The application has been reviewed and the following comments have been generated. These are to be satisfactorily
addressed prior to MOA approval:
1. Only the drainfield has been installed. A permit renewal will be required to complete the tank and well installation.
2. To close out the drainfield portion of the permit, a site plan and profile drawing is required to document the following:
a) Invert elevation of the beginning and end of the distribution pipe in relation to original grade.
b) Elevation of the bottom of test hole and water table in relation to original grade.
c) Construction of drainfield.
3. Enter number of bedrooms on IR form.
4. Since the well has not been drilled, please remove well to absorption field and sewer line separation distances entered
on IR form.
5. Since the foundation is not in, please remove the foundation to absorption field separation distance entered on the IR
form.
6. Gravel depth entered on IR form is 3.0 feet. Design had 0.5 feet. Please confirm.

Septic System Permit Request
Ladies and Gentlemen:
I am writing to request that a permit to construct a new well and septic system be issued for this property.
The proposed systems will serve a proposed three-bedroom house. The lot is not currently developed. The
proposed scope of work is to install a new 1,500 gallon S.T.E.P. tank with ~X20 podg auxiliary pump
vault, and absorption system along with a new private well that will serve this lot.
1. Soils. Two test holes were excavated by Gamess Engineering Group on January 11, 2010. See the
attached soil log. Ground water was monitored for seven days. Ground water was monitored to a depth of
7.5 feet below ground level in TH-1 after the monitoring period. Bedrock was not encountered in the test
holes. It is my opinion, based on the results of the percolation tests and overall soils appearance; an
application rate of 0.5 gallons/day/square feet should be used using an advanced wastewater system for the
primary and the reserve systems.

Soil Absorption System Design.
See Design Drawings sheet 2 of 3 for the design calculations.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and the
existing drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water
and drainage ditches.
4. Topography: The average topography in the area of the proposed septic system is approximately 10
percent. The area the septic system will be installed is approximately 15 percent. There are no steep slopes
within 50 feet of the proposed septic system.
5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and there are no further comments received from MoA On-Site Department, the
note will be removed and "Issued for Construction" drawings will be issued.

The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this

"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as Follows:
ADVANCED WASTEWATER TREAT: lENT SYSTEMS. Municipality

o.° Definitions.
A. Alteration. Any change to the design or function of an AWWTS that

included in the original construction permit and design. Prior
performing any alterations to an AWWTS the owner must obtain a

B. Ce~ificate of On-Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of propeay sale and title transfer in accordance wilh AMC 15.65.

they support and meet the codes that were in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the origina[ design which would allow the AWWTS m continue to
perfbm~ as designed.
E. Permit (Construction) An On-Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating) An Advanced Wastewatcr Treatment System
Operating Permit. An annual peri, it, issued by the Municipality. that
allows the Owner to operate an AWWTS. upon meeting all thc
requirements of th~s agreement, the conditions of Operating Permit, the
requirements of the On-Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. ~e_. Owner shall pay to Municipality an annual fee ot C
($ .00), payable on or befc~re the issuance of thc operating permit and annuall3
thereafter. The annuat fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Term. The term ot:this Memorandum of Understanding shall [~ Gr the life of the
AWWTS.
'lhe term begins o~ the dare of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On-Site approval is issued to
thc new owner or transferee of the property.
5~ Alterations, Instal!at:ion and Removal of Additional Equipment, Owner agrees
not to make any alterations, removal ol'parts or additions to the AWWTS without a
Construction Permit fi'om the Municipality.
6. Maintenance and Repairs.
A. Throughout the tem~ ot' this Memorandum of Understanding. the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance. (3) a4iustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply wilt all applicable ordinance, laws. regulations.
rules and orders f;t~r the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on thc system pursuant to the
terms and conditions contained in the Owner's AWW'FS Operating Permit. This
schedule shaft be submitted to thc Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS
Operating Pem~it is:

D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to thc system.

AWWTS upon 24 hours wri[ten notice.
F. Owner agrees that any sale or transfer of title of the properly will not occur
witl~out a new Certificate or On-Site Systems Approval,

8. Amemlment.
A. "[his Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by author[zed representatives of the parties, with the same
tbrmality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an ame~dment~
B. For the purposes of any amendment modification or change to the terms and
conditions oi'this contract, the only authorized representatives of the parties are:

unauthorized representative or tmauthorized means shall be void.
9. _Jurisdiction: Choice of Law. Any civil action arising from Ibis Memorandum
Understanding shall bc brought in the Superior Court fbr the Third Judicial District of the
State of Alaska at Anchorage. The taws. of the State of Ataska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. SeverabitiW, Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisi.ons
of the Memorandum of Understanding.
