To whom it may concern:
Per the request of the owner, we have revised the septic system design for the referenced
property. An additional test hole with soils classification, groundwater monitoring and percolation
test was performed. Due to the new location of the garage/shop, we have moved the proposed
septic site west of the original proposed location. The drainfield will be centered around TH#3
and TH#2. We have monitored TH#3 through high groundwater season and have modified the
design accordingly. We request the sewer permit be amended to include these proposed
changes.

Ref: Proposed Well and Septic System for T15N, RIW, Section 5; N 1/3 of BLM Lot 33
To whom it may concern:
The proposed 5 bedroom house will be served by a private well and septic system. Two test
holes were excavated on the property. The proposed septic system will be designed around the
30 foot radii of these test holes. We are proposing that a 1500 gallon septic tank and a bed type
drainfield with 2 foot sand filter be installed. The proposed well location is shown on the
attached site plan. Comments regarding the proposed design are summarized as follows:
1. SOILS: Attached is a log which shows the soil classifications, groundwater monitoring, and
the percolation test results. Since the percolation rate for the test holes were found to be less
than 1 minute/inch, a sand filter is required and we have used a 0.7 gallons/day/ft2 application
rate for this design.
2. BED DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached design drawing, the area around the
drainfield is generally flat. In short there are no slope concerns.

engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the tr:ansfer
of title (except between spOuses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
an.y warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is fo.r

Arsenic Advisory
Certificate of On-Site Systems Approval # OSC 111089
A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Lot 32 and 33 N2
of T15N R1W Sec 5 Subdivision. This inspection revealed an arsenic
concentration of 11.6 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On-Site Water and Wastewater Program
website (www.muni.org/onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.
