Open and Close on the same day. B. Covered, sealed, and heated to prevent freezing. Special Provisions: Minimum 7 day water monitoring must be provided. Construction may proceed at your own risk before the 7 day monitoring is complete. Please submit stamped and signed results with the as-built inspection report. If the results require a design change, construction of the system will stop pending on-site review and approval. Anchorage, AK 99516) Gentlemen: Attached please find our application for approval to construct the planned on-site septic system to serve a proposed garage with living area. On July, 21, 2010 a perc test was performed at a depth of 6 feet and on August 27, 2010 a perc test was performed at a depth of 3 feet and a groundwater monitoring tube was installed at a depth of 13 feet. The soils log and percolation test results are attached. The percolation rate for the SP and SM soils encountered were measured at 1.1 and 24 minutes/inch, respectively (Area weighted average is less than 15 minutes/inch). The general slope of this lot is from northeast to southwest at a grade of approximately 2-10%. We have designed a deep trench system to serve the proposed garage based on the measured percolation rate and soils encountered. The lot also contains a two bedroom dwelling served by an individual onsite wastewater system and water well. There are no known public or private wells within 200' of the proposed system location except as noted. There is no surface water within 100' of the proposed system. On-Site Water and Wastewater Program dept. as to who will be installing the septic system for 3541 E 142nd Ave for the Marc Eld Residence. I Marc Eid owner of L26, Sec 33, T12NR3W SM; aka 3541 E. 142nd Ave will be on site through the entire septic installation process and will also be installing the system myself. I have many years experience operating earth moving equipment and have been involved in several septic construction projects in the MSB. I will have a close friend with 10 years experience installing systems in the MOA assisting me in the installation of this system as designed. I have rented a 312 Cat excavator from NC Machinery and will be having my rock delivered by Kathy of Angel trucking. The 1000 gal Poly tank has been purchased from Stephan Helms at Greer tank.
