Box 196650 Anchorage, AK 99519-6650 RE: South Hills Blk 5 Lot 6A Proposed Septic System Upgrade To Whom It May Concern: Attached please find the application and supporting documentation to upgrade the septic system for the referenced 5 bedroom house. The house is currently served by a holding tank. A detached garage is also served by a holding tank. It is proposed to abandon the tanks in place. It is proposed to Install a new AdvanTex AX -20 with a pump basin and (2) 5 -wide drainfields. Both a primary and a secondary drainfield will be installed and will be alternated. The mounds will be covered with topsoil and seed. The proposed design is based upon a test hole dug on May 19, 2008. There was groundwater influx at 4.5 ft when the hole was dug, and it was determined to be 5.0 ft below grade after 7 days of monitoring. On October 2, the water level was at 6 ft. The maximum proposed depth is 0.5 ft. Soils will be excavated to the bottom of the organics and backfilled with clean filter sand to 0.5 ft. The test hole was dug to a total depth of 7.3 ft and encountered silty sand. A percolation test conducted between 1.5 ft and 2.0 ft resulted in a rate of 1.3 minutes per inch. A deeper perc test between 4.5 and 5.0 ft resulted in a rate of 1.2 minutes per inch. An application rate of 6.0 gpd/ff is used in the design. The soil log and perc test data are attached. Inc.; October 21, 2008; page 2 There is an abandoned well casing on the lot, and it will be decommissioned per code. It is proposed to tie in the house and both existing garages to the septic system. There are no living quarters in the garages. There is currently a bathroom in the southerly garage, and there are plans to install a bathroom in the northern one. There are no surface waters or private wells within 100 ft of the proposed septic system. The drainfields will be located more than 50 ft from the road cutbank to the north. There are no other slope concerns. I am not aware of any adverse effect that this upgrade would have on any adjacent properties' water and wastewater treatment needs. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as AdvanTex AX -20 South Hills Blk 5 Lot 6A located at 7401 E. 140th Ave. . Anchorage, Alaska. 2. Definitions. A. Alteration. Any change to the design or function of an A W WTS that Includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Prior to performing any alterations to an A W WTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On -Site Systems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. D. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. E. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an AWWTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of zero (b U .00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Terra. The term of this Memorandum of Understanding shall be for the life of the AWWTS. The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On -Site approval is issued to the new owner or transferee of the property. 5. Alterations, Installation and Removal of Additional Equipment. Owner agrees not to make any alterations, removal of parts or additions to the AW WTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AW WTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AW WTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's A WWTS Operating Permit is: annual C. Owner acknowledges that the fine schedule for failing to maintain and repair an AW WTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. E. Owner agrees to grant the Municipality reasonable access to test and inspect the A W WTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for AWWTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AW WTS. 7. Nonwaiver. The failure of either party at any time to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 8. Amendment. A. This Memorandum of Understanding shall only be amended, modificd or changed by a writing, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this contract, the only authorized representatives of the parties are: Owner. Michael or Beverly Blakeslee Anchorage: Purchasing Officer C. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 9. Jurisdiction: Choice of l sw. Any civil action arising from this Memorandum of Understanding shall be brought in the Superior Court for the Third Judicial District of the State of Alaska at Anchorage. The laws of the State of Alaska shall govem the rights and obligations of the parties under this Memorandum of Understanding. 10. Severability. Any provisions of this Memorandum of Understanding decreed invalid by a court of competent jurisdiction shall not invalidate the remaining provisions of the Memorandum of Understanding. During the installation, it was found that the existing 1500 gallon septic tank could not he used (as a holding tank), and had to be abandoned (crushed and filled). In addition, it was discovered that the garage was served by an another septic tank (near the northeast corner of the garage) and then discharged to the undocumented drainfield. In the garage there is a bathroom, stove, and kitchen sink, but no bedroom. In order to attain grade fi.om the garage to the new 4000 gallon holding tank, because of the burial rating on the tank, it would have been necessary to move the tank further to the north and rotate it 90 degrees. This would have resulted in the tank encroaching into the water service line setback (10 feet), or the natural gas service line. Moving the tank between the garage and the utility easement wouId have been to confining, and would likely have required the relocation of the electric and phone services line. Based upon these facts it seems that the best solution would be to put a small hotding tank at the northeast end of the garage (abandon the old, undocumented septic tank). Anchorage Tank, has indicated that they would be willing to construct a 500 gallon holding tank, complete with an alarm. The alarm will he mounted in the garage. Attached is a drawing which showSthe location of the proposed tank. 1 am unaware of any adverse impacts this installation would have on adjacent wells or septic systems. Numerous test holes have been dug on this lot by S&S Engineering, Toben Spurldand, P.E., and Alaska Water & Wastewater. Due to high groundwater, or impermeable soils, all of the test holes indicate the site is unsuitable for an onsite septic system. Attached are the logs for the test holes done by Alaska Water & Wastewater. Given the site restrictions it is my recommendation that your department allow us to install a 4000 gallon holding tank. If the existing 1500 gallon septic tank is still in good condition, it is the intent to connect the holding tank and septic tank in series, to provide a total volume of 5500 gallons. With the high groundwater conditions, it likely that there will be buoyancy forces acting on the holding tank when it is empty. The buoyancy force on a 4000 gallons tank is approximately 33,400 pounds. The resisting force is the soil cover (assume 2 feet of unsaturated soil) and the weight of the tank. The weight of the tank is about 3000 pounds, and the weight of the soil (assuming 90 lb./fi3) is approximately 19,300 pounds, for a total force of 22,300 pounds. Providing a factor of safety of 1.5, the total resisting force should be 50,100 pounds. I am proposing to place 3 feet of gravel fill over the tank area, and above the orynal grade, which will provide an additional resisting force of 36,465 pounds (assuming 110 lb./ri°). The total resisting force will then be 58,765 pounds, which will provide a factor of safety of 1.75. I am unaware of any adverse impacts this installation would have on adjacent wells or septic systems. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation. In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed above. The Municipality grants permission to the Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as located at (legal description) (Owner is required to read, understand and initial each section) `W Throughout the term of this Agreement, the Owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the Municipality or the manufacturer's representative. The AWWTS shall be maintained in a satisfactory condition capable of performing as designed and producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. 9 It shall be the responsibility of the Owner during the term of this Agreement to pay for all repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This includes an annual maintenance fee (typically $400 to $600). Owner agrees that only maintenance and repair personnel approved by the Municipality or the manufacturer's representative will inspect and make any necessary maintenance, repairs or permitted alterations to the system. Owner acknowledges that regular maintenance of an AWWTS reduces the potential failure of the system, which could include sewage backup and costly repairs or drainfield replacement. To whom it may concern: The subject lot has a 6 bedroom house on it which is served by a private well and two holding tanks. The house is served by a 4000 gallon tank, and the garage is served by a 500 gallon tank. Both holding tanks were installed just recently, and the as-built inspection report has been submitted to DHHS. Comments regarding the well are summarized as follows: WELL: The well is undocumented. Anchorage Pump and Well Service (Jim Ridgeway) inspected the well and determined it to be 168 feet deep, and cased to a depth of 31 feet. According to DHHS records, the well on Lot 10, Bk 5 (adjacent lot) is tapped into bedrock at a depth of 38 feet, and only has 38 feet of casing. Given this information, it is reasonable to assume that the well on the subject lot is also tapped into bedrock at a shallow depth (31 feet); however, it is not possible to determine this without mom sophisticated testing. According to Mr. Ridgeway, it appears that there is water weeping in at a depth of 31 feet (where the casing is seated into the bedrock?). This configuration has existed for approximately 15 years, and the well water has yet to be adversely eft~cted. Recent water sampling/analysis found nitrate levels to be approximately .4 mg/1, and 10.B (other bacteria) was encountered. On 7/12/97 a well adequacy test was performed. The test was limited by the fact that the pump was only set at a depth of approximately 30 feet. Prior to starting the test the static level was 14 feet BTC. Water was pumped at an average rate of .98 gpm for 265 minutes (260 gallons). At the end of the pumping period the water level had dropped 11 feet, down to 25 feet. Based upon this data, it is clear that the well will continually produce about 1 gpm. I believe a higher flow rate could have been achieved, had the pump been set at a deeper depth. Prior to the house being sold, the pump ~d~pflr should be set at a depth which will maximize the storage volume in the well casing, and allow for a greater drawdown. Based upon the aforementioned data, it was determined that the well is capable of producing greater than 900 gallons per day, as required for a 6 bedroom house.
