5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system.- All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.

I am writing to request that a Waiver be granted for this property. PES installed cleanouts on the existing drain field
April 6, 2015. The system appears to conform to the design drawings. This system is operating adequately for a
three bedroom system. The system has a minor code deficiency that need to be addressed. These deficiencies are the
subject of this waiver request.
1. Waiver Request. I request the following waivers:
a. Drain field to Property line, from ten feet to eight feet.
The drain field is a bed design and was installed in 1991. Based upon existing records the bed is 8.3 feet
from the northern property line.
The surrounding lots are developed. The existing septic systems are located greater than ten feet from the
property lines. Granting this waiver will not affect the operations or availability of the surrounding lots to
develop or install an on-site sewer disposal system.
The proposed waiver will not affect the future development of the surrounding or existing lots nor endanger the
health and safety of the public. If you have any questions or concerns, please contact me at 272-8218.

Subject : T12N, R3W, Section 33, Lot 57, Septic System Upgrade
This information amends the application submitted on September 14, 1990 for a
septic system upgrade on the above referenced lot. Per your suggestion, we have
conducted more extensive soils investigations on the filled portion of the lot.
Test hole #4 was excavated through the fill material to a total depth of 13' below the
top of the fill. As you can see from the soils logs, we found that the fill was
unsuitable as an accepting stratum, and we have revised the planned upgrade
accordingly. Please note the following changes:
• Although Mr. Smith (the homeowner) has stated that we excavated through the
least desirable portion of the fill, and that the other areas contain suitable
material, we are assuming that the entire filled portion is unacceptable. We
found what appeared to be overburden that had been stripped from another
area, including organic material, silty clays, and peat. The new design therefore
calls for removal of all material down to the elevation of the silty sand, and
then using a levelling course of compacted clean sand. The bottom of the
gravel bed will be a minimum of 6' above the elevation of the water levels in
Test Holes 1, 2, and 3.
• Because the thickness of the silty sand layer varies, it is unlikely that a wide
drainfield will have a consistent wall area within the silty sand layer. We
therefore have chosen a bed as being more suitable. The bed will be 15' wide,
60' long.
• Carl and I have decided that relocation of the existing culvert completely out
of the system area is feasible, and this lay -out would likely provide a better
long-term solution.

The original packet I submitted lacked information on how much fill was now
placed over Test Hole #J1. There currently is 3.1' of fill at that location,
however we are now proposing only to use the fill for cover material and
grading.
