Dear On Site Services Engineer:
The southwest end of the absorption trenches recently constructed for Lot 10A, Block 2,
Aspen Highlands Subdivision were constructed within 4' of the southwest lot line.
Placement of the trenches in this area will not conflict with other septic systems or wells
in the area. Lot 9 to the southwest was recently constructed with the well more than
100' from the trenches. We therefore recommend a waiver be issued to allow
placement of the trenches at 4' from the southwest property line.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The existing septic system on Lot 10A, Block 2, Aspen Highlands Subdivision No. 2 has
failed and must be replaced. We are therefore requesting a permit be issued for the
construction of a septic system to accommodate the five -bedroom home on the lot
home. The attached Site Plan and backup documentation identify the location and
configuration of the existing well and septic system along with the new septic system.
Existing wells and septic systems in the area are also shown on the attached plans.
Drainage arrows are shown on the site plan showing the grade and direction of flow on
the lot. The drainage pattern will not be changed as a result of this project.
The test hole placed on the lot in the area of the absorption trench for the new system
indicated silty gravel and sand (GM) material from 4.2' to 9' below the surface. The
percolation rate of the soil was determined to be 10 minutes per inch. No groundwater
was found during the placement of the test holes and none was noted during the
monitoring period. An application rate of .8 gallons per day per square foot was used in
the design of the absorption system. We are proposing to pump, cursh and backfill the
existing septic tank and absorption crib and place a new 1,500 -gallon septic tank and a
two 67' long by 5' wide by 2' effective depth absorption trenches to treat and dispose of
the effluent generated in the house. The elevation of the distribution line in the
absorption trenches will be 5.0' below the surface and the total depth will be 7.0'. A
minimum of 3' of fill or 2" of insulation and 2' of fill will be placed over the absorption
trench and 4' over the septic tank. A flow splitter valve will be placed to assure even
effluent flow to each absorption trench.
The ground surface on the lot in the area of the absorption trenches slopes to the
southeast at 6%. No major grade breaks or excessive slopes were noted within 100' of

the proposed septic system. The new trenches will be constructed parallel with the
slope as much as possible in conformance with Municipal requirements. The new
septic system will be constructed a minimum of 100' from all wells and any surface
water and 10' from the proposed and existing water services in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

ONSITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 10A, BLOCK 2, ASPEN HIGHLANDS SUBDIVISION NO.2
GENERAL:
The scope of this project includes the pumping, crushing and backfilling with
sand of the existing septic tank and absorption crib and the procurement and
placement of a new 1,500 -gallon septic tank at the location shown on the Site
Plan. Work also includes the construction of two new 67' long X 5' wide X 2'
effective depth absorption trenches at the location also shown on the Site
Plan. The distribution line in the trench will be placed at 5.0' below the
existing ground surface. Total depth of the trench will be 7.0' below the
existing ground surface. The new absorption trench and septic tank must be
placed a minimum of 100' from all private wells and any surface water in the
area and 10' from the water service line proposed for the lot. A minimum of
3' of cover is required over the absorption trench and 4' of cover over the
septic tank. A flow splitter valve must be placed to insure even flow to each
absorption trench.
2. Construction shall be in accordance with the approved site plan, design
drawings, Municipal Permit with any special provisions or conditions, and all
applicable State and Municipal Wastewater Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the location
of all lot lines. The Contractor must also verify the location of wells in the
area and assure that the separation distances are met.
4. Unless specifically agreed otherwise, the contractor shall be responsible for
final grading areas subsequently depressed from soil settling. Property
owner shall be responsible for revegetation of affected areas unless
specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified by the
Municipal Department of Health and Human Services for system installations.
Owners installing their own systems must receive prior approval from
D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
A new 1,500 -gallon septic tank must be procured from an approved source
and installed at the location shown.

2. A septic tank is to be constructed by a certified septic tank manufacturer.
Construction shall include two 4" cleanouts for pumping access.
3. The septic tank shall be sufficiently bedded to prevent settling or shifting of
the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches above
final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial
insulation.
6. A foundation cleanout shall be installed one to four feet from the building
foundation. Two cleanouts are required between the tank and the drainfield.
7. Final grading over the tank shall be such that a positive slope exists away
from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the design.
The bottom of the trench shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a level
bed of drainfield rock. Rock should then be placed over the pipe to provide a
minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield rock
and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations shown on
the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum 100' away
from any private water wells in the area, 150' from a Class "C" Well or 200'
from any community well.
6. Direct bury insulation must be placed over the distribution system if less than
3' of backfill depth is available. Finish grade over the trench must be
mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for a 5'
wide trench. Contractor shall verify this condition prior to placement of the
rock. All pockets of unacceptable materials must be removed and replaced.

MATERIAL SPECIFICATIONS:
Septic tanks must be constructed by a municipally approved septic tank
manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and
solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662
or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow
Chemical Co. Styrofoam HI or equal).
4.. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5 in diameter with less than 3% passing the
#200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These inspections
must be conducted under the supervision of a professional engineer registered
in the State of Alaska. The first inspection must be conducted after the
excavation of trenches, beds or pits and before the installation of any gravel. A
septic tank may be set in place, but may not be backfilled.
The second inspection must be conducted after the placement of the geotextile
fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No
backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction notes
for use in preparing the certified as -built of the completed system.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water

Permittee acknowledges the validity of the Easement. Permittee warrants and represents that
Permittee is the owner of the fee simple interest in the land subject to the Easement and the
following described real property to which the Easement is adjacent:

2. Encroachment. Permittee has constructed or purchased the following improvements
on or within the real property subject to said Easement:
A well that encroaches approximately Two Feet (2') into said Easement, for a distance
of approximately One Foot (I'), and a shed structure that encroaches across said
Easement for a distance of approximately Ninety-one Feet (9 Y), as shown on the As -
Built Survey of Lot 10A, Block 2, Aspen Highlands Subdivision Unit 2. by Gastaldi
Land Surveying LLC, dated July 24, 2012,
(herein "Encroachment").
3. Permit. Chugach hereby authorizes Permittee to continue the existence of the
Encroachment in its existing location subject to the covenants set forth in this permit.
4, Indemnification. Permittee shall indemnify and hold Chugach harmless from any
claims for personal injury, property damage or other loss arising in any way from the continuation of
the Encroachment.
5. No Expansion or Increase. Permittee shall not in any way expand or increase the
extent to which the Encroachment occupies any of the real property encumbered by the Easement
without the prior written consent of Chugach. Chugach has no obligation to provide such consent.
6. No Interest in Real Estate Acquired Permittee hereby acknowledges that the
Encroachment and any additions thereto, even if in violation of this permit, has not and shall not in
the fixture give rise to any interest or estate in the real property subject to the Easement or any other
real property. Without in any way limiting the foregoing, Permittee acknowledges that the
Encroachment is not adverse to any interest of Chugach and its continuation is entirely with the
permission of Chugach for purposes of the common law doctrine of adverse possession. This permit
creates no interest in the real property subject to the easement.
7. Modification of Utility Facilities. If the electric utility facilities located within the
Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay that portion of
Chugach's total costs for such modification which is reasonably attributable to accommodating or
preserving the Encroachment. If such payment is not made within thirty (30) days of written demand
for same, Chugach may terminate this permit in accordance with paragraph 9, below.
8. Covenants Running with the Land. The obligations ofthe Permittee arising from this

permit shall be covenants running with the land which shall burden the Parcel and the Permittee's
interest in the land subject to the Easement and shall benefit the Easement.
9. Termination. Chugach may terminate this permit after the giving of thirty (30) days
written notice of such termination if:
a. The Encroachment is enlarged, increased or extended within the real property
affected by the Easement.
b. The Encroachment is damaged by any means to an extent of more than thirty percent
(30%) of its replacement cost at the time of destruction.
C. Permittee fails to make the payment described in paragraph 7, above.
10. Term. This permit shall, if not sooner terminated by Chugach, expire forty (40) years
from the date hereof.
11. Entire Agreement. This written permit constitutes the entire agreement between the
parties with respect to the subject matter hereof and supersedes all other prior or contemporaneous
agreements, oral or written, between the parties. No modifications, amendments, deletions, additions
or alterations of the permit shall be effective unless in writing and signed by all of the parties hereto.

Well Encroachment
We have contacted all of the utility companies concerning the location of the well on
Lot 10A in the utility easement. Both ACS and GCI have responded with letters of
non objection. These letters are attached. Unfortunately, Chugach Electric will
require an encroachment agreement which must be recorded. This typically takes a
month to complete. We will complete all work to obtain the necessary letters of non
objection, but request that a conditional COSA be issued to allow the property to
close. Thank you for your consideration.

Nitrate Advisory
Certificate of On -Site Systems Approval # 121296
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 2, Lot l0A
of Aspen Highlands No.2 subdivision. This inspection revealed a nitrate
concentration of 9.41 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
