All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the. same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
• The tank is to be protected with a permanent barrier to prevent vehicles from parking or driving over the
tank. The record drawing is to show and label the barrier.

RE: SEPTIC TANK UPGRADE PERMIT
LEGAL: PROSPECT HEIGHTS #1 BLOCK 5, LOT 12A
To Whom It May Concern:
I am writing to request a permit to install a new septic tank for the residence located at Prospect Heights
#1 Block 5, Lot 12A. This proposed system upgrade will replace the existing septic tank that services the
properties three-bedroom residence. The existing 1,000 -gallon tank appears to be compromised. The
proposed replacement tank will be 1,250 -gallons, which exceeds the minimum 1,000 -gallon tank size
required for a three-bedroom residence; however, this will not change the permitted usage of the
residence at this time. Effluent from the proposed tank will tie into the existing absorption field.
The property has an existing, private water well located on the western side of the lot, over 100 feet from
the proposed septic tank location. The surrounding lots are also serviced by private water systems, which
are all over 100 feet from the proposed tank location. The proposed design will not impact the
aforementioned properties septic or water systems nor will it conflict with any neighboring water wells
or wastewater disposal systems.
Per the Municipality of Anchorage (MOA) Wetlands Mapping database, there is an "open channel' within
the vicinity of the tank as illustrated on the drawing. However, as defined by AMC 15.65, this natural
depression, or open channel, does not contain surface water nor is it cataloged as a wetland area. The
installation of this proposed septic tank complies with MOA codes and the drainage flowing onto and off
the property will not adversely affect the performance of the proposed wastewater system, or
neighboring systems. The proposed septic tank will not change the general slope of the area nor will
ponding and/or concentration of surface runoff result from this installation.

Gentlemen:
I am writing to request a lot line waiver be granted for the above referenced property.
The drain field needed to be moved to the north due the presence of a gas line and
electric line in the upgrade location. The new drain field is located 0.5 feet off the north
property line. The neighboring septic system is located greater than 50 feet away.
Granting this waiver will not affect the upgrade of the neighboring system to the north
when it is necessary.

Septic System Upgrade Permit Request
Gentlemen:
I am writing to request a permit to construct an upgraded septic system be issued for
this property. The proposed systems will serve an existing three-bedroom house. The
existing well is located in the western side of the lot, over 100 feet from the proposed
septic system location. The surrounding lots are served by private water systems and
are over 100 feet from the proposed soil absorption system.
1. Soils. See the attached soils log. Ground water was monitored for seven days
through the May high ground water period. Ground water was encountered in the test
hole at a depth of eleven feet after the water -monitoring period. Bedrock was not
encountered in the test hole. It is my opinion that the overall soils appearance of the
soils, an application rate of 0.6 gallons/day/square feet should be used using a
conventional treatment system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
The proposed systems will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The average topography in the area of the proposed septic system is
approximately 10 percent. The proposed system will be installed on the northern portion
of the lot adjacent to the north property line.
The proposed installation will not affect the future development of the surrounding or
existing lots. There are no wells or septic systems within 100 feet of the proposed septic
location. The existing septic tank is constructed of concrete. It will be exposed and the
integrity of the tank verified.

in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are)in compliance with all applicable Municipal and State codes, ordinances,and regulations in effect at
the time of installation.
In conducting an adequacy test,I attempt to provide a thorough,conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations.The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition,ground water levels that may fluctuate during the year,and the water usage of the family being served by the system.These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system,nor do they guarantee that there are no hidden defects or encroachments.Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed

Tank in Driveway
I am writing to document the tank in the driveway at the above property. The tank was installed in June of
1977 and is constructed on reinforced concrete. The lid is six inches thick (verified recently when the
baffle was inspected and repaired) and is in good shape with no signs of cracking or deterioration. The
tank has 80 inches of cover, which will dissipate any wheel loading to virtually nothing (doing the math: 80
inches extended out at a 1:1 will produce an effective loading area of160 square inches per inch of load. If
the load is 150 psi, the effective load on the tank is less than 1 pound. ) This concrete tank has been in
service for over forty years. It is structurally sound and will continue to support wheel loadings under this
driveway for years to come.
