3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions: A copy of the signed US Army Corps of Engineers construction completion
certification shall be submitted with the final inspection report.

Ref: Proposed Well and Septic System for Pioneer Estates S/D; Lot 9, Block 2,
To whom it may concern:
The existing 1 bedroom cabin is served by an outhouse. We are proposing that a well
be drilled in the area shown on the attached site plan and design drawing. We are also
proposing that a 1250 gallon S.T.E.P. tank and 5-wide trench type drainfield be
installed. Attached are a site plan and a design drawing showing the proposed septic
system area. Two test holes were excavated on the property. The drainfield will be
designed around the 30 foot radius of both test holes. As can be seen on the attached
site plan, there are several surface waters and streams on the property. In order to
install the septic system, we will be required to cross the stream. We have contacted the
MOA watershed department and they have mapped the surface water and are aware of
the situation. We are currently in the process of obtaining approval from the appropriate
agency (MOA watershed or Corp. of Engineers) in order to cross the stream with the
sewer line and equipment/materials. We are proposing to installs culverts in order to
make the crossings. Comments regarding the design are summarized as follows:
1. SOILS: See the attached logs which show the soil classifications, groundwater
monitoring, and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: The surface waters are shown on the attached site plan and
design drawing. We are requiring that the 100 foot setback to all surface waters be
flagged/staked by a registered land surveyor prior to construction. The proposed
drainfield and tank will be a minimum of 100'setback from all surface waters.
4. TOPOGRAPHY: As can be seen on the attached site plan, the average topography
below the drainfield is 10-15 percent running northeast to southwest. In short, there are
no slope concerns.

5. LOT LINE WAIVER: Due to site constraints, we are requesting a 1 foot lot line
waiver from the east lot line to the proposed drainfield and alternate site.

Dear Mr. Jenkins and MS. Williams:
This is in reference to your proposal to install two culverts on a
tributary of the Eklutna River as part of a well-septic system on your
property; which will require the discharge of 33 cubic yards of fill material
below the stream Ordinary High Water Mark (0.0046-acre)o In addition, in
2009 you placed 133 cubic yards of fill material in 0~043-acre of palustrine
wetlands for the construction of a driveway. It has been assigned file
number POA-2010-533, Eklutna River, which should be referred to in all future
correspondence with this office. The project site is located within Section
6, T~ 15 N., R. 2 E., Seward Meridian; USGS Quad Map Anchorage B-6; Latitude
61.4161° N., Longitude -149.1720° W.; ~i_oneer. Estates, Block 2, Lot 9.[ on
Raymond Avenue, along the Eklutna Road, Chugiak, Alaska.
DA permit authorization is necessary because your project involves
placement of structures and fill material into waters of the U.S., including
wetlands, under our regulatory jurisdiction.
Based upon the information and plans you provided, we hereby verify that
the work described above, which will be performed in accordance with the
enclosed plan (sheets 1-2), dated October 7, 2010, is authorized by
Nationwide Permit (NWP) No. 29, Residential Development. This verification
also authorizes the fill associated with the driveway construction after-the-
fact. NWP No. 29 and its associated Regional and General Conditions cab be
accessed at our website at www.poa.usace.army.mil/reg. Regional Conditions
D, E, and F apply to your project. You must comply with all terms and
conditions associated with NWP No. 29, as well.
Further, please note General Condition 26 requires that you submit a
signed certification to us once any work and required mitigation are
completed. Enclosed is tNe form for you to complete and return to us.
This verification is valid until the NWPs are modified, reissued, or
revoked. Ail. of the existing NWPs are scheduled to be modified, reissued, or
revoked prior to March 18, 2012. it is incumbent upon you to remain informed
of changes to the NWPs. We will issue a public notice when the NWPs are
reissued. Furthermore, if you commence or are under contract to commence
this activity before the date that the relevant nationwide permits are
modified or revoked, you will have twelve (12) months from the date of the
modification or revocation of the NWPs to '~omplete the activity under the
present terms and conditions of these nationwide permits.
