward toward
the north at 23%. Because of the relatively steep slope combined with a shallow water table, we
propose to install an "Advantex" advanced treatment unit that will discharge its residual into a 5 -foot
wide shallow drainfield. Based on a slightly conservative soil application rate of 4.0 gp(L'sq. ft. for the
treated residual the required area is (4x150)/4 = 150 sq. ft, which is achieved by construction of a 30 -
foot long 5 -foot wide drainfield. To achieve adequate separation from the seasonal high water table and
to further enhance the performance of the drainfield we plan to have it underlain by a minimum of I foot
of coarse filter sand.
The proposed treatment system consists of a 1500 -gallon epoxy -coated steel septic tank
manufactured by Anchorage Tank, with an AX -20 Mode la filter pod that discharges by gravity into the
residual disposal field. Due to the relatively steep slope of the lot we are calling for creation of a
seasonal access road down to the level of the house to facilitate pumping as necessary.
The proposed project will have no significant impact on present or future water supply and
wastewater disposal systems serving adjacent properties, nor will it have any significant impact on
reserved space -surface and subsurface, or on drainage.

Wastewater disposal system installation
Specifications
1.0 General:
1.1 The scope of the project consists of installing an advanced treatment system using a 1500 -
gallon epoxy coated steel tank manufactured by Anchorage Tank and an "Orenco" AX -20, Mode 1 a
treatment pod. The filtered effluent will flow by gravity into a 5 -foot wide by 30 -foot long soil
absorption drainfield with an effective gravel depth of 0.5 feet. The drainfield is underlain by 1.0 — 1.5
feet of coarse filter sand.
1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor
deviations from these drawings may be allowed or required by the engineer conducting the inspections.
All construction procedures and material specifications shall conform to Municipal and State
requirements. All separation distances shall be in conformance with Municipal requirements, unless
specifically waived.
1.3 The contractor shall be responsible to obtain any necessary utility locates, and to work
around any buried utilities.
1.4 The contractor shall provide adequate cover material and rough grading over all system
components to ensure that proper drainage is achieved after settlement and that there are no residual
depressions. Extra fill material will have to be imported to the project site in order to achieve the
required 3:1 slope on the downhill side of the drainfield.
1.5 Unless specifically agreed otherwise, the homeowner shall be responsible for finish grading
after the soil is compacted, as well as placement of topsoil and reseeding all areas disturbed by the
construction.
2.0 "Advantex" AX -20 Mode lb Advanced Treatment System:
2.1 The "Advantex" treatment unit major components include (1) a 1500 -gallon, 2 -complement
epoxy coated steel tank and (2) a single Mode 1 a filter pod. All components shall be Municipally
approved and shall be set level on undisturbed soil and backfilled in accordance with Municipal
requirements as well as Orenco's and the supplier's instructions.
2.2 All pipe connections to the tank shall be equipped with waterproof mechanical couplings.
The waste line from the residence to the tank shall have a minimum slope of 1/4"' per foot, and the
waste line between the filter pod and the residual disposal drainfield shall have a minimum slope of 1/8
inch per foot.
2.3 The contractor shall be responsible to obtain the necessary Municipal electric permit and
inspection, and shall provide a copy of the electrical inspector's report to the engineer overseeing the
construction of the wastewater disposal system.
2.4 The second compartment of the tank shall be equipped with a watertight, insulated manhole
riser and cover extending a minimum of 6" above final grade with positive surface drainage away from
the manhole. The contractor shall be responsible for determining the necessary length of the manhole
riser, and for properly positioning the Recirculating Splitter Valve (RSV) in the riser.
2.5 The control and alarm floats shall beset at the elevations specified by the supplier, and their
proper operations shall be verified by the contractor.
2.6 The treatment unit shall be set up with a "VeriComm" remote monitoring system as supplied
by the distributor, and the control panel for the unit shall be mounted at an approved location.
3.0 Residual soil absorption system:
3.1 The soil absorption system shall be constructed by creating a 6 -foot wide by 30 -foot long
excavation to expose the top of a silty sandy gravel stratum at a depth of approximately 1.2 feet b=low
ground level. The long axis of the drainfield shall be parallel to a natural contour of the ground and no
closer than 10 feet from any property line.
3.2 The bottom of the excavation need not be level. Any compacted or smeared surfaces shall
be raked to allow proper infiltration. A 12 - 18 inch thick layer of coarse filter sand shall be placed in
the bottom of the excavation and compacted by foot to promote desired infiltration and to reduce the
tendency to form a biomat. The top surface of the filter sand shall be 5 -feet wide by 30 -feet long at an
elevation of 0.5 feet below ground level at the uphill side of the dminfield.
3.3 A total of 1.0 feet of washed sewer gravel shall be placed on top of the filter sand with the
perforated distribution pipes laid level such that the pipe inverts are no less than 0.5 feet above the
bottom of the sewer gravel. Sewer gravel shall be 0.5" - 2.5" washed gravel, with less than 1% passing
the #200 sieve. The distribution pipe shall be 4" diameter F-810 perforated pipe.
3.4 The monitor tube and cleanouts shall be of 4" diameter and installed in the locations shown
on the design drawings. The portion of the monitor tube extending through the sewer gravel shall be
perforated.
3.5 Approved filter fabric shall be placed over the entire top surface of the sewer gravel. Frost
protection consisting of 2 inches of rigid burial type insulation plus 2 feet of soil cover is to be placed
over the filter fabric.
3.6 The top surface of the cover material shall be raised a minimum of 6 inches higher than the
surrounding terrain to allow for subsequent settlement, and shall be graded to smooth contours. Fill
slopes shall be no steeper than 3:1, and shall be extended out until they intersect natural ground.
3.7 Unless specifically agreed otherwise the homeowner shall be responsible for arranging to
have the site finish graded after the backfill material has stabilized, and for placement of adequate
topsoil and seeding to promote rapid revegetation of all areas disturbed by the construction.
4.0 Inspections:
4.1 A minimum of 5 engineering inspections will be required during the course of the project:
(1) initial stakeout with the contractor to establish the location of the system and to discuss the plans,
specifications and construction procedures, (2) after the native material has been excavated to expose the
infiltrative surface to ensure that it is level and at the right elevation, and conforms with the soil test
information, (3) after the filter material has been placed, (4) after the sewer gravel is in place and the
distribution pipe has been laid and connected up to the septic tank, but prior to placement of insulation
or filter fabric, and (5) after rough backfill and grading is complete. The "Advantex" treatment unit
requires one inspection after the tank is set level and the piping connected, but prior to backfill. This
inspection may be incorporated with any of the above inspections.
4.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far
in advance to ensure the availability of the engineer.
ADVANCED WASTEWATER TREATMENT SYSTEM MAINTENANCE AND
REPAIR AGREEMENT

"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED \VASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AW WTS), described as amn7-6u located

2. Definitions.
Anchorage, Alaska.
A. Alteration. Any change to the design or function of an AW WTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
These approvals certify that the systems are adequate for the homes that
they support and meet the codes that were in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction) An On -Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating) An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On -Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. Fee. Owner shall pay to Municipality an annual fee of 2Ci2D
($fi/�_.00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Term. The term of this Memorandum of Understanding shall be for the life of the
AWWTS.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On -Site approval is issued to
the new owner or transferee of the property.
5. Alterations installation and Removal of Additional F, ui mcnt. Owner agrees
not to make any alterations, removal of parts or additions to the AWIVTS without a
Construction Permit from the Municipality.
6. ATaintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
O%vner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS

C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AW WTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification guidebook
for AW WTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AW WTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

Anchorage: Purchasing Officer
C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.

1. The site plan shall have a description of the topography, areas of excessive slope, and
extent of slopes and surface drainage patterns within 100 feet of any part of the system
or portion of the lot intended for use for on-site wastewater disposal. AMC 15.65.030 D.
Please provide this information on the site plan.
2. Will the proposed location of the well possibly prevent Lot 5A from developing? Could the well
be moved to the south-east comer?
