Box 196650 Anchorage, AK 99519-6650 RE: US Survey 3042, Lot 74C 351 Toadstool Dr., Girdwood Proposed Septic System Upgrade To Whom It May Concern: Attached please find the application and supporting documentation to upgrade the septic system for the subject property. The existing 2 bedroom house was built in 1969, and is currently served by a crib. A test hole dug on August 20, 2008 showed that the crib encroaches on the groundwater, so it will be abandoned in place per code. A new drainfield and lift station are proposed. The proposed design is based upon a test hole dug on August 20, 2008. The hole was dug to 12 ft, with water entering at 8.75 ft. The high water level was 8.6 ft on September 10, 2008. The test hole encountered silty sand overlaying clean sand. A percolation test conducted between 3.5 ft and 4.0 ft resulted in a rate of 3.6 minutes per inch. An application rate of 1.2 gpd/ft2 is used in the design. The soil log is attached. The lot is served by a Class C well; the septic system will be greater than 150 ft from it and greater than 100 ft to an adjacent private well. The proposed tank and drainfield are to be placed at the foot of a hill with slope of 25%. To the west, the slope ranges from 2 to 5%. Therefore, there are no slope concerns. The lift station will be installed near the existing driveway, with ballards to prevent driving over the tank. There is adequate space to re-route the driveway further west, after the crib is abandoned. A waiver is requested to allow the drainfield to be placed less than 10 ft from the foundations of the garage and the house. Both buildings are built on a concrete slab, so any potential softening of soils due to septic effluent should not compromise the foundations and vapors will not be likely to enter the buildings. The ends of the drainfield will be facing the foundations; the drainfield is not parallel to the buildings. There is insufficient room to move the drainfield, due to an electric line and the well radii to the east, and surface waters to the west. I am not aware of any adverse effect that this upgrade would have on any adjacent properties' water and wastewater treatment needs. Thank you for consideration of this permit application. Certificates of On -Site Systems Approval are required for the transfer of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work. 4.
