T15N RIW SEC 18 5-8642 Lot 5 051-172-58 Vp C,k-4 Eagle River Engineering Services Christopher IL Wood, P.E. 10421 VFW Rd. Suite tot Eagle River, AK 99577 WELL & SEPTIC ADEQUACY TEST REPORT TEST DATE: 1/19/2007 LEGAL: TI 5N, RI W, SEC. 18 LOT 93A LOCATION: 19045 Birchwood Loop Rd RESIDENCE: Single Family, I Bedrooms WELL YIELD: 6 Gallons per Minute flow rate WATER SYSTEM: Private Well (9o7) 694-5195 tel (9o7) 694-3297 fax WATER SAMPLES: Arsenic: 0 ug/l, Nitrates: 0 mg/I, Bacteria 0 coliform, 12 other bacteria colonies/I00ml Taken by Engineer on: 1/19/07 SEPTIC SYSTEM: From Municipal Records Tank: 1500 gallons Absorption System: 32 foot trench System designed for commercial use INSTALLATION DATE: 8/14/84 INSTALLED ABSORPTION AREA unknown Square Feet ORIGINAL SOIL RATING: 0.8 gpd/ftt SQ.FT/BDRM , 10 minutes per inch percolation rate, From Engineer's Design TEST PROCEDURE The leachfield was charged with water from the on-site well at an average rate of 6 GPM for a total of 1005 gallons. The leachfield consists of a 32' trench of unknown dimensions. Based on the tank size, it appears that the leachfield was sized for a minimum of 600 gallons per day of effluent. The septic tank and leachfield liquid levels were monitored referencing a measurement below the top of the standpipes. During the test, water was added through the clean out tube and the leachfield water level was monitored as water was added and then absorbed into the surrounding soil. The water level in the septic tank did not rise during the addition of 1005 gallons of water showing that the leachfield had accepted the entire amount. This indicates the septic system will accept the required 150 gallons per day of effluent, per bedroom, which is the required absorption rate for MOA approval. S { ?'1D 7Cu 1- X2003106-101 Lot93 W SAT V%olo; e 7-3 /- 07 Page 1 nr2 kpc4,%k,d 7/fo% WELL: The well was pumped into the first trench cleanout Total drawdown from a static water level of 48 feet was 23 feet, after 60 minutes of pumping, at an average flow rate of 6.2 gallons per minute. The well level in the casing stabilized at that level for the remainder of the test indicating a well production rate of 6 GPM. This flow rate exceeds MOA's minimum requirements for private well water production. TEST RESULTS The septic system absorption rate and well flow rate exceed the basic requirements of the Municipality of Anchorage for a 1 bedroom single family residence as of the day the system was tested. Arsenic, Nitrate, and Coliform levels are within the limits allowed by the Municipality. 12 other bacteria were noted in the water sample and the well vas shock -chlorinated. CONDITIONS: Assessment of existing subsurface conditions by the inspecting engineer is limited to information obtained from the available monitoring tubes and Municipal record search. We do not guarantee the validity, accuracy or quality of subsurface tests and inspections performed by the original inspecting engineer or authority. This report is limited to absorption rate testing and surface separation measuremarnts as currently required by the Municipality of Anchorage and does not verify the integrity of the piping for the water supply or water quality other than the bacterial and nitrate content. The operational life and the matter of compliance with State and Municipal codes, for all water and septic systems depends on the local soil conditions, groundwater levels that may not be observed from the surface without additional testing, water usage of the homes being served by the system, and the detail of required testing procedure. There is no guarantee that the well and septic system tested will meet the requirements for approval in the future. The test data and investigation of existing conditions is provided to our client for submittal to the Municipality Health Department for their review and approval. Any concerns with this test report should be discussed with the testing engineer. If it is requested we will submit the report directly to the Municipality. \2003\06-101 IAT93 W SAT Pace 2 of 2 LECIGN0 SEPTIC SYSTEI-+1 S7AN0PIPE EziSTINC, W, o PROPOSED /WEI R 1 5uit Jr EST Por J iroo 0.0' 0.2' 1.3' 2.0' G 6.0' 16.0' T.D. OF HEALTH 6 ENVIRONMENTAL PROTECTION PEN 161987 RECEIVED Dear Mrs. Oswaldt: Attached, please find the current D.E.C. approvals for a Class "C" well and commercial septic system serving the above referenced lots. Our client Matanuska Telephone Association, Inc., is required to replat these lots as per a Conditional Use ruling. The replat is to combine the two existing lots into one continuous lot. The existence of an operational well and septic system on the newly formed lot should satisfy any future Health Department requirements. It should be noted that expansion of the building use is not allowed under this conditional approval, which precludes any expanded use or requirement of additional septic area. If it is desired by Matanuska Telephone Association, Inc., to donate a portion of the replat area as a Municipal Park, on-site services would not be required in this instance either. We are therefore requesting Health Department Approval to the Platting Authority. If there are any questions or concerns, please feel free to call me at 694-5196.
