I am writing to request a renewed permit to inst I a septic system be issued for this property. The
proposed systems will serve a }bedroom house. Currently the lot is developed. This
system will be over 100' from the private well on this lot. The surrounding developed lots are also served
by private wells that are over 100 feet from this septic system.
P.E.S. Visited the site in July of 2013 and conditions remain the same as at the time the original permit
was issued. The test hole monitoring tube was found to be dry on 7/1/2013.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

residence. We request the permit be reissued for a 4 bedroom house. Attached is a revised design
drawing. The original design showed that we were proposing to use a 0.8 application rate. The percolation
rate was 5 minute/inch which is a 1.2 application rate. We have changed the design to use a 1.0
application rate. We are proposing that 1250 gallon septic tank and a dual 5 -wide trench type drainfield be
installed as shown on the design. An alternate site for a 4 bedroom system is shown on the design which
will require the use of a advanced wastewater system.
TOPOGRPAIIY: As can be seen on the attached design drawing the average topography below the
drainficld is a 5-15 percent slope running approximately east to west. There also a 5 to 6 foot cutbank
along the road approximately 35 feet down hill from the proposed alternate site. As can be seen on the
attached cross section drawing, at a 25 percent slope 2.5 feet above the top of the distribution line, the line
docs not daylight. In short there is no slope concern downhill form the proposed alternate site.
We arc unaware of any adverse impacts this installation would have on adjacent wells or septic systems.
