All construction must be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal Code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations ( 18AAC72 ) and Drinking Water Regulations ( 18AAC80 ).
3, The engineer must notify DSD at least 2 hours prior to each inspection. Provide notification by catling
(907) 343-7904 ( 24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
a.) The southern property line in the vicinity of the proposed septic tank and primary absorption field, and the 100 -foot
protective radius of the proposed well shall be marked by a professional land surveyor prior to construction.
b.) TH#3 shall be extended or an additional test hole shall be conducted in the area of the western portion of the
reserve absorbtion field to a minimum of 16.5 feet at time of construction. 7 -day groundwater monitoring shall also be
conducted. Soil log with groundwater monitoring results shall be provided with the Inspection Report.
c.) This permit is for a gravity feed wastwater disposal system. The engineer shall submit a revised design if a
pressure distribution system shall be required.

Thanks for taking may call last week. As I mentioned Brian Harten's schedule has become congested and I can't
afford any further impacts from delays, therefore, with your permission I would like to install my septic system
myself with Brian performing the required civil engineer Inspections. As stated I believe 1 have plenty of
experience within this scope of work and am comfortable in doing so. I was no able to get a copy of my resume
as it's in my personal computer which is buried in storage so I will hit the highlights of my relative experience
below.
TAM Construction -1 worked for Bill McKeever for about 3 years where we performed a variety of civil
construction including water, sewer, and storm water utilities installations in Anchorage, Girdwood, and Big Lake
areas.
Potter Creek Water Company - I managed this small to medium sized private water system for approximately 4
years. My responsibilities included water system maintenance and repair, new installation inspections and
Installation services, locates, and testing.
have about 12 years experience within the construction industry here in Alaska. I am currently a project engineer
for UIC Construction and we specialize in rural construction of all types. We also have a civil department within
the company and I work for the manager of this department regularly performing civil construction project
management and field supervision.
The plan for my system is to rent and operate equipment and Install the septic system myself. I am not going to

Attached please find an updated site plan for my proposed redesign of the above mentioned property.
At the time of my redesign, I did not have the latest design layout for the adjacent property, Lot 10 Block 4 Southfork
North Subdivision, showing the latest proposed location for the water well on this property, and thus was not able to
accurately show its location on my site plan.
As the owner's of Lot 10 currently have the well location staked, it is 110 feet south of the common lot line, thereby
creating no conflict with my proposed redesign. Please refer to the updated site plans showing the current proposed
well location. In addition, my client, Justin Jones, met with Jeremy Hollaway, the owner of Lot 10 and confirmed that
this is indeed the proposed well location.

Lot 11, Block 4, Southfork North Subdivision
Dear Mr. Roth:
We were issued a permit to construct a well and septic system on the above referenced property. Through the course
of site development and foundation excavation, an area of high quality sand was discovered which will be much better
able to support an on-site septic system than the area that the system was originally designed for. In the interest of my
client and the long term success of the system, I have elected to redesign the septic system to take advantage of
these better soils.
The subject property is a large lot, 2.59 acres, however, the majority of the lot is low tying and potentially wet as it
slopes to the creek which forms the western boundary of the lot. At the time of the original design, the only viable area
for an on-site septic system is the east third of the lot This is still the case; however we will be shifting the drainfield
location further to the west within this area.
As you recall, there was a well and septic system designed, but not constructed for the adjacent Lot 10 Block 4,
Southfork North Subdivision. There was a conflict identified with the proposed well location for Lot 10. Much
discussion ensued and an amicable solution was worked out I hate to further inconvenience the owner of Lot 10 and
your department, but in the interest of my client, I must ask that we revisit this issue.
With this revised design, the well for Lot 10 Block 4 will need to be located 100' from the proposed primary and reserve
drainfield systems on Lot 11. We ask that the well be located to allow us to retain as much of the real estate in the
immediate vicinity of the proposed drainfield as possible for future reserve drainfields. 1 have asked the owner, Justin
Jones, to coordinate with the owner of Lot 10, John Hollaway, and his engineer, Chris Wood, P.E., regarding this
design change. We will keep your department posted.
We are proposing a 1,250 gallon S.T.E.P. septic tank and a 5 -wide drainfield, 45 feet in length, with 36 inches of
effective sewer rock. The soils in the area of the drainfield were sandy, with clean sands from a depth of 3 feet to 11
feet This sand was moist from the spring thaw and accepted 5 gallons of water in less than 8 minutes on two
successive applications. Because of the clean, well graded nature of the sands, sufficient treatment and filtration of
the effluent will occur and no'filter sand" liner will be necessary.

Surface Waters
There are no adjacent surface waters within 100 feet of the proposed well and septic improvements.
TopouraphV
The subject lot has predominantly gentle slopes of approximately 2 to 4 percent to the north and northwest. There is a
steeper slope of approximately 12-15% which delineated the eastem third of the lot from the western two-thirds. This
creates a higher "bench* on the eastern third of the lot that is the primary area of development for the proposed single
family home. The proposed construction will not alter or impede the IoYs natural drainage pattems.
Groundwater and Bedrock
No groundwater or bedrock were observed at any time during the original test hole excavations.
Closing
The development of this property will not preclude the adjacent properties capacity to construct future wastewater
systems. I am unaware of any adverse impacts this installation would have on any existing adjacent wells or septic

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

The Municipality of Anchorage DevelopmAnt Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system Is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations in effect at the time of installation.

RE: Lot Line Waiver Request
Lot 11, Block 4, Southfork North Subdivision
Dear Mr. Roth:
We are requesting a waiver of lot line setback requirements for the above referenced system.
The primary system is encroaching into the required 10 foot setback from the property line by 1.5 feet
At the time of construction, the southerly lot line was flagged, and the drainfield was laid out and constructed to be 10
feet from this lot line, however, at the time of as -built, it was noted that the piping in the drainfield was 10 feet from the
lot line, but the sidewall of the trench was 8.5 feet from the lot line.
Through my construction documentation, I merged the house location from the as -built built survey into my electronic
drawings. From this point, I displayed the septic system components based on my field measurements (swing -ties,
etc.) from the building foundation. While the as -built survey shows the drainfield to be well outside of the required 10 -
foot setback, I feel it only prudent to request the lot line waiver, as I feel the drainfield may be encroaching into this
setback.
I am herby requesting a waiver from the 10 -foot setback distance to a 5 -foot setback distance. This waiver will cover
not only the primary drainfield, but will also allow construction of the reserve drainfield (if required) within the vicinity of
our latest test hole, and within the sandy material known to be in this vicinity.
Approval of this waiver will not preclude the adjacent properties capacity to construct future wastewater systems. I am
unaware of any adverse impacts this waiver would have on any existing adjacent wells or septic systems.
