Certificate of On Site System Approval (COSA)
The following modifications have been made in accordance with your review
comments for the conditional COSA for the subject property:
1. The curtain drain was not installed. No groundwater or runoff water was
encountered during October of last year. A drainage swale was completed
along the eastern property line prior to the construction of the absorption bed.
This swale may be effective in controlling the surface water which may affect
the new bed. We intend to monitor the area during breakup of this year. It
surface water becomes a problem, the drain will be installed. The final approval
for the COSA will not be requested until after breakup. At that time we will
know whether the curtain drain is necessary.
2. The percolation test was required because the alternate site was not totally
within the 30' radius of a percolation test. The final as built shows both the
primary and alternate sites within the 30' radii of the percolation tests. No
further testing is therefore necessary. All percolation tests completed during
construction however indicate the soil is consistent throughout the absorption
area.
3. The buyer, Mr. Gunnar Benson, completed the Maintenance Agreement during
the permitting phase of the project.
4. The waiver to surface water request has been modified to show the current
status the absorption bed. It is attached to this memo.

Septic Tank and Absorption Bed to Surface Water Waiver
Dear Onsite Services Engineer:
Very little surface area is available for construction of a septic system on Lot 16A, Block
3H, Rabbit Creek Heights Subdivision. The new absorption bed was constructed in the
southeast corner of the lot which is located directly north of a flowing ditch on the south
side of Diane Drive. The southwest corner of the new absorption bed site is 54' from
the ditch. A waiver is therefore necessary from the 100' setback required by ordinance.
An Advantex Treatment System was recently constructed on the lot which provides
additional treatment to the septic effluent. In addition, the surface drainage path is
away from the ditch at a 7% grade. If the system should ever overflow the effluent
would follow the natural drainage path away from the ditch. The elevation of the ditch is
currently higher than the existing ground in the area of the new absorption bed.
The soils underlying the absorption bed are very good to excellent. The likelihood of a
failed septic system and overflowing effluent are very Iow. Should the situation occur
the flow is to the northwest away from the surface water. We therefore recommend
the waiver be issued allowing the septic system to remain in its constructed location at
54' from the surface drainage.

require a design change, construction of the system will stop pending On-Site review and

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 16A, Block 3H, Rabbit Creek Heights Subdivision intends to construct
a three-bedroom home on the lot. We are therefore requesting a permit be issued for a
well and septic system to serve the new home. The available area for the septic system
is limited by the presence of ground water close to the surface which make it difficult to
place a conventional septic system. We therefore propose to place an Advantex
Treatment System on the lot to provide the most effective treatment of the septic
effluent prior to discharge into the surrounding soil strata. The location of the
absorption system is shown on the attached Site Plan. A subdrain will also be placed to
control surface and subsurface drainage in the area of the new absorption bed.
Drainage patterns are also shown on the plan and will be modified somewhat as a
result of the proposed construction. The ditch on the south side of Diane Drive flows
nearly year round. A waiver request is attached to decrease the separation distance
from the ditch to the absorption bed to 72'.
Two test holes were placed on the lot by Watkins Engineering in November of 2004. A
third test hole was recently placed in the area of the Watkins Test Hole No. 2. Ground
water was found at a depth of 4' below the surface. The percolation rate in the
materials encountered was 9 minutes per inch as opposed to 4 minutes per inch on the
Watkins test. Our design provides for the bottom of the absorption bed to be at 2.5'
above the ground surface. We are therefore proposing to place a three-bedroom
Advantex Treatment System. The absorption bed will be 10' long x 15' wide for a total
of 150 square feet of absorption area. The distribution line in the trench will be placed
at 3.5' above the existing ground level. 2' of fill and 2" of insulation will be placed
above the bed and 4' over the septic tank to meet Municipal Code and prevent freezing.
The side slopes of the mound will be at 4:1 and extensive landscaping and fill will be
reqUired to insure positive drainage away from the finished mound.

The ground surface of the lot slopes from the southeast to the northwest at a 7% slope.
The bed will be constructed parallel with the contours of the slope as much as possible
in conformance with Municipal requirements. All components of the new septic
system will be constructed a minimum of 100' from the well proposed for this lot. No
conflicts exist between the proposed well and septic system and those in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

We reviewed your comments and offer the following response:
1. We measured the water levels in the monitor tubes yesterday, September 15,
2010, and found the water level in our Test Hole No. 3 at nearly 7'. Cindy Ellis'
Test Hole No. 2 was at 4'. We found more than 2' of native unsaturated soil and
invite your inspection to verify the levels. The previous summer has been
much wetter than past years and the water table is probably higher now than
typically found at this time of year. In addition, a ditch was installed in the recent
past along the east property line which may be assisting in lowering the water
table. The ditch is currently dry, but redirects surface runoff to the north and
away from the test hole area. We have revised our Site Plan to utilize the ditch
for the new subdrain planned for the lot. We therefore request that our
measurements be used for the evaluation of the septic system design and
issuance of the permit.
2. The calculations have been corrected on the soils log. The percolation rate
changes by slightly more than a minute and is actually less conservative than
the rate shown.
3. We are unsure of Cindy Ellis' water monitoring and prefer to rely on our data. As
previously mentioned the past summer has been much wetter than the summer
the test holes were placed. The water table today should therefore be higher
than the year Cindy Ellis performed her monitoring.
4. The maintenance agreement will be forwarded directly by Spinell Homes.
5. We have modified the Site Plan somewhat based on our last field inspection.
The modification resulted in moving the proposed absorption bed closer to the
open ditch to the south. The request for modification to surface water has been
change to 60'.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,

due to subsurface conditions that may not be observed from the surface, changes in land use,

THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this Day of

Lyndsay & Travis Hammond , herein the "OWNER," and the Municipality of
Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
4-1k Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
``t` `� It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
1 includes an annual maintenance fee (typically $400 to $600).
14�V Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
AAZ Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
L Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be
assessed in accordance with AMC 14:00,030.
Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
�j Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On -Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
�. Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B Wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
