Dear Jeff,
You communicated in your letter of February 23,2011 for additional information for the final
On-Site Wastewater Disposal System Inspection Report. You requested bottom of, original, and
final ground elevations of the test hole monitor tubes used for the installation of the above single
family residences wastewater disposal system (WWS).
Ground water elevations were monitored and logged per MOA regulations prior to installation of
the WWS. There was not any ground water infiltration into either of the test holes during the
monitoring stage as per our report. Additionally, the septic tank and the absorption trench were
installed at the maximum vertical elevation allowable (two (2) feet of soils cover with two (2)
inches of insulation installed.)
The wastewater installing contractor removed the monitor tubes after the monitoring of ground
water per permit documentation. This was done during the clearing and grubbing stage of
construction. Unfortunately, this was done before our personnel were informed and had the
opportunity to collect the test hole elevation data. The missing monitor tubes and final grading
negates the possibility of collecting and documenting the requested pre-construction elevation
datum.
All additional requests in your communication have been addressed and forwarded with this
letter.

Impacts to Adjacent Lots and Water Bodies
Dear On-Site Services Engineer:
The owner of Lot 3, Tract "F", Fire Lake Subdivision intends to construct a new well and a four-
bedroom wastewater system on the lot. We are therefore requesting a permit be issued for
the construction of a new well and septic system to accommodate the existing home. The
attached Site Plan and backup documentation identify the location and configuration of the
proposed septic system along with the alternate site
No existing wells or septic systems are closer than 100 feet from the proposed well or septic
system. The National Wetlands Inventory states there are no wetlands on Upper Fire Lake.
Therefore, there are no wetlands closer than 100 feet of the proposed wastewater system. The
drainage arrows are shown on the site plan showing the grade and direction of flow on the lot
to the North. The drainage pattern will not be changed as a result of the project.
The above lot East and West property lines, as well as the adjoining Park Lot to the West, are
inhabited by natural Birch, White Spruce, and Green Alder. The stream and lake edge are
populated with Wild Roses and other smaller natural indigence foliage cover. The center of the
lot is lawn grasses indigence to Alaska. The lawn is mowed and maintained.

1. A foundation cleanout shall be installed one to four feet from the building foundation.
Two deanouts are required between tank and the drain field on gravity flow systems.
2. Final grading over the tank shall be such that a positive slope exists away from the septic
tank.
DRAIN FIELD CONSTRUCTUION:
1. The drain field shall be constructed to the dimensions shown on the design. The bottom
of the trench shall be within 2 inches of level.
2. Distribution piping must be placed level with perforations down atop a level bed of
drain field rock. Rock should then be placed over the pipe to provide a minimum of 2
inches of cover.
3. A silt barrier or geo-textile fabric must be placed between the drain field rock and the
natural soil backfill.
4. Monitor tubes must be 4 inches in diameter and installed at then locations shown on
the design. The portion below the distribution line must be perforated. The portion
above the distribution line must be solid.
5. Contractor shall verify the septic tank and drain field are a minimum 100 feet away from
any private wells in the area, 150 feet from a Class "C" Well or 200 feet from a Class "A"
or "B" community well.
6. Direct bury insulation must be placed over the distribution system if less than 3 feet of
backfill depth is available. Finish grade over the trench must be mounted to prevent
settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum of 2 feet of natural unsaturated and 4' of accepting soil is required below
the drain field rock for a 5 foot wide trench. Contractor shall verify this condition prior
to placement of the rock. All pockets of unacceptable materials must be removed and
replaced.
MATERIAL SPECIFICATION:
1. Septic tanks must be constructed by a municipality approved septic tank manufacturer.
2. The following pipe materials are approved for use in septic system installations in the
Municipality of Anchorage:

If the system is constructed with our design the following statements apply:
1. The system, if constructed as designed, will have no adverse impact on the wells in the
area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing septic
systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve space,
either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
Subject: Lot 3, Tract "F", Fire Lake Subdivision
September 13, 2010
GENERAL:
1. The scope of this project includes the procurement and placement of a new (AWTS Class
III) 1,500 gallon Advantex septic tank and treatment system at the location shown on
the Site Plan. Work also includes the construction of a new 20' long X 5' wide X 3'
effective depth distribution trench (see attached calc sheet) at the location shown on
the Site Plan. The distribution line will be placed 2.0' below the existing ground surface.
Total depth of the trench will be 5.0' below existing ground surface. The new
absorption trench and septic tank must be placed a minimum of 100' from all private
wells and any surface water in the area as well as 10' from any water service line
proposed for the lot. A minimum of 3' of cover is required over the absorption trench
and 4' of cover over the septic tank.
2. Construction shall be in accordance with the approved site plan, design drawings,
Municipal Permit with any special provisions or conditions, and all applicable State and
Municipal Wastewater Disposal Regulations.
3. The contractor will be responsible for obtaining all underground utility locates and for
the layout of the septic system and verification of the location of all property lines and
easements.
4. Unless specifically agreed otherwise, the contractor shall be responsible for final grading
areas subsequently depressed from soil settling. Property owner shall be responsible
for vegetation of affected areas unless specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified by the Municipal
Development Services Department Building Safety Division Onsite Water and
Wastewater Program for system installation. Owners installing their own systems must
receive prior approval from the Onsite Water and Wastewater Program before
beginning installation.
SEPTIC TANK INSTALLATION:
1. A new (AWTS Class III) 1,500 gallon Advantex septic tank and treatment system must be
purchased from Anchorage Tank and Welding and installed at the location shown
2. A septic tank is to be constructed by a certified septic tank manufacturer. Construction
shall have two 4" cleanouts for pumping as well as vault lid for pump service.
3. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank.
4. Tanks installed without 4' of cover shall have a minimum of direct burial insulation.
5. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade.

1. A foundation cleanout shall be installed one to four feet from the building foundation.
Two cleanouts are required between tank and the drain field on gravity flow systems.
2. Final grading over the tank shall be such that a positive slope exists away from the septic
tank.
DRAIN FIELD CONSTRUCTION:
1. The drain field shall be constructed to the dimensions shown on the design. The bottom
of the trench shall be within 2 inches of level.
2. Distribution piping must be placed level with perforations down atop a level bed of
drain field rock. Rock should then be placed over the pipe to provide a minimum of 2
inches of cover.
3. A silt barrier or geo-textile fabric must be placed between the drain field rock and the
natural soil backfill.
4. Monitor tubes must be 4 inches in diameter and installed at then locations shown on
the design. The portion below the distribution line must be perforated. The portion
above the distribution line must be solid.
5. Contractor shall verify the septic tank and drain field are a minimum 100 feet away from
any private wells in the area, 150 feet from a Class "C" Well or 200 feet from a Class "A"
or "B" community well.
6. Direct bury insulation must be placed over the distribution system if less than 3 feet of
backfill depth is available. Finish grade over the trench must be mounted to prevent
settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum of 2 feet of natural unsaturated and 4' of accepting soil is required below
the drain field rock for a 5 foot wide trench. Contractor shall verify this condition prior
to placement of the rock. All pockets of unacceptable materials must be removed and
replaced.
MATERIAL SPECIFICATION:
1. Septic tanks must be constructed by a municipality approved septic tank manufacturer.
2. The following pipe materials are approved for use in septic system installations in the
Municipality of Anchorage:

Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and solid),
ASTM F810 or H.D.P.E. (perforated, but not solid), and ASTM D2662 or A.8.S.
(perforated and solid).
3. Insulation shall be at least 2 inches thick extruded direct burial polystyrene (Dow
Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Ferco, or
equal).
5. A permeable geo-textile fabric (Typar, Mirafi, or equal) must be installed between the
final drain rock layer and the native soil layer.
6. All drain rock shall be .5 inch to 2.5 inch in diameter with less than 3% passing the #200
sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These inspections must
be conducted under the supervision of a professional engineer registered in the State of
Alaska. The first inspection must be conducted after the excavation of trenches, beds,
or pits and before the installation of any gravel. A septic tank may be set in place, but
may not be backfilled.
The second inspection must be conducted after the placement of the geo-textile fabric,
gravel, distribution piping, standpipes, cleanouts, and insulation. No backfill should be
in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction notes for use
in preparing the certified as-built of the completed system.

ADVANCED WASTEWATER TREATMENT SYSTEM MAINTENANCE AND
REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN MUNICIPALITY OF ANCHORAGE
AND
THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this
herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as /'~ ~//~;t ~,-t -~/--G.~' located
2. Definitions.
A. Alteration. Any change to the design or function of an AWWTS that
Includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On-Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On-Site approval is issued to
the new owner or transferee of the property.
5. Alterations~ Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS
Operating Permit is:

C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.
