Ref: Proposal for revisions to Septic Design for T15N, R1 W, Section 5, Lot 35
To whom it may concern:
Per the request of the owner, we have revised the septic system design so as to eliminate the need for a
long sewer line from the house. We are proposing to install 1250 gallon septic tank and dual 5 -wide type
drainfields. Two test holes were excavated on the property. The drainfield will be designed around the 30
foot radius of test hole ii2. Comments regarding the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography around the
drainfield is generally Oat.
5. WAIVER OF SAND FILTER: The soil perked at less than one minute per inch. Based upon our
visual assessment, there appeared to be enough sand content so as to negate the need for a sand filter.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Per the request of the owner, we have revised the septic system design so as to eliminate the need for a
sand filter. A soil sample was provided to Dan Roth and his visual assessment was that there is enough
sand present so as to negate the need for a sand filter. We are proposing to install 1250 gallon septic tank
and dual 5 -wide type drainfields. One test hole was excavated on the property. The drainfield will be
designed around the 30 foot radius of this test hole. Comments regarding the design are summarized as
follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for dminfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography around the
dminfield is generally flat.
We are unaware of pny adverse impacts this installation would have on adjacent wells or septic systems.

Ref: Proposed Septic Design for T15N, R1W, Section 5, Lot 35
To whom it may concern:
The proposed 4 bedroom house will be served by a private well and septic system. We are proposing to
install a new 1250 gallon septic tank and a bed type drainfield. One test hole was excavated on the
property. The drainfield will be designed around the 30 foot radius of this test hole. Comments regarding
the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAT\FIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography around the
drainfield is generally flat.
5. WAIVER OF SAND FILTER: The soil perked at less than one minute per inch. Based upon our
visual assessment, there appeared to be enough sand content so as to negate the need for a sand filter.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Ref: Proposed Septic Design for T15N, R11N', Section 5, Lot 35
To whom it may concern:
The proposed 4 bedroom house will be served by a private well and septic system. We are proposing to
install a new 1250 gallon septic tank and a bed type drainfield. One test hole was excavated on the
property. The drainfield will be designed around the 30 foot radius of this test hole. Comments regarding
the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE RATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography around the
drainfield is generally flat.
5. WAIVER OF SAND FILTER: The soil perked at less than one minute per inch. Based upon our
visual assessment, there appeared to be enough sand content so as to negate the need for a sand filter.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Ref: Waiver Request from New Septic Tank to Foundation for T15N, R1 W, Section 5; Lot 35
To whom it may concern:
We are requesting a 2 foot waiver from the edge of the tank to the closest part of the foundation
(footer). During our initial inspections of the tank and drainfield installation, the foundation for the
main house was not installed. We recently performed a septic adequacy test for the purpose of
obtaining a COSA and discovered that the standpipe cleanout for the first compartment was
exactly 5 feet from the visible part of the foundation. Since the tank has a 5 foot diameter (2.5
foot radius), it is our assessment, that the tank is slightly more than 2 feet from the footer.
Therefore, we are requesting a waiver to the code required 5 foot separation:
• Attached is profile drawing showing the encroachment, with an imaginary 45 degree
angle of repose from the bottom of the footer. As can be seen this imaginary slope line
slightly intersects a portion of the tank.
• Typically when septic tanks collapse, it occurs at top of the tank, between the end plates
and the interior baffle. With the angle of repose intersecting in the lower portion of the
tank, if it were to collapse (at the top), it is reasonable to believe the foundation would
not be significantly compromised as long as corrective was taken immediately to
address the problem.
It is our recommendation that you issue an advisory notice with the COSA regarding this
