LEGAL: SPENDLOVE VIEW HEIGHTS BLOCK 3, LOT 11
Per the recent attached as-built survey by Elizabeth L Walatka dated August 12,
2025, the newly rebuilt lined ISF system is approximately 3’ to the lot line and
within the 5’ separation distance to this southern lot line. This lined ISF was also
noted previously in the MOA record documents in the comment section of the 2017
COSA to be approximately 4’ to the south property lot line.
There have been no known previous issues with this location, there are no adjacent
septic absorption systems within 20’+ and no 100’ well radii encroachment upon
this lined ISF. We do not expect there to be any adverse effect on the adjacent lots
with the continuance of this lined ISF location.

RE: LINED IDSF REBUILD SEPTIC UPGRADE PERMIT
LEGAL: SPENDLOVE VIEW HEIGHTS BLOCK 3, LOT 11
A recent septic adequacy test was performed and the existing absorption field performed
satisfactorily, however the existing lined IDSF is saturated and needs to be rebuilt. The air
compressor and other components will be verified and upgraded as needed at construction. The
property owner has requested we obtain a permit to upgrade this failed lined IDSF of the above
referenced lot.
Additional information of this lined IDSF is attached and is in the MOA file. The lot and area are
served by private water. The design will not impact any of the neighboring properties. Please
contact us if you have any questions.

(intermittent Sand Filter - ISF)
To whom it may concern:
The existing 5 bedroom house is served by a private well and two holding tanks. Given the
steep topography, well location, the presence of a groundwater spring, there is limited options
regarding the sitingtiof a septic system. Two test holes were excavated in what was believed to
be the most suitable location for a drainfield. Bedrock was encountered in both holes at a depth
of 7.0 feet. No impermeable soils or groundwater was encountered. The percolation rates varied
from 5.3 to 6.0 minutes per inch. We are proposing that a 2000 gallon S.T.E.P. tank and a lined
Intermittent Sand Filter (ISF) system with a secondary drainfield be installed. Comments
regarding the proposed upgrade are summarized as follows:
A. LINED INTERMITTENT SAND FILTER DESIGN:
a. Allowable Application Rate ISF: 2.0 gallons/day/ft2
b. Number of Bedrooms: 5
c. Design Flow: 750 gallons per day
d. Minimum Absorption Area: 375 ft2
e. Dimensions: 19' x 20' (standard M.O.A. approved configuration)
f. Effective Absorption Area: 380 ft2
g. Air Supply: Thomas Industries, Model 5070, "Anchorage Tank".
h. Air Supply Line: "Wasteflow" emitterline, 1/2 inch I.D, "Anchorage Tank".
i. Sand Material: In accordance with M.O.A. latest standards
j. Pea Gravel: 100% passing 3/8" sieve, less than 20% passing the 1/4" sieve, and
less than 1% passing the 48 sieve.

B. DRAINFIELD DESIGN: For ISF Effluent
a. Percolation Rate: 5.3 - 6.0 minutes/inch

C. SURFACE WATERS: There is no surface water within a 100 foot of the proposed septic
system upgrade.
D. TOPOGRAPHY: As can be seen on the attached topography site plan, the slope of the lot
runs from northeast to southwest and is quite steep in many places. We will have fill brought in
to eliminate all slopes greater than 25% within 35 feet of the proposed trench. We request that
your office grant a separation distance waiver of 35 feet from the proposed drainfield to slopes of
25% or greater. Given the porosity of the receiving soils, and the quality of the effluent (highly
treated) it is reasonable to assume that the wastewater will migrate downward rather rapidly, with
very little lateral migration. In short, it is appears that there is minimal risk in granting the
requested waiver. The trench is to be installed parallel to slope contours.
E. MATERIALS AND CONSTRUCTION PRACTICES: We will use a conventional lift
station (Anchorage Tank), equipped with a programmable timer so that flow can be
intermittently dosed to the ISF The materials used, and the construction practices will comply
with DHHS' "Intermittent Sand Filter Design, Installation & Maintenance Manual". The
contractor should read this document prior to construction. Copies are available at the Municipal
Onsite Services office (5th floor, 9th & L St.).
F. CLOSING: I am open to any suggestions from your department, which would be an
improvement to the proposed design. I am unaware of any adverse impacts this installation
would have on adjacent wells or septic systems. If you have any questions, please contact meat

Dear Mr. Bandt:
Recently I was asked to design a septic system for Spendlove Heights
Subdivision, Block 3, Lot 11. There is presently an old crib on the lot
which has failed. The homeowner wants to sell the 4 bedroom house and
has put money in escrow. The proposed septic site once had slopes of 40
to 508. Fill has been placed on it to level it for a driveway and
parking space. A test hole was dug on May 25, 1984. The soil consisted
of 6 feet fill, 1 foot peat, and silty gravel down to bedrock. The
bedrock depth varies from 10 to 12 feet in this location. On the east
side of the 351 x 401 septic area, the bedrock is at 3 foot depth.
Because of the requirement to provide 6 foot distance between the
bedrock and the bottom of the leaching gravel, I would have to design a
system that lies in the fill area. Furthermore, it will have to be at
varying depths to fully utilize the site. I propose to design a shallow
trench system. I would like to excavate below peat level, backfill with
sand untill 6 foot above bedrock and design 5 foot wide shallow trenchs.
Would you approve of this. If not, what do your recommend?

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the AGREEMENT" made and
entered into as of this Day of A of 201 , by and between

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).
Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
Owner acknowledges that the fine for failing to maintain and repair an AWWTS maybe
assessed in accordance with AMC 14.60.030.
Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
Z__ Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On -Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
AOwner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

coated steel septic tank is 20 years. Typical replacement costs are $15,000 or more,

Steel septic tanks utilizing "Coal Tar Pitch" coatings are subject to corrosion as the
steel reacts to water and rusts. These tanks have a finite lifespan buried in the

The septic system located at Lot 11 block 3 Spendlove View
Heights was constructed during the summer of 1969. The actual
construction was done by the then owner, Calvin Bourn, under my
my supervision. Although no permits were required at that time by
the City of Glenn Alps, the system was constructed to and
exceeding the codes at that time. The septic tank was
constructed of concrete blocks (on top of and covered by a
concrete slab) measuring 4' x 6' x 101.' Cast iron drain tile
lead to a 1000 gallon leach tank constructed by Greer Tank.for
that purpose.
