To whom it may concern:
We request a well permit be issued for each referenced property. An application and the
MOA permit fee have been submitted for each property. As can be seen on the
attached site plan/design drawing, we are proposing that a .community wastewater
system be installed to serve these properties. A design package was submitted to
ADEC and on June 16, 2010, we received an "Approval to Construct" for the
wastewater system. We have attached a copy of the Approval to Construct.
Construction of the wastewater system started on June 17, 2010 and is progressing.
The contractor would like to drill all four wells sequentially so the well driller can remain
on-site until all the wells are completed. As can be seen on the attached site
plan/design drawing, there are no encroachments concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells
or septic systems. If you have any questions, please contact us at 337-6179. Thank

Operational Approval for a Community Wastewater Treatment and
Disposal System-ADEC Plan Tracking Number 8125
Dear Mr. Garness:
On December 15, 2010 the Department received documentation on the
construction, agreements and contact information along with the record
drawings for an advanced onlot wastewater treatment system. This system is a
new Quanics treatment system and a new soil absorption system that will
serve four (4)- three (3)-bedroom homes (total of 12 bedrooms served) at the
above location. In addition the existing single family source well has been
decommissioned to allow this wastewater system to be constructed at the site.
The new 5,500 gallon STEP tank will pump into four in parallel 650 Aerocell
units. The soil analysis on site was confirmed to be an SP and the soil
absorption field was built slightly larger than the approved design. (2 feet
longer and 2 feet wider)
The Department has reviewed the documentation and the record drawings for
the community onsite wastewater disposal and treatment system and per the
Wastewater Regulations 18 AAC 72.240, therefore Final Operational Approval
for this construction is hereby issued. A completed "Certificate to Construct"
for the community wastewater system is enclosed. Please use the referenced
plan number in correspondence regarding this system.
This approval does not imply the granting of additional authorizations nor
obligate any state, federal or local regulatory body to grant required
authorizations.
Any person who disagrees with this decision may request an adjudicatory
hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal
review by the Division Director in accordance with 18 AAC 15.185. Informal
review requests must be delivered to the Division Director, 555 Cordova
Street, Anchorage, Alaska 99501, within 15 days of receiving the decision.
Guidance information on the informal review process may be found at
http://www.dec.state.ak.us/commish/ReviewGuidance.htm. Adjudicatory
hearings requests must be delivered to the Commissioner of the Department
of Environmental Conservation, 410 Willoughby Avenue, Suite 303, P.O. Box

Dear Mr. Garness:
On June 14, 2010 the Department received an engineering plan submittal for
an onlot advanced wastewater treatment system with, a new Quanics
treatment system and a new soil absorption system that will serve four(4)-
three (3)-bedroom homes (total of 12 bedrooms served) at the above location.
In addition an existing single family source well must be properly
decommissioned to allow this system to be constructed at the site.
The new 5,500 gallon STEP tank will pump into 'four in parallel 650 Aerocell
units. The soft analysis on site determined 10 minutes per inch and 2.5
minutes per inch for field percolation ratings an onsite soil absorption system
using an application rate of 2 gallons per day per ft2. Thus for the 1,800
gallons per day/2 gallons per day per ft2 application rate a field of 900 ft2 is
required and has been sited for use on Lot 7 of this subdivision.
The Department has reviewed the engineering plans submitted for the
community onsite wastewater disposal and treatment system and per the
Wastewater Regulations 18 AAC 72.235 Conditional Approval to Construct the
proposed system is hereby issued. A "Certificate to Construct" for the
community wastewater system is enclosed. Please use the referenced plan
number in correspondence regarding this system.
This approval is contingent upon compliance w/th the following
conditions:

the design is met, please provide an engineer to inspect the site at the
time the soil absorption system is installed.

This information must be kept current at the local ADEC Office should
providers or individuals change contact (907) 269-7519 Bill Rieth.
Tests for Nitrate as nitrogen will need to be provided along with the
record drawings. Even though a nitrogen reducing system is being
installed, the nitrate level of 12.6 mg/1 found at lot 9, Block 6, Schroeder
East Subdivision is a concern and additional protection of Public Health
may need to be provided based upon sampling information.
If the applicant fails to construct, alter, install, or modify the system
within two (2) years after the date that the department issues an
approval to construct, the approval is void, and plans must be
resubmitted, [along with associated fees], for department review and
approval.
Deviations from approved plans which affect capacity, flow, operation,
major design of units, point of discharge, materials of major system
components (such as pipe, lagoon liners, etc.), or separation distances,
must be approved by DEC in writing prior to their implementation.
This approval is contingent upon compliance with the conditions of
Wastewater Disposal Regulations, 18 AAC 72.235, Construction
Certification. The noted section of the regulations requires that a
"Certification of Construction" be completed and submitted to the
Department within ninety (90) days of completion of construction. As-
built plans or record drawings should indicate any changes or deviations
from the approved plans to facilitate final review. A "Certification of
Construction" form is enclosed for your use.
This approval is contingent upon your receipt of any other state, federal
or local authorizations which are required for your project. You are
required to obtain al! other necessary authorizations before proceedhng
with your project.

Army Corps of Engineers and the Alaska Department of Natural
Resources may be required. A Coastal Projects Questionnaire will help

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal'and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by.a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the' local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
