Dear Onsite Services Engineer:
The owner of the subject property has requested a design for a standard gravity flow
septic system rather than the Nitrate Reducing System previously permitted. We are
therefore requesting the permit be revised to allow for placement of a 1,250 gallon
septic tank and absorption trenches in the configuration shown on the site plan. The
Site Plan shows the location of the new system and alternate site along with the
proposed location of the well. Also identified on the plans are the locations of the
adjacent lots with their respective wells and adjacent septic systems. Protective radii
are shown to the adjacent wells. Drainage arrows are also shown indicating the
current drainage patterns. The drainage patterns will be maintained after construction.
Several test holes were recently placed on the lot with varying results. The test hole
placed by Garness Engineering encompasses the majority of the new absorption
system. Their percolation rate indicates silty gravel with an absorption rate of 30
minutes per inch. An additional test hole was placed to 22' with slower percolation
results, but less than 60 minutes per inch. We are proposing to place a new 1,250
gallon septic tank and 2 - 34' long x 10' effective depth absorption trenches. Total
depth of the trenches will be 13'. The alternate site will continue to be a Nitrate
Reducing Category III System. A minimum of 3' of cover will be placed over the
absorption trenches and 4' over the septic tank.
The ground surface on the lot slopes slightly to the west in the area of the absorption
trench. The trench will be placed parallel to the contours of the ground as much as
possible and in conformance with design criteria. A steep bluff occurs on the lot

approximately 40' from the end of the trenches. The bluff slope will be flattened during
construction to minimize the slope to as close to 25% as possible. A minimum of 100'
will be maintained between all components of the septic system and the proposed well
and from any surface water in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

Absorption Trench to Slope Waiver
Dear Onsite Services Engineer:
The ground slope on the subject lot is relatively flat in the area where the primary
absorption trench will be constructed. Approximately 40' from the end of the slope
however the grade breaks at a 40% slope to the south at a grade approximating 38%.
We therefore request a slope waiver allowing the trenches to be placed at the planned
location. The 25% slope from 2' above the top of the absorption trench when extended
does not daylight. In addition, the 38% slope will be flattened during construction with
excessive excavated material to nearly 25%.
The alternate site is a Category III System and will be placed approximately 20' from
the slope. The 25% slope from 2' above the top of the absorption trench when
extended downward will not daylight. A cross section of the finished slope will be
submitted with the as built documentation of the constructed system to verify the slope
has been flattened and the separation distance has been maintained.

Dear O[~ Site Services Engineer:
The surface area to place a septic system on Lot 5, Block 6, Mountain Air Estates No. 1
is restricted by several protective well radii on this and the surrounding lots. A bluff at
the midpoint of the lot also restricts the available area. We are therefore requesting a
lot line waiver to allow the placement of the absorption trench within 1' of the west
property line. Soils at this location are gravel with silt and some sand which is normally
effective in the absorption and treatment of septic effluent. Placement of the absorption
trenches within 1' of the western lot line will not adversely affect the neighboring lots.
We hereby recommend a lot line waiver be issued allowing placement of the absorption
system within 1' of the western lot line.

The Municipality of Anchorage Development Services Department (DsD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a priVate or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further vedfy that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable MuniciPal and State codes,
ordinances, and regulations in effect at the time of installation.
